Introdução: A colonoscopia é o principal método para avaliação, diagnóstico e prevenção de doenças colorretais. O preparo de boa qualidade é fundamental para realização do exame adequado. É imprescindível um preparo que permita a visibilidade adequada da mucosa colorretal, sendo o manitol a 10%, via oral, por suas propriedades hiperosmolares e ausência de toxicidade, a substância mais utilizada atualmente. Devido ao banimento do manitol, não há estudos sobre as repercussões hidroeletrolíticas desta substância. Dessa forma, torna-se pertinente o estudo das alterações hidroeletrolíticas relacionadas ao seu uso no preparo de cólon e possíveis repercussões hemodinâmicas no paciente saudável. Objetivo: Avaliar alterações hidroeletrolíticas em pacientes submetidos ao preparo de cólon antes e após a ingestão do Manitol a 10%. Métodos:Estudo prospectivo, onde 73 pacientes, do sexo feminino e masculino, foram encaminhados ao Serviço de endoscopia para realização de colonoscopias eletivas. Foram excluídos pacientes com doença renal aguda ou crônica, dialíticos, nefrectomizados, colectomia parcial ou total, idade < 18 anos e > 80 anos, grávidas, com dificuldade de deambulação ou neuropatias. Foram submetidos à pesagem corporal e punção venosa para coleta de sangue para avaliação das variações de Sódio, Potássio, Uréia e Creatinina séricos e Gasometria venosa atribuíveis ao preparo antes da ingestão do manitol e após duas horas de jejum, segundo o protocolo do serviço. O trabalho foi aprovado pelo Comitê de Ética da Instituição. O termo de Consentimento Livre e esclarecido foi obtido dos participantes. Resultados: Avaliando os resultados de peso corporal, dosagem de Sódio, Potássio, Uréia, Creatinina e Gasometria venosa, encontramos variações estatisticamente significativas para o excesso de bases do fluido extracelular, que mostraram variação negativa das médias,(p<0,0001) comprovando que, embora não haja desequilíbrios hidroeletrolíticos importantes, houve consumo de bases do sistema tampão do sangue. Conclusão: Não foram observadas variações nos níveis séricos nas dosagens de Sódio, Potássio, pH e Bicarbonato pré e pós preparo de cólon estatisticamente significativas, porém, houve variação negativa significativa do excesso de bases e bases extracelulares, indicando que houve consumo de bases no sangue atribuíveis ao preparo de cólon sem, contudo, levar a acidose metabólica. (178) foram endoscopia digestiva alta (EDA) e 13% (44) colonoscopia, sendo os demais, broncoscopia (28%), laringoscopia (6%) e colangiopancreatografia retrógrada (0,3%). Analisaremos, somente os exames de EDA e colonoscopia. Com relação à sedação, 75,6% (168) dos procedimentos foram assistidos pelo médico anestesista e 24,4% (54) pelo endoscopista, sendo que 108 (48,6%) foram realizados sob intubação orotraqueal. Principais indicações de EDA: ingestão de corpo estranho (18,5%), epigastralgia (17,4%), disfagia (14,6%), hemorragia digestiva alta (14,1%), hemorragia digestiva baixa (10,6%), dor abdominal (10,1%) e outras (14,9%). Principais indicações de colonoscopia: distensão abdominal em (40,9%), hemorragia digestiva baixa em (29,5%), diarreia (15,9%) e dor abdominal (13,9%). Principais resultados de EDA: Exame normal (67,5%), corpo estranho (8,9%), gastrite (5%), esofagite (3,9%) úlcera gástrica (2,2%) e duodenal (2,2%). Principais resultados da colonoscopia: Exame normal (43,2%), Intussuscepção (38,6%), pólipos (11,3%). Conclusão: Os resultados dos exames de endoscopia e colonoscopia por nós observados na faixa etária pediátrica estão em consonância com os dados descritos da literatura, com exceção da maior incidência de intussuscepção intestinal. Isso pode estar relacionando a característica do Serviço que é referência da região e disponibiliza colonoscopia de urgência.
