When a traumatic dental injury affects the deciduous teeth, the main management is to regularly review the patient until the successor permanent tooth erupts to avoid further consequences to the affected tooth and, especially, sequelae in the tooth germs of the successor permanent tooth. This report describes a case of a tooth extrusion in a 4-year-old male treated with a minimally invasive approach. After 4 years of follow-up, there was a mild disturbance (demarcated opacity in the vestibular surface) and a satisfactory result with normal eruption of the permanent successor tooth. Background: Dental bleaching agents show the ability to permeate through dental hard tissues, which may lead to pulp tissue changes. This systematic review (PROSPERO register: CRD42020213767) is aimed at understanding the effects of bleaching agents on the process of mineralization of the pulp tissue.Methods: Only in vitro studies evaluating the influence of hydrogen peroxide (HP) on mineralization in dental pulp cells were included. Studies without a non-bleached control group or cells after co-treatment with a bleaching agent other than HP and/or carbamide peroxide were excluded. The primary outcomes evaluated were alkaline phosphatase (ALP) activity and mineralized nodule deposition. The mineralization markers analysis in dental pulp cells and the cell viability were considered secondary outcomes. Two independent authors conducted a systematic search (PubMed/MEDLINE, Scopus, Embase, Cochrane Library, and OpenGrey until January 2021) with no language restrictions and performed data extraction. The quality assessment was appraised according to a modified Joanna Briggs Institute critical appraisal checklist.Results: The search resulted in 473 studies, and 11 were considered eligible. Overall, a reduction in the process of mineralization was observed among pulp cells after bleaching. A reduction in the ALP activity was reported in the mostly bleached groups using different protocols and analysis periods of nine studies. Regarding mineralized nodule deposition, 6 studies reported a significant reduction from 7 to 21 days among bleached groups. Of those three studies that investigated other mineralization markers, two found a reduction in the expression of dentin matrix acidic phosphoprotein (DMP)-1, dentin sialophosphoprotein (DSPP), and matrix extracellular phosphoglycoprotein (MEPE) among some bleaching gel concentrations. In contrast, one study showed a greater expression of osteopontin (OPN) and osteocalcin (OCN) in 100 μmol/L HP after 5 or 10 min of exposure, and another study showed significant induction of DSPP in concentrations of up to 0.5 mmol/L HP.Conclusion: Especially, high concentrations of bleaching gel reduce the potential of mineralization in pulp cells in in vitro studies; however, different HP concentrations, bleaching protocols, and analysis periods can influence this outcome.
O conhecimento da anatomia é um dos fatores primordiais para o sucesso da comparação de registros dentários durante uma investigação de um indivíduo na odontologia forense. Para realizar esse tipo de procedimento, o profissional deve estar apto a identificar e diferenciar estruturas anatômicas do complexo maxilofacial. O objetivo desse estudo foi verificar se existe correlação entre ângulo goníaco e o forame mentoniano a idade e o gênero, de pacientes submetidos à exames de Tomografia Computadorizada da mandíbula. A metodologia desse trabalho baseou-se na obtenção e análise dos arquivos DICOM, de forma que estas imagens foram convertidas para um programa específico de tratamento de imagens ImplantViewer (Anne Solutions, São Paulo, Brasil), e avaliadas utilizando a Janela Panorâmica e Cortes Transversais de Tomografias Computadorizadas. Indivíduos do gênero masculino apresentaram o forame mentoniano mais distante da crista alveolar do que os indivíduos do gênero feminino. A TCFC é uma importante ferramenta na visualização das estruturas anatômicas, favorecendo as atividades inerentes a Odontologia Forense. Neste estudo, o gênero masculino apresentou o forame mentoniano mais distante da crista alveolar do que o gênero feminino.
Os traumatismos dentários são situações que ocorrem com frequência na população, a prevalência é maior em pessoas do gênero masculino, em idade escolar ou atletas. Dependendo da severidade do trauma, podem surgir complicações que devem ser corretamente diagnosticadas e tratadas. Tais complicações incluem a necrose pulpar, as reabsorções radiculares externas ou por substituição e as calcificações da câmara pulpar. A obliteração do canal radicular ou metamorfose calcificante é caracterizada pela deposição de tecido duro no espaço pulpar, podendo ser observada radiograficamente, e pela coloração amarelada da coroa dentária. Em alguns casos pode estar associada à necrose pulpar e presença de lesão periapical e o tratamento pode ser considerado complexo. A maioria das calcificações pulpares são assintomáticas e são classificadas de acordo com a localização e morfologia. O principal método de diagnóstico tem sido através de radiografias intraorais e panorâmicas, embora as Tomografias Computadorizadas de Feixe Cônico (TCFC) ofereçam melhores detalhes. Por isso, o objetivo desse trabalho foi revisar a literatura acerca dos padrões de obliteração do canal radicular mais frequentemente relatados na literatura científica relacionados aos traumatismos dentários, de forma a ajudar o profissional na orientação, diagnóstico, planejamento do tratamento e devido prognóstico.
O presente artigo é um estudo de caso, com estrutura descritiva e qualitativa, com objetivo de relatar o tratamento endodôntico em um segundo molar decíduo esquerdo (75), em um paciente com oligodontia. Após anamnese e exame clínico o diagnóstico de necrose pulpar foi estabelecido. Posteriormente o tecido cariado foi removido com broca esférica em baixa rotação, o elemento foi isolado, acessado e o preparo químico-mecânico foi realizado com auxílio de limas de níquel-titânio rotatórias do sistema Sequence BabyFile, hipoclorito de sódio a 2,5% e ácido etilenodiamino tetra-acético a 17%, utilizando protocolo de agitação. A obturação do sistema de canais radiculares foi realizada com pasta de hidróxido de cálcio iodoformada, inserida com espiral lentulo. Pacientes com oligodontia requerem acompanhamento multidisciplinar e o tratamento endodôntico de dentes decíduos, especialmente quando necrosados, evita extrações precoces e, na ausência do sucessor permanente, tem grande valia do plano de tratamento, visando manutenção de espaço e osso alveolar, ancoragem de dispositivos ortodônticos e impedir extrusão de elementos antagonistas.
