Objective: To assess the performance of central obesity indicators by correlation and agreement considering the body mass index in the nutritional assessment of adolescents according to sex and age. Methods: Cross-sectional census study with 635 adolescents, aged 10-19 years (342 males and 293 females), studying in a public school in Rio de Janeiro, Brazil. Study variables were body mass index (BMI), waist and abdominal circumference, waist-to-hip ratio, waist-to-height ratio, nutritional status, sex and age. Univariate and multivariate analyses adjusted by stratification were developed. Results: Waist and abdominal circumferences and BMI were positively and significantly correlated to sex, age group and nutritional status (p < 0.05). For the waist-to-hip ratio, co-relation variation was also influenced by age (p < 0.05). Waist and abdominal circumferences depended on BMI values in all groups (R 2 = 0.66-0.80, positive β), which did not occur in waistto-hip ratio and waist-to-height ratio. Only the waist circumference showed moderate agreement with BMI (Kappa: 0.41-0.57) in all groups, while for the remaining indicators this agreement ranged from poor to moderate. Conclusions: Among the central obesity indicators analyzed, waist and abdominal circumferences showed the best performances, but waist circumference was the most adequate to be used in addition to BMI in the nutritional assessment of adolescents. O transtorno de compulsão alimentar periódica (TCAP) é um distúrbio psiquiátrico com alta morbidade, associado à obesidade e ao alto risco cardiovascular. O objetivo deste trabalho foi identificar alterações antropométricas e/ou metabólicas em obesos com TCAP e compará-las àqueles sem TCAP. Estudo transversal realizado no ambulatório do Napta/ UERJ, envolvendo 39 adultos obesos, sem doenças malignas ou consumptivas e medicamentos que interferissem nos resultados, sendo 22 com e 17 sem TCAP. As variáveis coletadas foram peso, estatura, perímetro da cintura, pressão arterial; e foram analisadas glicemia, perfil lipídico, dosagem de leptina, adiponectina e insulina, sendo calculado o Índice HOMA IR. Os dados foram expressos como média (desvio-padrão), sendo utilizados os testes: Qui-quadrado e t-Student. O valor de significância foi 5%. Amostra composta por 82% de mulheres, com elevada prevalência de hipertensão arterial, resistência insulínica e sedentarismo, sem diferenças entre os grupos com ou sem TCAP. Todos apresentaram o perímetro da cintura acima da recomendação, mais de 60% apresentaram HDL-colesterol baixo e um terço, triglicerídeos elevados. Não houve diferença dos níveis de adiponectina nem de leptina entre obesos com ou sem TCAP, apesar de os valores estarem fora dos limites normais. Obesos, independentemente do diagnóstico de TCAP, têm elevado risco cardiovascular com alta prevalência de hipertensão arterial, dislipidemia, circunferência de cintura alterada e sedentarismo. Além disso, apresentam inflamação crônica de baixo grau com baixos níveis de adiponectina e altos de leptina. Entretanto, apesar de estas afirmativas terem sido confirmadas, não foram identificadas diferenças significativas entre obesos com o transtorno alimentar.Descritores: Obesidade; Adulto; Doenças cardiovasculares. Em 2002 foram criadas três disciplinas pelo Serviço de Medicina Integral, resultado de mudanças curriculares promovidas pela Faculdade de Ciências Médicas (FCM) da Universidade do Estado do Rio de Janeiro (UERJ), impulsionadas pelas Diretrizes Curriculares Nacionais (DCN) de 2001. O objetivo deste artigo é apresentar um relato da experiência dos treze anos de implantação de uma das disciplinas cujo foco é a abordagem familiar no âmbito da medicina de família e comunidade. Os processos para a inclusão da disciplina no currículo foram relativamente rápidos, mas o fato de não ter havido uma ampla reforma curricular impossibilitou a integração desta disciplina com as demais da FCM. Não foram poucos os desafios enfrentados ao longo dos anos para o aprimoramento do ensino-aprendizagem da abordagem familiar na graduação médica, cuja importância se confirma pela ênfase que o tema recebe nas novas DCN de 2014. É necessário que a abordagem familiar seja percebida como um valor e uma prática a serem incorporados ao longo dos seis anos de graduação. Para isso, deve-se aproveitar a reforma curricular da FCM atualmente em curso e sensibilizar docentes e estudantes sobre o assunto.
