Violence against women attending public health services in the metropolitan area of São Paulo, Brazil ABSTRACT OBJECTIVE: To estimate the prevalence of (physical, psychological, and sexual) violence against women by an intimate partner and non-partner perpetrators among users of public health services and to compare these women's perception of having ever experienced violence with reports of violence in their medical records in the different services studied. METHODS:The study was conducted in 19 health services, selected as a convenience sample and grouped into nine research sites, in metropolitan area of São Paulo from 2001 to 2002. Questionnaires on having ever experienced violence in their lifetime and in the last 12 months and perpetrators were applied to a sample of 3,193 users aged 15 to 49. A total of 3,051 medical records were reviewed to verify the notifi cation of violence. Comparative analyses were performed by Anova with multiple comparisons and Chi-square test followed by its partition. RESULTS:The following prevalences were found: any type of violence 76% (95% CI: 74.2;77.8); psychological 68.9% (95% CI: 66.4;71.4); physical 49.6% (95% CI: 47.7;51.4); physical and/or sexual 54.8% (95% CI: 53.1;56.6), and sexual 26% (95% CI: 24.4;28.0). The prevalence of physical and/or sexual violence by an intimate partner in their lifetime was 45.3% (95% CI: 43.5;47.1), and by non-partners was 25.7% (95% CI: 25.0;26.5). Only 39.1% of women reporting any episode of violence perceived they had ever experienced violence in their lifetime and 3.8% of them had any reports of violence in their medical records. The prevalences were signifi cantly different between sites as well as the proportion of perception and reports of violence in medical records. CONCLUSIONS:The expected high magnitude of the event and its invisibility was confi rmed by low rate of reports in the medical records. Few perceived abuses as violence. Further studies are recommended taking into account the diversity of service users.

É difícil o relato de violência sofrida por mulheres. Trata-se da invisibilidade da violência que afeta as relações usuárias - profissionais, criando impasses comunicacionais. Buscou-se caracterizar este silêncio, estudando usuárias de atenção primária na rede pública de São Paulo, quanto a prevalência de violência, a percepção de ter sofrido violência, a definição de violência em geral e a nomeação dada por quem a experimentou. Entrevistaram-se 322 usuárias de 15 a 49 anos, sobre agressões física, sexual e/ou psicológica, o agressor, e a percepção de ter sofrido violência, solicitando-se o relato de um episódio marcante, o nome que daria a este e a definição de violência em geral. Das entrevistadas, 69,6% referiram alguma agressão física, psicológica ou sexual e, destas, 63,4% não consideraram haver sofrido violência na vida; 64,3% relataram algum episódio marcante e 46,5% atribuíram um nome ao vivido. A definição de violência mais comum foi a de agressão física (78,8%), seguida pela psicológica (39,7%) e sexual (24,2%). Conclui-se que a maioria das mulheres que referiu alguma agressão não considerou haver sofrido violência na vida. Houve grande dificuldade em contar episódios e nomeá-los, e apesar de a maioria desses episódios serem do âmbito doméstico, na definição de violência esta referência não aparece.
Buscou-se identificar e analisar a inserção dos psicólogos na rede intersetorial de serviços para mulheres em situação de violência. Foram analisadas entrevistas com profissionais de serviços específicos da Grande São Paulo (policial, saúde, psicossocial, abrigo, orientações básicas). Os psicólogos estão presentes e são solicitados em todos os tipos de serviços. Têm lugar na capacitação e supervisão dos profissionais, além do atendimento às mulheres. Na assistência, nota-se grande diversidade de práticas, com frequente ajustamento das intervenções aos objetivos, à cultura institucional e à vocação assistencial dos serviços. Há uma relativa indefinição na especificidade do trabalho do psicólogo, o que pode representar impasses para uma melhor articulação em equipe e em rede, mas, por outro lado, também pode criar oportunidades para inovações na prática.

The aim of the present investigation was to identify and analyse the insertion of psychologists working in the network of services for assisting women experiencing violence in the metropolitan region of Sao Paulo. The organization of psychological assistance in diverse assistance vocations and the integration of psychologists in work teams and in other services were studied from the perspective of women's needs, and multi-professional and intersectoral actions in facing the situations and providing care for gender-based violence. For that, normative documents on violence care were researched (Ministry of Health and Department of Special Policy for Women) and data from interviews with professionals working in 109 services with diverse assistance vocations in Sao Paulo (police, justice, health, psychosocial assistance, shelters, basic assistance) were analysed. Psychologists are present and their work is requested in all types of services, but with less participation in police and justice facilities. They work in the training and supervision of other professionals, as well as directly assisting the public. A great diversity of practices was observed in the assistance -clinical, educational, orientation and judicial mediation activities -with frequent adjustment of these interventions to the objectives and hierarchical culture within each category of service. In general, psychologists are called to work in the strengthening of self-esteem, the resuming of sexual life, restructuring life itself, understanding of the situation of violence, working-out of and escape from the victim condition, promotion of autonomy and search for transformation in family/ intimate relationships or gender patterns. These objectives are put to the general teams working in the services, especially the first one. The influence of conceptions and proposals from the women's movement is perceived in the formulation of the services' objectives and in the way the activities are developed. The distinction between the psychologists' work and the work of other professionals is not always clear, especially regarding social work. This results in imprecise functions and actions, which may represent difficulties in the dialogue between professionals and services, and in the integration of multi-professional actions, potentially compromising integral care to women experiencing violence. On the other hand, this impreciseness opens up space for innovations in assistance practices. Multi-professional integration is, in general, also sought within work teams of each service, but in some of them the professionals search for complementarity in their actions and integration with services with different assistance vocations. There is a need for better definitions on the specific role of psychologists in the assistance of women experiencing violence, considering the specific network and the broader assistance network in Sao Paulo.Descriptors: violence against wo...

A concepção que norteia esta proposta é a da atenção integral e da atuação em rede pelos serviços, trabalhando em uma abordagem integrada e inclusiva. A perspectiva adotada é a de uma atenção integral, em que os serviços estejam pautados em trabalho de equipes com abordagem interdisciplinar e desempenho multiprofissional, além de uma integração intersetorial. Nesse sentido, não só no interior de cada serviço, mas entre os serviços de um mesmo setor de assistência, assim como entre os distintos setores que prestam assistência às mulheres em situação de violência, espera-se que o trabalho se dê de modo interligado, para que a um conjunto dado de serviços se possa atribuir a qualificação de 'rede'.A atuação em 'rede' requer, assim, um determinado modo de funcionamento, que não é dado apenas pela existência de um conjunto de serviços, pois o conjunto pode expressar só uma somatória de intervenções, com serviços justapostos cuja atuação não necessariamente estabelece alguma integração assistencial. E isto é bem provável que aconteça, em razão dos serviços operarem como unidades de vinculações institucionais e tradições assistenciais distintas entre si, já que estão aderidos a intervenções pertencentes a culturas profissionais bastante diversas.
