Artículo originAlResumen El objetivo del presente estudio in vitro fue evaluar la fuerza de adhesión, de un sistema adhesivo, a la superficie de esmalte blanqueado con peroxido de carbamida al 10% con fluor y sin fluor incorporado. Con este fin se emplearon nueve terceras molares sanas, las cuales fueron divididas en tres grupos: grupo I: blanqueado con Opalescence al 10% PF (con Fluor), grupo II: blanqueado con Opalescence 10% (sin fluor), grupo III: grupo control sin blanqueamiento. Los grupos de blanqueamiento pasaron por un periodo de blanqueamiento de 6 horas por día a una temperatura de 37°C durante 14 días consecutivos y fueron almacenados en saliva artificial. Se esperó 7 días para la colocación de un bloque de resina (Filtek Z350, 3M) de 5 x 5 x 3 mm. (anchura, longitud, altura) colocada en la superficie bucal, palatina o lingual de cada diente, previamente se uso ácido fosforico al 35% (Scotchbond, 3M) y el Sistema Adhesivo (Single Bond 2, 3M). Luego mediante el uso de un disco diamantado biactivo se realizaron cortes horizontales y verticales para obtener los especimenes, los cuales fueron almacenados en agua bidestilada. Luego de 24 horas los especimenes fueron examinados con el método de microtensiometro a 0,5 mm/min obteniéndose los valores de fuerza de adhesión. Los datos fueron analizados con el test ANOVA y post test de Tukey para determinar las diferencias significativas entre los grupos experimentales (p<0.05). Los resultados obtenidos fueron: grupo I = 28.47 ± 5.89 MPa, grupo II = 24.51 ± 7.96 MPa, y grupo control = 30.51 ± 7.48 MPa. La fuerza de adhesión del sistema adhesivo en el grupo con superficie, de esmalte blanqueado con peroxido de carbamida 10% y con 0.11% de ion fluoruro fue significativamente mayor que en el grupo cuya superficie del esmalte fue blanqueado con peroxido de carbamida 10% sin fluor incorporado. Se concluyó que bajo las condiciones de éste estudio la incorporación del ion fluor al 0.11% en el peroxido de carbamida 10% (Opalescente), preserva la fuerza de adhesión a esmalte, en comparación con el peroxido de carbamida 10% que no contiene fluor en su composición (Opalescente PF) después de un régimen de blanqueamiento. Rehabilitación oral con prótesis fijaOral rehabilitation with fixed prosthesis ResumenEl tratamiento fue realizado a un paciente de sexo masculino, de 58 años de edad, con antecedente de accidente de tránsito de hace dos años, con pérdida de dientes, fracturas, necrosis pulpar, y alteración del plano oclusal. Debido a la patología existente, se realizó un tratamiento multidisciplinario con Periodoncia, Endodoncia, Cirugía y Ortodoncia, y en lo referente a prótesis se colocaron espigos, coronas y puentes fijos. AbstractThis treatment was carried out in a 58 years old male patient, who has had the antecedent of a car crash accident two years before; he loosed several teeth and fractured others. Pulp necrosis and alteration of occlusal plane occurred. Because of the pathologies presented, it was installed a multidisciplinary treatment phase having periodontal, endodontic, surgical and orthodontic procedures, tehe installation of posts, crowns and fixed bridges. INTRODUCCIÓNEl éxito de los tratamientos con prótesis fija en la práctica clínica diaria está directamente asociado a una planificación correcta y con criterio, que debe ser individualizada y ejecutada con el fin de atender las necesidades de cada paciente, orientándola para la determinación de un correcto plan de tratamiento. Es esencial un enfoque completo, secuencial para la planificación del tratamiento para la rehabilitación de prostodoncia fija no debe ser independiente de otras disciplinas de la Odontología. La planificación presurosa, segmentada, que ignora los principales aspectos del tratamiento requerido, desafía los modernos conceptos de tratar al paciente en su totalidad, más que a los dientes individuales 1,2,3 .Todos los factores identificados a través del interrogatorio deberán ser considerados cuando se planee el tratamiento. Como ejemplo de esto tenemos la longitud clínica de las coronas, o los dientes que serán restaurados o utilizados como pilares de los aparatos protésicos 4,5,6 . En caso de coronas cortas deberán considerarse los procedimientos de alargamiento coronario, a lo cual se puede arribar por vía ortodóncica o periodontal 14 . Asimismo deberá prestarse especial atención a las piezas extruidas en espacios desdentados extensos. Es frecuente la necesidad de colocarles coronas a estas piezas con el propósito de restablecer el plano de oclusión. No es inusual que a los pacientes que requieran próte-sis se les haya efectuado previamente un tratamiento periodontal 7,8,9,10 . Una prótesis parcial fija es un medio de restituir una o más piezas ausentes utilizando para ello una o más piezas remanentes.
