Ankara Atatürk Eğitim ve Araştırma Hastanesi, Anestezi ve Reanimasyon Kliniği, Ankara, TürkiyeAim: Characteristics, need for organ support therapies and lengths of stay of surgical and medical patients in a mixed intensive care unit (ICU) were compared for effective utilization of ICU beds in mixed ICU's. Material and Methods: The prospectively collected data of 154 patients who were cared for in our mixed ICU between 01.03.2010 and 31.07.2010, were retrospectively evaluated. The patients were grouped into two as medical and surgical patients. Trauma patients were included in the surgical patients group. The demographic data, APACHE II scores, invasive procedures performed, ICU and hospital lengths of stay (LOS) were analysed using the Mann-Whitney U, Chi-square, student's t-test and Spearman's correlation test. Results: Of the 154 patients, 55 (35.7%) were medical and 99 (64.3%) were surgical patients. Of the surgical patients, 7 (7.1%) were non-operated patients admitted after trauma. Groups were similar in age, sex, APACHE II scores, and expected mortality rates. The number of intubated patients, patients needing invasive hemodynamic monitorization, transfusion or central venous catheterisation were similar in both groups. However, in the medical patients group, the need for hemodialysis or tracheostomy was significantly higher (p<0.001, p=0.039). Also, the medical patient ICU LOS, ICU and hospital mortality were significantly higher than the surgical group (p<0.001, p=0.002). There was a weak, positive, statistically significant correlation between ICU LOS and APACHE II score (r=0.261; p=0.007). Conclusion: Even though the APACHE II scores may be similar, medical patients required organ support systems more often, and their LOS and mortalities were higher compared to the surgical patients in our ICU. During the planning of ICU admissions, keeping this knowledge in mind will optimize utilization of the ICU beds. (Yoğun Bakım Derg 2012; 1: 5-8)

İnternetin 1990'lı yılların sonuna doğru insanların yaşamına girmesi ile dünyanın herhangi bir yerindeki bir cihazla başka bir cihazın birbirleriyle iletişim kurması mümkün hale gelmiştir. İnternet teknolojisinin 2000'li yılların başında olağanüstü gelişimini akıllı mobil teknolojilerinin (akıllı telefon, saat, gözlük ve diğer düşük güçlü giyilebilir ve takılabilir cihazlar) büyük bir hızla gelişmesi takip etmiştir. Bu akıllı mobil teknolojilere entegre edilen sensörlerden faydalanılarak bireyin bulunduğu ortamdan birçok farklı verinin elde edilmesi sağlanmıştır. Elde edilen bu veriler, kablolu veya kablosuz olarak internet yoluyla bir merkezde toplanıp, incelenip, analiz edilmiştir. Bu sayede cihaza sahip kişi veya cihazın bulunduğu ortam hakkında çeşitli bilgilere kısa sürede ulaşılmıştır. Yaşanan bu gelişmeler internet üzerinden nesnelerin birbiriyle haberleşmesi(IoT) fenomenini ortaya çıkarmıştır. IoT ile ilgili çok kapsamlı araştırmalar ve uygulamalar günümüzde çeşitli alanlarda devam etmektedir. IoT' un en çok kullanıldığı alanlardan birisi de sağlık hizmetleri alanıdır. Hastalıkların doğru teşhisi, tedavisi ve takibinde özellikle hastanın hastane dışındaki günlük yaşantısından alınacak veriler büyük bir önem taşımaktadır. Bu verileri elde etmenin en iyi yolu IoT giyilebilir veya takılabilir sağlık cihazlarını kullanmaktır. Bu çalışmanın amacı, şimdiye kadar yapılan IoT tabanlı geleneksel ve akıllı sistem olarak yapılan giyilebilir ve takılabilir sağlık cihazı uygulamlarından elde edilen bulguları özetlemektir. Bu bulgular ışığında da IoT tabanlı uygulamaların geleceği hakkında temel sorunları ele alarak çeşitli öneriler getirmektir.

El objetivo principal fue describir las alteraciones respiratorias y hemodinámicas y las complicaciones relacionadas con la presión intraabdominal elevada y con la posición de Trendelenburg profunda en pacientes sometidos a prostatectomía robótica. El objetivo secundario fue revelar criterios seguros de alta del quirófano. Métodos: Cincuenta y tres pacientes sometidos a prostatectomía robótica entre diciembre de 2009 y enero de 2011 fueron incluidos en un estudio prospectivo. Las principales medidas de resultado fueron: monitorización no invasiva, monitorización invasiva y gasometría realizada en decúbito dorsal (T 0 ), Trendelenburg (T 1 ), Trendelenburg + neumoperitoneo (T 2 ), Trendelenburg predesinsuflación (T 3 ), Trendelenburg posdesinsuflación (T 4 ) y posiciones supinas (T 5 ). Resultados: Cincuenta y tres pacientes sometidos a prostatectomía robótica fueron incluidos en el estudio. El principal reto clínico en nuestro grupo de estudio fue la elección de la estrategia de ventilación para controlar la acidosis respiratoria, que es detectada por medio de la presión de dióxido de carbono espirado y la gasometría. Además, la presión arterial media permaneció inalterada, y la frecuencia cardíaca disminuyó significativamente y fue necesario intervenir. Los valores de la presión venosa central también estaban por encima de los límites normales. Conclusión: La acidosis respiratoria y los síntomas clínicos «similares a la obstrucción de las vías aéreas» fueron los principales desafíos asociados con los procedimientos de prostatectomía robótica.

Respiratory acidosis and "upper airway obstruction-like" clinical symptoms were the main challenges associated with robotic prostatectomy procedures during this study.
