Introdução: Os sarcomas primários do pulmão são tumores malignos raros com incidência estimada em torno de 0,5% de todas as neoplasias pulmonares. De suas tipificações descritas na literatura, duas especialmente, os leiomiossarcomas e os rabdomiossarcomas, apresentam semelhanças em seu perfil genético, morfológico e imuno-histoquímico, o que os levou a receberem uma mesma classificação: tumor rabdomioblástico inflamatório. Esse tipo de tumor costuma acometer tecidos moles em extremidades e tronco, e predomina em meio a homens jovens e de meia-idade. Em razão do pequeno número de casos de tumores rabdomioblásticos relatados na literatura, tanto seu diagnóstico quanto seu tratamento são pouco descritos. Relato de caso: Paciente, sexo feminino, 19 anos, apresentou três episódios de infecção respiratória no lobo inferior do pulmão direito em um período de 18 meses. A tomografia computadorizada do tórax evidenciou obstrução do brônquio intermediário e áreas de bronquiectasias no lobo inferior direito. Diante disso, realizou-se bilobectomia inferior-média, conduzida por cirurgia robótica. A paciente recebeu alta hospitalar três dias depois do pós-operatório. A imuno-histoquímica revelou tumor rabdomioblástico inflamatório de baixo grau. Conclusão: Este relato apresentou um caso de tumor pulmonar raro, abordado por uma técnica cirúrgica ainda não relatada para esse tipo de patologia.
Introduction Obstructive sleep apnea (OSA) is a prevalent and underdiagnosed clinical condition, characterized by the presence of repetitive airway collapse during sleep, resulting in oxyhemoglobin desaturation and sleep fragmentation. Among the most frequent reports of patients with OSA, is morning headache or chronic daily headache, which in 2004 received its own classification in the International Classification of Headache Disorders (ICHD-2) as sleep apnea headache. Objective The present review aims to evaluate the available scientific literature on the association between OSA and headache after classification, totaling a period of 18 years. Results In 8 included studies, 1,637 patients were analyzed, observing a bilateral and frequent association between the two pathologies. Conclusion However, there is still no consensus on the mechanism behind this relationship, and the results of studies are divergent.
IntroductionAttention deficit hyperactivity disorder (ADHD) has been related to the presence of primary headaches. Among them, migraine presents a wide range of comorbidities shared with ADHD, both from other psychiatric disorders and somatic conditions.ObjectiveThis review proposes to describe the association between ADHD and migraine.MethodsBased on literature research in the major medical databases and using as descriptors “Migraine Disorders” and “Attention Deficit Disorder with Hyperactivity”. Observational studies that addressed the relationship between migraine and ADHD and written only in English were included. Of the 49 articles found, only 6 met the inclusion criteria and were analyzed.ConclusionThis review shows a possible sharing of symptoms between migraine and ADHD, requiring further studies to investigate this relationship.
Theodor Kocher foi um grande cirurgião suíço que atuou no final do século XIX e início do século XX, contribuindo em diversas áreas da Medicina e sendo o primeiro cirurgião congratulado com um Prêmio Nobel de Medicina e Fisiologia, em 1909. Durante sua presidência da Clínica Cirúrgica da Universidade de Bern, Kocher se tornou referência entre os cirurgiões da Europa e influenciou grandes nomes da cirurgia, tais como Harvey Cushing e William Halsted. Além disso, seu rigor metodológico colaborou com a inserção do método científico na prática médica. Esse artigo de revisão propõe revisitar suas principais contribuições e discutir como seu trabalho influenciou no desenvolvimento da Medicina atual, com destaque para a cirurgia e fisiopatologia da tireóide, cirurgia da hipófise e cirurgia valvar para epilepsia.
Chest tube insertion is one of the most performed surgical procedures in medical practice, being performed in Brazil by both general practitioners and experienced surgeons. This study aimed to report an experience with the application of an animal model to simulate chest drainage, as well as review the scientific literature available on the subject. In our experience, pork rib pieces were used to simulate the human rib cage, where medical students trained the procedure with the help of a thoracic surgeon. The porcine model used proved to be satisfactory, allowing the study of anatomical correlations and surgical technique among the students. In our review, we found 5 other experiences reported, 3 in a synthetic anatomical model, 1 in a porcine model and 1 comparative study between a synthetic model and a cadaver model. All models proved to be efficient for training students, but they present differences in terms of cost and durability, with the synthetic model being the most cost-effective alternative.
