Primary infection agents occurring during pregnancy and causing congenital anomalies are Toxoplasma gondii (T.gondii), rubella virus and cytomegalovirus (CMV). It was revealed that these agents infected the fetus by crossing the placenta and increased the rate of fetal morbidity and mortality. This study aimed to investigate the relationship between seroprevalences of T. gondii, rubella virus and CMV antibodies. Methods:The results were retrospectively evaluated. Pregnant women's serum samples sent to the Medical Microbiology Laboratory of Ankara Gülhane Training and Research Hospital between January 2018 and December 2018 were included in this study. T. gondii, rubella virus and CMV IgM and IgG antibodies were analysed using the chemiluminescent immunoassay method.Results: In this study, the results of 647 pregnant patients were retrospectively analysed. The median age of the attendants was 28.0 (18.0-49.0) years. IgM positivities for T. gondii, rubella virus and CMV were found as 3 (0.6%), 3 (0.5%) and 61 (15.0%) respectively and IgG positivities were 66 (13.8%), 529 (91.7%) and 394 (99.3%), respectively. Conclusion:In this study, the results were consistent with the findings obtained in the regional studies. Seroprevalence studies in pregnant women will be guide for the necessity of prenatal screening tests. Therefore, the findings suggest that pregnant women and women of childbearing age should be investigated concerning T. gondii, the rubella virus and CMV antibodies.
Vulvovajinal enfeksiyonlara sebep olan en önemli mikrobiyal etkenler Candida türleri, Trichomonas vaginalis, Gardnerella vaginalis, enterobakteriler, stafilokoklar, enterokoklar ve B grubu streptokoklardır. Bu çalışmanın amacı vulvovajinal enfeksiyonların tanısında kullanılan yöntemleri karşılaştırarak, enfeksiyon prevalansını ve predispozan faktörlerin etkilerini belirlemektir. Gereç ve Yöntemler: Bu çalışmaya yaşları 18-60 arasında değişen 300 kadın hasta dahil edildi. T. vaginalis, bakteriyel vajinoz ve Candida türleri tanıları için direkt bakı, Gram ve Giemsa boya, kültür yöntemleri kullanıldı. Bulgular: Hastaların %35,3'ünde bakteriyel vajinoz, %28,3'ünde T. vaginalis, %28'inde Candida türleri saptandı. Bakteriyel vajinoz için Gram boya yöntemi referans test olarak alındığında direkt bakının duyarlılığı %72,6, T. vaginalis için direkt bakı yöntemi referans test olarak alındığında Gram boyanın duyarlılığı %95,3; Giemsa boyanın duyarlılığı %68,2, Candida türleri için kültür yöntemi referans test olarak alındığında direkt bakının duyarlılığı %42,9; Gram boyanın duyarlılığı %60,7 olarak belirlenmiştir. Özgüllükleri ise tüm yöntemler için %100 olarak bulundu. C. albicans en fazla saptanan Candida türüydü. Korunma yöntemi kullanmamanın T. vaginalis enfeksiyonu için risk faktörü olduğu belirlendi. Sonuç: Vulvovajinal enfeksiyonların tanısında tanı yöntemlerinin duyarlılık ve özgüllükleri dikkate alındığında, hiçbir testin tek başına yeterli olmadığını düşünmekteyiz. Hastaya ait epidemiyolojik ve klinik bulguların yanı sıra, direkt bakı, Gram boya ve kültür yöntemlerinin bir arada uygulanmasının daha güvenilir sonuç vereceği kanısındayız. Comparison of Vulvovaginal Infection Diagnostic Methods and Effects of Predisposing Factors Vulvovaginal InfectionsABSTRACT Aim: The most important microbial factors that are caused vulvovaginal infections, are Candida species, Trichomonas vaginalis, Gardnerella vaginalis, enterobacteria, staphylococci, enterococci and group B streptococci. The aim of this study is to determine the prevalence of the infection and the effects of predisposing factors by comparing the methods used in the diagnosis of vulvovaginal infections. Material and Methods: A total of 300 female patients between the ages of 18-60 were included in this study. For the diagnosis of T. vaginalis, bacterial vaginosis and Candida species, direct examination, Gram and Giemsa stain, culture methods were used. Results: Bacterial vaginosis was found in 35.3%, T.vaginalis in 28.3%, Candida spp. in 28%. ÖzetAmaç: Üriner sistem infeksiyonları (ÜSİ), yetişkinlerde bakteriyel infeksiyonların en sık görülen sebebi olup, her iki cinste ve tüm yaş gruplarında görülebilmektedir. Bu çalışmada, ÜSİ'nin hızlı ve güvenilir tanısında, Gram boyaması, Thoma sayma kamarasında lökosit sayımı, nitrit testi ve lökosit esteraz (LE) testinin etkinliği, ÜSİ tanısında altın standard olan idrar kültürüyle karşılaştırılmıştır. Yöntemler: Mayıs-Kasım 2010 arasında polikliniklerden ya da yatan hastalardan gönderilen 658 idrar örneği incelenmiştir. Klinik bilgilerine ulaşılan, ÜSİ'yi düşündüren semptomları bulunan ve yaş aralığı 0-106 olan hastalar çalışmaya dahil edilmiştir. İdrar örnekleri, Gram boyaması, Thoma sayma kamarasında lökosit sayımı, nitrit testi ve LE testiyle incelenerek kültürleri yapılmıştır. Üreyen mikroorganizmalar klasik yöntemler ve API ® (bioMeriéux, Fransa) kullanılarak saptanmıştır. Bulgular: Örneklerin 137 (%20.8)'sinde üreme tespit edilmiş, 143 (%21.7)'ü kontaminasyon olarak değerlendirilmiştir. Gram boyamasının duyarlılığı %82.2, özgüllüğü %96.8, yalancı pozitifliği %3.2, yalancı negatifliği %17.8 olarak saptanmış olup, bu oranlar sırasıyla Thoma sayma kamarasında lökosit sayımı için, %78.8, %81.5, %18.5, %21.2; nitrit testi için, %40.1, %95.8, %4.2, %59.9; LE testi için, %88.1, %34.1, %65.9, %11.9 olarak tespit edilmiştir. Sonuçlar: Gram boyamasının, duyarlılık ve özgüllüğünün oldukça yüksek, yalancı pozitif ve yalancı negatifliklerinin diğer yön-temlere oranla düşük ve uygulanabilirliğinin kolay olması nedeniyle, ÜSİ'nin hızlı tanısında kullanılabilecek en uygun yöntem olduğu sonucuna varılmıştır. AbstractObjective: Urinary tract infections are the most frequent bacterial infections in adults and may be seen in all age groups and both genders. In this study, urine culture which is the gold standard diagnostic method of urinary tract infections is compared with Gram stain, Thoma counting chamber, nitrite reductase test and leukocyte esterase (LE) test in efficiency for rapid and reliable diagnosis of urinary tract infection. Methods: This study was performed in our laboratory between May 2010 and November 2010. 658 urine samples of patients with urinary tract infection symptoms from outpatient and inpatient clinics were included in the study. Urine samples were examined by Gram stain, leukocyte count in Thoma chamber, nitrit test and LE test, and cultured. Microorganisms were identified using conventional methods and API ® (bioMeriéux, Fransa) identification systems. Results: Cultures were found positive in 137 (20.8%) samples and 143 (21.7%) cultures were evaluated as contamination. For Gram stain, the results for sensitivity, specificity, false positivity and false negativity were 82.2%, 96.8%, 3.2%, and 17.8%, respectively.
