The functional capacity of flow limiting myocardial conductance vessels was evaluated in canine hearts. In an isolated heart preparation transmural coronary flow distribution during maximal vasodilation was measured in the unloaded diastolic arrested left ventricle with tracer microspheres. The ratio of subendocardial versus subepicardial (ENDO/EPI) flow in the left ventricular free wall was 1.6. Measurements in 8 different wall layers showed a successive increase in maximal coronary flow from the subepicardium towards the deeper layers. A decreased subendocardial vascular resistance due to a better vascularization is forwarded as a mechanism to compensate for the extravascular compression during cardiac contraction. This statement contradicts the commonly accepted hypothesis that a diminished vascular tone with a reduction of the dilatory reserve in the subendocardium accounts for a homogeneous flow distribution in the normal beating heart. An augmentation of subendocardial supplying vessel capacity could be established from the angiographic determination of the coronary arterial volume of intramural small arteries and arterioles. From a strict parallelity in maximal coronary flow and coronary arterial volume within the wall, it becomes probable that these vascular structures are the flow-limiting factors which determine regional coronary flow reserve in the absence of extravascular compressive forces.
A case is reported of ethylene oxide polyneuropathy after 5 months of exposure. There was symmetrical distal weakness of both lower extremities and transitory reduced nerve conduction velocities with increased latencies. Sural nerve biopsy revealed nerve fibre degeneration of the Wallerian type, associated with reduction of axonal cross-sectional areas and some degree of nerve fibre regeneration that could be confirmed morphometrically. In addition, there was conspicuous paranodal vesicular disintegration of individual myelin lamellae. Unusual cisternae with introverted hemidesmosomes were noted in endoneurial fibroblasts.
In der Faehliteratur der Tuberkulose hat man in der letzten Zeit wenig fiber die Indikationsstel/ung, den Verlauf und die Beurteilung der Dauerwirkung des Pneumothorax lesen kSnnen. In der Pneumothoraxliteratur der vergangenen Jahre dominieren fraglos Abhandlungen fiber unangenehme ZwischenfSlle be[ der Anlage und bei der Naehfiillung des Pneumothorax, fiber Spontan-Pneumothorax und fiber rein chirurgisehe ]Y[ai3nahmen, die den Kollaps trotz weitgehender Adh~sionen herbeifiihren sollen. Daraus geht jedenfalls das Bestreben hervor, die Pneumothoraxtherapie noeh welter auszubauen. Andererseits kommen aus dem Lager der nieht aussehliel~lichen Tuberkulose~rzte doeh Stimmen zur Geltung, die darauf schlieBen lassen, dab in manchen Kreisen der KoUegen die Ansicht Platz greift, dab die Pneumothoraxtherapie wie manche andere ,,Modebehandlung" allmi~hHch wieder in den I-[intergrund trete. Die Ansicht, dal~ der kfinstliche Pneumothorax bei der Behandlung der Tuberkulose nieht mehr zu missen ist, bedarf wohl keiner Begrfindung. ~qach meiner Kenntnis der Literatur der letzten Jahre liegen Mitteilungen fiber ]ang beobachtete PneumothoraxfSlle nicht vor. Es ersehien daher nicht wertlos, das Material des Krankenhauses, das bei Absehlul3 der Arbeit aus 102 Pneumothoraxfallen bcsteht, beziiglich seines weiteren Schicksals zu durchforsehen. Es war zunKchst niebt beabsichtigt, eine Kasuistik zu bringen. Bei der Zusammenstellung der Arbeit hat sieh aber, was ich aus redaktionellen Grfinden wegen der Inanspruchnahme eines gr61~eren Raumes bedauere, gezeigt, dal~ eine Wiedergabe aller unserer Eindrfieke bei der Behandlmlg ohne Anffihrung der einzelnen Daten nur unvollsthndig erfolgen kann. Im folgenden sollen nun die einzelnen Krankengeschichten nach bestimmten Gesichtspunkten zusammengestellt m6gliehst kurz wiedergegeben werden. Am Ende jeder Gruppe wird das die einzelnen F~lle Charakterisierende niedergelegt. Einzelheiten mfissen in den Krankengesehichten naehgelesen werden.
