Se objetivó investigar la producción científica sobre la práctica de ortotanasia y cuidados paliativos en pacientes con cáncer terminal. Para eso, se realizó una revisión sistemática de la literatura por medio de búsqueda de artículos en el portal de periódicos de la “Coordenação de Aperfeiçoamento de Pessoal de Nível Superior” (Capes), publicados en portugués, en el intervalo de 2011 a 2016, que presentaban los descriptores: "Ortotanasia" AND "Cáncer" (f = 6); OR "Cuidados Paliativos" AND "Cáncer" (f = 70). Después de la selección por medio de criterios de exclusión, se analizaron 19 artículos. Los resultados evidencian la existencia de la obstinación terapéutica. Por otro lado, refuerzan la importancia del proceso de humanización de la muerte y de la toma de decisión sobre la ortotanasia hecha por profesionales, pacientes y familiares.
Objetivou-se apreender a percepção e os significados expressos por pacientes oncológicos em cuidados paliativos exclusivos sobre a morte, o câncer e sua experiência do processo de finitude. Contou-se com a participação de dez pacientes com câncer: cinco em atendimento domiciliar e cinco em internação hospitalar. Utilizaram-se de diferentes instrumentos e técnicas, em etapas distintas. Para a realização da Etapa 1, realizaram-se observações, por meio de visitas sistemáticas e escuta livre, guiadas por um roteiro de observação, com registro em diário de campo. Para a Etapa 2, utilizou-se o Teste de Associação Livre de Palavras (TALP), com estímulos indutores ‘Câncer’ e ‘Morte’. Os dados foram compreendidos por meio de análise descritiva (Diário de campo) e categorização (TALP). Os resultados mostram que o câncer e o seu tratamento estão associados à dor e ao medo. Os participantes apresentam dualidades entre a esperança de cura e a certeza de prognóstico reservado, e entre a aceitação e a negação da morte. A família tem papel fundamental no acolhimento e na oferta de autonomia e dignidade do paciente em processo de morte, embora também sofram com o coadoecimento. A espiritualidade/religiosidade é uma estratégia de enfrentamento utilizada pelos pacientes, reduzindo a angústia espiritual, apesar de também poder sustentar a esperança de cura e negação da morte, dificultando o processo de terminalidade. A autonomia também é um fator que contribui para a dignidade do paciente. Conclui-se que a aceitação da morte, suporte familiar e espiritual/religioso e respeito à autonomia contribuem para a ortotanásia e permitem a morte com mais dignidade.
Objective: to analyze the perception about dignified death and orthothanasia in the view of health professionals who provide curative and palliative care in oncology. Method: this is a research with exploratory, qualitative approach. Saturation criteria included the participation of eight health professionals from the Cancer Institute of Ceará, Brazil: a physician, a nurse, a social worker and a psychologist of the palliative care team, and an oncologist, a nurse, a social worker and a psychologist of the curative care team. Three instruments were used: semi-structured interview; Free Word Association Test (WAT) with "cancer" and "orthothanasia" as inductive stimulus; and a Photo Projection Test (PPT) using five images. A textual analysis was carried out using the Iramuteq software to analyze the interviews. For the analysis of the WAT, the frequencies of the words of each stimulus were counted; then they were submitted to the word reduction technique and understood through simple frequency analysis. For the analysis of the PPT, the frequencies were counted, followed by analysis of meanings. Results: cancer is perceived by professionals as synonymous with death, pain and suffering, and orthothanasia as a practice of care and affection. The importance of the transition from curative care to exclusive palliative care is evident when there is no prognosis of cure, so as to offer quality of death and pain relief. This process can be facilitated by the acceptance of death and participation on the part of the family. The family must also receive care. Conclusion: health professionals recognize the importance and challenges of orthothanasia.
