Objectives. The use of thin skin flaps in the reconstruction of postburn cervical contractures associated with functional impacts. Material and Methods. We conducted a descriptive observational study on about five patients who had reconstruction of postburn cervical contractures using various thin skin flaps in a plastic surgery department. The follow-up period was ranged from 12 to 18 months. We determined the characteristics of the retraction according to the Vandenbussche classification, the indications of the flaps, and the functional and aesthetic results. Results. Most of our patients were young adults presenting segmental retraction of the neck associated with moderate functional impact. The occipito-cervico-dorsal flap was used in three cases. The occipito-cervico-shoulder flap was used in two cases. The aesthetic and functional results were satisfactory for all patients, but a “neck-collar” aspect of the cervical-chin angle was noticed. Conclusion. Postburn cervical contractures remain frequent. Their reconstruction is a difficult process as it is ideally done with a skin similar to the original one. The use of thin skin flaps seems to be an excellent option.
Nous rapportons à travers une étude rétrospective faite de 35 cas d’exostose sous unguéale de l’hallux dite exostose de Turrett's colligée entre 1995 et 2015 au service d’Orthopédie Traumatologie de l’Hôpital Militaire Principal d’Instruction de Tunis les résultats de la prise en charge de cette pathologie faite d’adultes jeunes d’âge moyen de 29 ans avec un sexe ratio de 1,7. Le délai moyen de consultation est de 18 mois. Ce retard de consultation est expliqué par une errance diagnostique en rapport avec une ressemblance du tableau clinique avec celui d’un ongle incarné. Le diagnostic est toujours confirmé par une radiographie de face et de profil de l’orteil intéressé. Le traitement a consisté en une exérèse radicale de l’exostose soit à travers une large fenêtre unguéale soit par un abord latéro-unguéal. L’examen anatomopathologique est systématique. Il a permis de confirmer la bénignité de l’affection dans tous les cas. La guérison a été de règle dans tous les cas avec une reprise de l’activité antérieure en 2 mois en moyenne. Aucune récidive n’a été notée.
Hydatid disease is prevalent throughout much of the world. However, bone and muscle echinococcosis are rare and often misdiagnosed. We report three cases: the first case, 38-year-old man had a primary sacral hydatid cyst, the second one, 37-year-old man, and the third one both had a primary muscle hydatidosis of the adductors of left thigh. The treatment was surgical for all three patients. Healing was obtained for the 2 muscular sites whereas the recurrence was the characteristic of the osseous localization despite several surgical resections.
