This case study demonstrates a 33-year old nursing mother who had noticed that her breastmilk had traces of blood giving the appearance of a strawberry coloured milk while expressing for her infant. This is an uncommon encounter but important issue to raise concern among Muslim nursing mothers in terms of medical safety and Islamic ruling of the standpoint of legal theorists (usul al-fiqh) and legal maxim (qawai’id fiqhiyyah).
Introduction: Spirituality is essential in patient care. In dietetics, the patient-centred care approach has a significant role in promoting caring relationships with the patients in dietary counselling. While integrating spirituality in clinical practice influences patients’ ability to cope with illness, there is a paucity of research on spirituality in dietetics. This study aimed to discover the proportion of Muslim dietitians who inquired about patients’ spiritual needs and its association with socio-demographic factors, identify selected conversations on spiritual concern and its challenges. Method: This cross-sectional study adopted selected items from The International NERSH-Cohort questionnaire. A new semi-structured question on responses to patients’ spiritual concerns was also evaluated (4 items; alpha=.79). Data were gathered through an online survey and analysed using descriptive analysis and Pearson chi-square.Results: Out of ninety-eight respondents, only 3% reported always making an attempt to inquire about patients’ spiritual needs, whereas 44% had never made such an inquiry. The duration of practising dietetics was not associated with inquiring spiritual needs practices. When the Muslim patients brought up a discussion on spiritual concern, 38% of the Muslim dietitians had responded that only Allah has the power to heal and grant happiness. This study highlights that Muslim dietitians’ most common obstacles to addressing spiritual concerns during dietary counselling were the fear of offending the patients and insufficient knowledge/training. Conclusion: There are ways for Muslim dietitians to engage in spiritual conversations during dietary counselling, but the existing challenges indicate a calling for formal training.
Introduction: Medical futility and advanced medical directive are related issues in end-of-life medical decisions that present challenge to doctors and patients in terminal condition and to doctors and family members of patients who are critically ill in the ICU. Materials and Methods: A qualitative study comprises of literature search and an in-depth interview of experts was carried out to determine the clinical situations in medical futility and the ethical considerations from the Islamic perspective that justify forgoing medical treatment and also the practice of advanced medical directives in the country. Results: In such scenarios, clinical data must be interpreted alongside patient values, as well as the physicians’ ethical commitments. From the Islamic perspective, doctors are the authorised person to diagnose medical futility and forgo medical treatment. Forgoing treatment is permissible as long as it conforms to the maqasid and qawaid al-shariah principles that guide the decision-making process. The practice of advanced medical directive is still at its infancy in this country. From the Islamic perspective, upon considering the maqasid and qawaid al-shariah principles, it should be permissible and its practice should be encouraged as it assists the doctors and the surrogate decision-maker to decide to the withdrawal of treatment. Conclusion: Ethical justification on medical futility conforms to the maqasid and qawaid al-shariah principles and forgoing medical treatment is permissible in Islam. Advanced medical directive assists in the decision-making of forgoing treatment in the presence of utility and thus its practice should be promoted.
This paper offers an examination of the scholarship of Dāwūd al-Faṭānī in dealing with the revealed sources (naqlī) and qualified scholarship in the exercise of the intellect (‘aqlī) in affirming the Islamic creed. The author emphasizes the determination of Dāwūd al-Faṭānī on the need to refer to the authority of naqlī and ‘aqlī so as to guard against deviation and exceeding the proper limits of methodological practices towards theological goal of knowing God (ma‘rifat Allāh). A qualitative methodology in which the textual analysis and comparative study was employed to analyse textual materials related to Dāwūd al-Faṭānī especially his writing, the Ward al-Zawāhir. Findings indicate that Dāwūd al-Faṭānī attempted to integrate the naqlī and ‘aqlī approaches, and harmonize such distinctions in dealing with theological approaches especially that of Salaf and Khalaf’s.
