Background: In 1992, the concept of female athlete triad was introduced to describe the interrelated problems of amenorrhea, eating disorders and osteoporosis seen in female athletes. To gain a clearer picture of amenorrhea/oligomenorrhea in Iran, one of the main components of the female athlete triad, we therefore established this study on the prevalence of amenorrhea/ oligomenorrhea in elite Iranian female athletes, also evaluating the risk factors of these disorders in the same population.
Background: Eating disorder inventory (EDI) is a self-reported questionnaire which has been used widely for the assessment of symptoms in clinical samples and research. Objectives: In the current study we sought to evaluate the validity and reliability of the Persian form of the 3rd variant of EDI (EDI-3) questionnaire. Methods: 452 individuals (299 women and 153 men) completed the EDI-3 questionnaire. The translation and back-translation procedure was adopted to provide the Iranian form of the questionnaire. A voluntary response sample of university students among different departments completed the survey in Tehran, Iran. Psychometric aspects of the instrument such as reliability (test-retest analysis and internal consistency), validity (a comparison of known groups) and item correlation with their speculated domain were evaluated.
ObjectiveWe compared autografts and allograft using partial and complete transphyseal anterior cruciate ligament (ACL) reconstruction techniques among skeletally immature individuals.MethodsMale and females younger than 18 and 16 years old, respectively, diagnosed with ACL tear from April 2006 to March 2012 entered the study. One group had four-strand hamstring autograft, and the other had tibialis posterior allograft reconstruction. Those who had allografts either had hyper-laxity or recurvatum.ResultsAchieved mean (± SD) 2000 International Knee Documentation Committee subjective score was not statistically different (P = 0.385) between allograft (n = 13) (84.3 ± 3.2) and autograft groups (n = 18) (85.6 ± 4.4). Mean Knee injury and Osteoarthritis Outcome Score (KOOS) subscale Knee-Related Quality of Life at 2 years was 78.0 ± 7.2 and 75 ± 7.4 for allograft and autograft groups, respectively (p = 0.261). Mean 2-year KOOS subscale Sports and Recreation was 82.1 ± 5.8 and 84.8 ± 6.6 for allograft and autograft groups, respectively (p = 0.244).No patient reported instability, giving way, or locking of the knee. Pivot shift test was negative in all patients; however, a minor positive Lachman test was found in six cases (46%) within the allograft group and seven cases (39%) in the autograft group. One postoperative septic arthritis was documented in the autograft group.ConclusionConsidering existing concern that joint laxity and recurvatum are among the precursors of non-contact ACL injury in adolescents, bone-patellar-bone autografts are not applicable in this age group because of the open physis; furthermore, considering that hamstring autografts are insufficient (size thickness and stretchability), we recommend soft tissue allografts for ACL reconstruction in skeletally immature patients.
Purpose: Oligo/amenorrhea, as a part of the Female Athlete Triad has adverse effects on the athlete's bone mineral density (BMD) and cardiovascular system. Hypoestrogenism, due to suppression of hypothalamus-pituitary axis (HPA) as a result of energy imbalance, is the possible cause of the Triad. This study was designed based on following up and reassessment of elite female athletes who were diagnosed as menstrual dysfunction about two years ago. Methods:This study was conducted in three phase sections: 1) Reassess the pattern of menstrual cycle among athletes who reported menstrual dysfunction about two years ago; 2) Bone mineral density was measured twice in the same machine and same center with a two-year interval; 3) The laboratory data including blood glucose, lipid profile and inflammatory markers was assessed in phase 3. Conclusion:Findings of this study show that negative changes in BMD and cardiovascular biomarkers of female athletes with functional hypothalamic menstrual dysfunction could occur if proper therapeutic intervention (including increase in calorie intake, decrease in exercise load or hormonal replacement) will not consider.
