Purpose: We sought to evaluate the long-term functional outcomes and identify the potential risk factors for rebleeding in patients with brain stem cavernous malformations (BCMs) who presented with hemorrhages and were surgically or conservatively treated and prospectively monitored. Methods: From January 1990 to July 2015, we included patients with first hemorrhagic episodes secondary to single BCMs. Modified Rankin score (mRS) was used for neurological status assessment. Univariate and multivariate regression statistics were used to identify the risk factors for rebleeding. Results: A total of 99 patients with BCMs hemorrhages were included (59 [59.6%] women, mean age 37± 13 years). As initial treatments, 37 patients (37.4%) underwent surgery and 62 (62.6%) received conservative treatment. The median follow-up was 3.33 years (interquartile range 1.16-7 years; 408.3 patient/years). The rebleeding rate by patient/year was 10% in conservatively treated patients. Deterioration was significantly more frequent in patients with rebleeding (p = 0.0001). At the end of the follow-up, the mRS were favorable in 49 patients (65.3%) without rebleeding, whereas only 8 (33.3%) with rebleeding evolved to favorable outcomes (p = 0.006). Lesion size >18 mm (hazards ratio, HR 3.34, 95% CI 1.54-7.26; p = 0.0001) and ventral location or crossing the brain stem's midpoint (HR 2.5, 95% CI 1.14-5.46; p = 0.022) were associated with a major risk of rebleeding in the univariate analysis, but only a lesion >18 mm remained statistically significant (HR 2.7, 95% CI 1.2-6.21; p = 0.016) in the multivariate analysis. Conclusion: A lesion size >18 mm was the principal factor associated with hemorrhage recurrence. The overall functional outcome was good. However, significant morbidity was attributable to rebleeding.
Intracranial aneurysms uncommonly present with ischemic stroke. Parent artery occlusion due to local extension of the luminal thrombus, aneurysms ejecting emboli to distal arteries, or increased mass effect have been described as possible pathogenic mechanisms. Guidelines for the management of these patients are absent. We present the clinical outcome and radiological characteristics of three patients with spontaneous thrombosis of intracranial aneurysms as a cause of ischemic stroke. This information is relevant given the possible benign history in terms of stroke recurrence and risk of bleeding.
Resumen El Drip and Ship consiste en administrar trombolisis endovenosa en centro primario y remisión para trombectomía mecánica. Objetivo: Describir la experiencia Drip and Ship como intervención del ictus isquémico en dos regiones de Colombia. Métodos: Serie de casos retrospectiva de pacientes tratados con Drip and Ship durante 2019 y primer cuartil de 2021. Se compararon variables como puerta-imagen, puerta-aguja, puerta-ingle y desenlaces clínicos. Se calcularon medidas de tendencia central y estadísticas descriptivas. Resultados: Se registraron seis pacientes con ictus isquémico y oclusión proximal. Mediana de 60 minutos de inicio de síntomas, puerta-aguja 75 minutos, tiempo door-in door-out 167.5 minutos, tiempo de traslado 91.5 minutos para remisión desde otra ciudad. Cinco pacientes recibieron trombectomía mecánica con TICI 2c-3. 50% de los pacientes con Rankin modificado favorable al egreso y 84% a tres meses. Conclusiones: En el registro RES-Q entre 2019 y el primer cuartil de 2021, se consignaron 5954 casos de ictus isquémico en Colombia, 20 pacientes tratados con Drip and Ship (0,33%). Los tiempos de puerta- aguja y de remisión fueron mayores a lo recomendado en guías internacionales, siendo mayor en los pacientes provenientes de otras ciudades. Se requiere estructurar redes de atención para mejorar tiempos de tratamiento y número de pacientes tratados.
INTRODUCCIÓN.
El tiempo es un factor importante en el tratamiento del ACV agudo, con la pandemia de covid-19 se puede ver afectada la adherencia a las guías. Se han reportado tasas de ACV en presencia de covid-19 alrededor del 5%, además un aumento en el tiempo de consulta de los pacientes en otras regiones del mundo y una mayor ocupación de camas de UCI para patologías respiratorias que usualmente se destinan a pacientes con ACV.
Este documento recoge las recomendaciones del comité de Enfermedad Cerebrovascular de la Asociación Colombiana de Neurología basados en la literatura y votadas por consenso.
OBJETIVOS.
Proporcionar una guía rápida adaptada a los diferentes escenarios de atención de ACV en Colombia, divididos en centro listo, primario y avanzado incluyendo una ruta de atención para paciente con sospecha de covid-19 denominada Código ACV protegido, que incluye el uso de elementos de protección personal para pacientes y personal de salud.
PUNTOS PRINCIPALES.
Dentro de las recomendaciones principales está el cuestionario de tamizaje al ingreso a urgencias, uso de tapabocas para el paciente, considerar todo Código ACV sospechoso de covid-19, toma de PCR para covid-19 en todos los ACV y llevar a zona buffer. En sospecha de oclusión proximal incluir tomo-grafía de tórax a la evaluación de imágenes.
CONCLUSIONES.
Establecer pautas de atención durante la pandemia covid-19 permite optimizar protección para pacientes y personal de salud, disminuir retrasos y tratar de evitar que pacientes no reciban el tratamiento adecuado en los diferentes niveles de atención del sistema de salud.
