Patients with polycystic ovary syndrome (PCOS) usually are obese, insulin resistant and hyperinsulinemic. The known association between leptin, obesity and insulin action suggests that leptin may have a role in PCOS but this has only been addressed peripherally. This study was designed to assess the relationship between serum leptin and the anthropometric, metabolic and endocrine variables of obese (body mass index, BMI ≥30 kg/m 2 ) and non-obese (BMI <30 kg/m 2 ) PCOS patients. Twenty-eight PCOS patients and 24 control women subdivided into obese and non-obese groups were evaluated. Leptin, androgens, lipids, gonadotrophins and insulin-glucose response to the oral glucose tolerance test were measured by radioimmunoassay in all participants. The assays were done all in one time. The areas under the insulin curve (AUC-I) and the glycemia curve were calculated to identify patients with insulin resistance. Mean leptin levels were not significantly higher in patients with PCOS compared to the control group (21.2 ± 10.2 vs 27.3 ± 12.4 ng/ml). Leptin levels were found to be significantly higher in the obese subgroups both in patients with PCOS (26.9 ± 9.3 vs 14.1 ± 7.0 ng/ml) and in the control group (37.3 ± 15.5 vs 12.9 ± 5.8 ng/ml). The leptin of the PCOS group was correlated with BMI (r = 0.74; P < 0.0001) and estradiol (r = 0.48; P < 0.008) and tended to be correlated with the AUC-I (r = 0.36; P = 0.05). Of the parameters which showed a correlation with leptin in PCOS, only estradiol and probably insulinemia (AUC-I) did not show a significant correlation with BMI, suggesting that the other parameters were correlated with leptin due to their correlation with BMI. Estradiol correlated with leptin in PCOS patients regardless of their weight.

Doppler analysis of the uterine arteries and ovarian stroma was performed by transvaginal ultrasound in 24 patients with polycystic ovary syndrome (PCOS) and 22 ovulatory women. Vascularization of the ovarian stroma was more abundant in patients with PCOS than in control women, but no significant difference in the mean pulsatility index (PI) was observed between groups (1.14 ± 0.28 for the PCOS group and 1.05 ± 0.19 for the control group). The mean PI of the uterine arteries was significantly higher in the PCOS group (PI = 3.7 ± 0.8) than in the control group (PI = 2.9 ± 0.4). In the patients with PCOS, no correlation was observed between PI and luteinizing hormone, testosterone or androstenedione levels. Obesity had no effect on uterine artery PI, with no significant differences in this index when the 3 groups were subdivided into obese and non-obese groups.

Efetuaram-se anamnese, exame fí-sico e ginecológico, e a seguir, realizou-se ultrasonografia transvaginal com medida da espessura do eco endometrial. Em toda paciente, com espessura do eco endometrial maior ou igual a quatro milímetros, procederam-se aos exames de histerossonografia, histeroscopia e biópsia endometrial dirigida. Observaram-se 40 casos (68,9%) de alterações endometriais e 18 (31,1%) de endométrio atrófico. Encontram-se pólipo em 30 casos (51,7%), hiperplasia sem atipia em 5 (8,6%), adenocarcinoma do endométrio em 2 (3,4%), mioma submucoso em 2 (3,4%) e endométrio proliferado em 1 (1,8%) caso. Resumo de Tese (02): 123, 2001 RBGOA síndrome dos ovários policísticos (SOP) é uma afecção complexa cujos achados endócrinos principais são o hiperandrogenismo e a anovulação. A insulina tem importante papel na sua fisiopatologia. Para avaliar a eficácia do uso da biguanida metformina no tratamento da SOP foram selecionadas aleatoriamente 29 pacientes obesas e não-obesas, que fizeram uso de metformina 500 mg, via oral, de 8 em 8 horas (15) ou placebo na mesma posologia (14) durante seis meses. Foram divididas em 4 grupos e avaliados peso, ciclo menstrual, dosagens de testosterona total e livre, androstenediona e área sob a curva de insulina (ASCI) e glicose (ASCG) antes e após o tratamento. A identificação das pacientes com SOP que são mais insulino-resistentes poderá ser útil na seleção daquelas que se beneficiarão com o uso de drogas que melhoram a sensibilidade à insulina.Para se determinar a presença de RI, mé-todos invasivos como o clamp euglicêmico são considerados padrão ouro, mas são complexos. Estudo de Legro et al 2 teve como objetivo testar medidas indiretas da sensibilidade à insulina. Foram avaliadas 40 pacientes americanas brancas não-hispânicas obesas com SOP e 15 obesas controles através do FSIVGTT com determinação dos níveis de insulina (I) e glicemia (G) de jejum e durante TOTG-75g. A relação G/I foi considerada como anormal (presença de RI) quando menor que 4,5, com uma sensibilidade de 95% e especificidade de 84%, valor preditivo positivo de 87% e negativo de 87%. Foi demonstrado ainda que a relação G/I é mais acurada que a medida da glicemia ou insulinemia de jejum isoladamente.
