Öz Amaç: Çalışmamızın amacı; acil serviste akut iskemik inmeli hastalarda, hastaların görüntüleme bulguları ve bu bulgulara etki eden faktörleri araştırmaktır. Materyal ve Metot: Bu çalışma, yerel etik komitesince onaylandıktan sonra retrospektif, vaka-kontrol çalışması olarak yapıldı. Çalışmaya 01.06.2013-31.05.2014 tarihleri arasında Keçiören Eğitim ve Araştırma Hastanesi Acil Servisine başvuran, 18 yaşından büyük, acil serviste akut iskemik inme tanısı alan, hem Bilgisayarlı Beyin Tomografileri (BBT) hem de Difüzyon ağırlıklı Manyetik Rezonans görüntülemeleri (DAMRG) çekilen 103 hasta dahil edildi. Hastaların demografik verileri, fizik muayeneleri, görüntüleme bulguları kayıt edildi. Hastaların DAMRG ile BBT karşılaştırıldı. İstatistiksel analiz SPSS 15.0 paket programı kullanılarak yapıldı. p<0.05 değeri istatistiksel olarak anlamlı kabul edildi. Bulgular: Yaş gruplarına, cinsiyet, özgeçmişindeki hastalıklar, nörolojik muayeneye göre BT ve DAMRG bulguları değerlendirildiğinde istatistiksel olarak anlamlı bir farklılık saptanmadı (p> 0.05) Hastaların şikayetinin başladığı saat dilimlerine göre BT bulguları değerlendirildiğinde şikayetleri 00: 01-06: 00 ve 18: 01-00: 00 saatlerinde başlayan hastaların BT bulgusu pozitifliği anlamlı derecede yüksekti (p=0.03). Hastaların şikayetinin başlamasından başvuruya kadar geçen süreye göre BT bulguları değerlendirildiğinde; geçen süre artıkça BBT'de bulgu olma oranı istatistiksel anlamlıydı. DAMRG bulguları değerlendirildiğinde ise, başvuru süresine göre anlamlı bir farklılık saptanmadı (p=0.43). Sonuç: Çalışmamızda, hastaların yaş, cinsiyet, komorbidite, nörolojik muayene ve lezyon lokalizasyonlarına göre BT ve MRG bulguları değerlendirildiğinde değişkenler arasında anlamlı bir farklılık saptamadık. Hastaların başvuru süresi uzadıkça BBT'de bulgu olma oranı artmakta, DAMRG ise hiperakut dönemde bulgu vermeyebilir.