Background Studies evaluating the role of both corticosteroids and platelet-rich plasma (PRP) in the treatment of rotator cuff (RC) tendinopathies have been contradicting. We compared structural and clinical changes in RC muscles after corticosteroids and PRP injections. Methods This is a randomized double-blind clinical trial. All individuals with diagnosis of RC tendinitis during 2014–2017 were considered. Individuals were randomly allocated to either receive PRP or corticosteroids. Overall, 3cc of PRP was injected within the subacromial joint and another 3cc was injected at the site of the tendon tear, under the guide of sonography. For the corticosteroid group, 1cc of Depo-medrol 40mg and 1cc of lidocaine (2%) was injected within the subacromial joint. Results Overall, 58 patients entered the study. Comparison of pain, range of motion (ROM), Western Ontario RC (WORC), Disability of Arm-Hand-Shoulder (DASH) scores, and supraspinatus thickness showed significant improvement during follow-ups in both groups (p<0.05). During 3 months of follow-up, pain improvement was significantly better within the PRP group during (from 6.66±2.26 to 3.08±2.14 and 5.53±1.80 to 3.88±1.99, respectively; p=0.023). Regarding ROM, the PRP group had significant improvement in adduction (20.50°±8.23° to 28°±3.61° and 23.21°±7.09° to 28.46°±4.18° for the PRP and corticosteroid groups, respectively; p=0.011) and external rotation (59.66°±23.81° to 76.66°±18.30° and 57.14°±24.69° to 65.57°±26.39°, for the PRP and corticosteroid groups, respectively; p=0.036) compared to the corticosteroid group. Conclusion We found that PRP renders similar results to that of corticosteroids in most clinical aspects among patients with RC tendinopathies; however, pain and ROM may show more significant improvement with the use of PRP. Considering that the use of corticosteroids may be contraindicated in some patients and may be associated with the risk of tendon rupture, we suggest the use of PRP in place of corticosteroid-based injections among patients with RC tendinopathy. The mainstay of treatment for anterior cruciate ligament (ACL) tear is considered to be surgery, especially when associated with medial collateral ligament (MCL) tears. We aimed to evaluate our hypothesis that some patients with concomitant ACL and MCL tears may develop spontaneous healing without surgical intervention. This study was conducted during 2013 to 2017. A total of 707 patient referred with combined ACL and MCL injuries. Patients were divided into three groups according to type of ACL and MCL management as follows: (1) group 1 as those who only had ACL reconstruction without any surgical treatment of MCL; (2) group 2 as those who had ACL reconstruction and MCL surgery (reconstruction, reefing, or both); (3) group 3 as those who showed spontaneous healing of ACL and MCL. Overall, 206 and 129 patients entered groups 1 and 2, respectively. Overall, 15 patients showed spontaneous healing of ACL and entered group 3. Skiing and soccer were the most common causes of injury within the third group, followed by traffic accidents (35.7%, 35.7% and 21.4%, respectively). All these were noncontact injuries within this groups. In group 3 only three patients had concomitant meniscal injury. Mean healing time for patient with spontaneous healing was 8.66 ± 3.41 months. All patients showed ACL tears at the proximal part of its attachment. During follow-up, all pivot shift, Lachman and MCL tests were either negative or one plus. All these individuals returned to previous physical activity. Among the total number of patients with skiing injuries presenting with concomitant ACL/MCL injuries (27 patients), five patients (18.5%) showed spontaneous healing. Valgus and external rotation injuries with dominancy of valgus force, apart from a MCL tear, may lead to proximal tear of ACL as well, and the inflammation from the torn MCL can potentiate and stimulate the healing process of ACL; thus, patient with this mechanism of injury is better followed before surgical intervention is planned as spontaneous healing may occur.
Although the prevalences of clinical functional hypothalamic menstrual disorders and stress fracture were less than those reported in some other countries, athletes in the high-risk group had significantly more stress fractures than those in the low-risk group. Future studies should focus on screening, diagnosing, preventing, and treating all components of the newly defined Triad, especially in high-risk sports in Iran.
