La utilización de electrocauterios como elementos de apoyo en los procesos de hemostasia ha sido ampliamente reportada y hace parte de las herramientas en procedimientos quirúrgicos en diferentes áreas médicas, aunque su uso en procedimientos en cavidad oral sea menor. Actualmente su aplicación ha venido aumentando y se encuentra involucrado en diversos procedimientos odontológicos. Al analizar la literatura, se encuentra muy poca información en cuanto a las complicaciones asociadas al uso del electrobisturí, las cuales son poco frecuentes o no reportadas. Se presenta un reporte de caso sobre Osteonecrosis inducida por electrobisturí en zona proximal de incisivos centrales, posterior a frenillectomía vestibular en maxilar superior. El proceso de sobreexposición del hueso maxilar a la acción de puntas de electrocirugía puede desencadenar Osteonecrosis. Los secuestros óseos generados crean posteriores defectos periodontales tras su remoción. Los movimientos ortodónticos acompañados de recontorneo de puntos Zenith e injertos de tejido conectivo pueden ser una alternativa en el manejo estético de la complicación.
