Obiective. Scopul acestui studiu a fost să aflăm care sunt cele mai frecvente cauze de trombocitopenie la copiii care se prezintă în Unitatea de Primire Urgenţelor şi să evaluăm importanţa investigaţiilor paraclinice şi a măsurilor terapeutice imediate în cazul pacienţilor cu purpură trombocitopenică imună (PTI). Material şi metodă. Studiul a inclus 73 pacienţi cu trombocitopenie evaluaţi în Unitatea de Primire a Urgenţelor pe parcursul a 6 ani, cu vârste cuprinse între 2 luni şi 16 ani. Datele colectate au fost: vârsta şi sexul pacienţilor, manifestările clinice, investigaţiile efectuate, diagnosticul de etapă sau diagnosticul final şi tratamentul efectuat în Unitatea de Primire a Urgenţelor. Rezultate. Cei mai mulţi dintre pacienţi au fost diagnosticaţi cu sepsis (40%). Cel deal doilea cel mai frecvent diagnostic a fost PTI (35%). Valoarea trombocitelor înregistrată la pacienţii cu PTI a fost cuprinsă între 1.000 şi 87.000, iar 14/25 (56%) dintre pacienţi au avut valori mai mici de 20.000/mm3. Cei mai mulţi dintre pacienţii cu PTI au primit imunoglobuline i.v. Concluzii. PTI şi sepsisul sunt cele mai frecvente cauze de trombocitopenie cu care se prezintă pacienţii în Unitatea de Primire a Urgenţelor din cadrul Spitalului Clinic de Urgenţă pentru Copii Cluj-Napoca. Suspiciunea hemopatiilor maligne a fost ridicată pe baza datelor anamnestice şi a modificărilor decelate la examenul obiectiv. Tratamentul cu imunoglobuline i.v. a fost administrat la un număr mare de pacienţi cu PTI, mulţi neavând hemoragii active. Concluziile acestui studiu ne-au permis să elaborăm un protocol de investigaţii şi tratament pentru pacienţii cu PTI.
