Although CS usage was determined to increase the costs in this study, it significantly decreases intraoperative and postoperative ABT requirements. We believe that the increase in cost may be neglected when the complications and prolonged hospitalization due to ABT usage were regarded.
BACKGROUND: Six acute traumatic hip dislocations in pediatric patients were retrospectively analyzed. Types of dislocations, associated lesions, treatment methods, complications, and clinical and radiological outcomes were reviewed.
Fluid extravasation is possibly the most common complication of shoulder arthroscopy. Shoulder arthroscopy can lead to major increases in the compartment pressure of adjacent muscles and this phenomenon is significant when an infusion pump is used. This article describes a case of pectoral swelling due to fluid extravasation after shoulder arthroscopy. A 24-year-old male underwent an arthroscopic Bankart repair for recurrent shoulder dislocation. The surgery was performed in the beach chair position and lasted two hours. At the end of the procedure, the patient was found to have left pectoral swelling. A chest radiography showed no abnormality. Pectoral swelling due to fluid extravasation after shoulder arthroscopy has not previously been documented.
BACKGROUND:The aim of our study was to determine the efficacy and cost-effectiveness of intraoperative autotransfusion that uses the cell saver system (CSS) in patients undergoing posterior instrumentation and fusion of thoracic and lumbar vertebral fractures. METHODS:We divided 121 patients who were to undergo posterior instrumentation and fusion due to thoracic and lumbar vertebral fractures into two groups: 59 patients (23 males and 36 females) were in the cell saver group, and 62 patients (22 males and 40 females) were in the control group. Hemoglobin, hematocrit, and red blood cell (RBC) values were recorded for all patients preoperatively, on the postoperative first, second, and third days, and on the hospital discharge day. Transfusion rates and numbers of allogeneic erythrocyte transfusions, as well as the costs of transfused total auto-and allogeneic transfusions were compared. RESULTS:The numbers of erythrocyte suspensions transfused perioperatively were 0.2±0.6 units in the cell saver group and 0.7±1.4 units in the control group (p=0.01). Statistically significant differences were noted between the two groups on the postoperative first, second, and third days in terms of hemoglobin, hematocrit, and RBC values. These differences had disappeared by the hospital discharge day. The average cost of perioperative blood transfusions was $431±27.4 in the cell saver group and $34.5±66.25 in the control group (p<0.001). CONCLUSION:The use of the CSS was not cost-effective, but it was particularly successful at reducing the rate and the number of units of postoperative allogenic blood transfusions.
ÖZETHafif ve orta derecede gonartrozun artroskopik debridman ile tedavisinin orta dönem sonuçları ve yaşam kalitesi üzerine etkileri Amaç: Çalışmamızda, en az 3 ay süreyle uygulanan tıbbi ve destek tedavilere yanıt vermeyen ağrısı olan, günlük yaşam aktivitelerinde ilerleyici kısıtlamalar ile karşılaşan, radyolojik olarak ileri derecede artrozu olmayan semptomatik diz osteoartritli hastalarda uygulanan artroskopik debridmanın ve eklem lavajının sonuçlarını ve yaşam kalitesi üzerindeki etkilerini değerlendirmeyi amaçladık. Gereç ve Yöntem: Çalışmamızı, 2004-2010 tarihleri arasında kliniğimize, 3 ay ve daha uzun süre ile uygulanan konservatif tedaviden fayda görmeyen diz ağrısı ile başvuran, modifiye ACR kriterlerine göre gonartroz tanılı, radyolojik olarak Kellgren-Lawrence kriterlerine göre 4. evreden düşük ve artroskopik debridman yapılan, 40 yaş ve üzerindeki, kontrollerine düzenli olarak gelmiş toplam 96 hasta ile yaptık. Hastalarımızın yaşam kalitesi, ağrı ve fonksiyonel değerlendirmelerini, ameliyat öncesi ve sonrası olacak şekilde SF-36 ve WOMAC puanlama indekslerini kullanarak değerlendirdik. İstatistiksel analiz için NCSS (Number Cruncher Statistical System) 2007 & PASS 2008 Statistical Software (Utah, USA) kullanıldı. Sonuçlar Student t test ve Mann Whitney U test ile değerlendirildi. Anlamlılık p<0.05 düzeyinde değerlendirildi.Bulgular: Çalışmamızda hastalarımızın fiziksel fonksiyon, sosyal fonksiyon, rol güçlüğü, mental sağlık ve total SF-36 puanlarının, ameliyat öncesi değerlere göre ameliyat sonrası takiplerinde istatistiksel olarak anlamlı düzeyde yükseldiğini; WOMAC puanlarının ise istatistiksel olarak anlamlı düzeyde düştüğünü bulduk (p<0.01). VKİ<30 ve VKİ≥30 olarak değerlendirdiğimiz iki grup arasındaki WOMAC ve SF-36 indekslerinin, ameliyat öncesi ve sonrasındaki puanları arasındaki farkın istatistiksel olarak anlamlı olmadığını tespit ettik (p>0.05). Hastaların takiplerinde 18 ay ve daha öncesindeki fiziksel fonksiyon, sosyal fonksiyon, mental sağlık ve total SF-36 puanlarının, 18 ay sonrasındaki puanlarına göre istatistiksel olarak anlamlı ölçüde yükselmiş olduğunu saptadık (p<0.01). 50 yaşın altındaki hastaların ameliyat öncesi ve sonrası fiziksel fonksiyon, mental sağlık ve total SF-36 puan farklarının, 50 yaş ve üzerindeki hastalara göre istatistiksel olarak anlamlı düzeyde daha fazla arttığını bulduk (p<0.01). Sonuç: Uygun seçilmiş hastalarda uygulanan artroskopik eklem debridmanı ve lavaj; ağrının giderilmesinde, fonksiyonların iyileştirilmesinde ve yaşam kalitesinin arttırılmasında ucuz, morbiditesi az olan uygulanılabilecek bir yöntemdir.
