Zusammenfassung Refrakturen an den Extremit?tenknochen nach Materialentfernung sind seltene, aber ernst zu nehmende Komplikationen, die in den meisten F?llen einen weiteren Eingriff notwendig machen. In einer retrospektiven Analyse der eigenen F?lle sowie unter Ber?cksichtigung der vorhandenen Literatur zeigt sich, dass durch eine vollst?ndige pr?operative Diagnostik, durch Einhalten eines ausreichenden Intervalls von initialer Osteosynthese bis zur Materialentfernung, durch zur?ckhaltende Belastung des vorgesch?digten Knochens direkt nach der Materialentfernung und unter Ber?cksichtigung der Art der Frakturheilung die Komplikationsrate vermindert werden kann. Jedoch ist eine vollst?ndige Vermeidung kaum m?glich, da oft der ausdr?ckliche Wunsch des Patienten zur Materialentfernung vorhanden ist und biomechanisch eine Minderbelastbarkeit durch die residuellen Schraubenl?cher und die Demineralisationszonen im Bereich der ehemaligen Frakturzonen zumindest f?r einige Wochen postoperativ besteht. Entsprechend sollte in einigen F?llen die Empfehlung zum Verbleib des Osteosynthesematerials ausgesprochen werden.
Zusammenfassung Beckenringfrakturen geh?ren zu den seltenen Verletzungen. In den letzten Jahren erfolgt die Stabilisierung des hinteren Beckenrings ?berwiegend perkutan mittels transiliosakraler Schraubenfixation. Der vordere Beckenring wird in Abh?ngigkeit vom Verletzungsmuster mit Platten, Schrauben oder supraazetabul?r eingebrachten Fixateur-externe-Montagen stabilisiert. Anhand der Literatur sowie der retrospektiven Aufarbeitung der eigenen Ergebnisse und Erfahrungen bei 80 Patienten mit einer Beckenring-B- und -C-Verletzung wird aufgezeigt, dass die Indikation zur Entfernung des Osteosynthesematerials streng zu stellen ist. Die Indikation ist fraglos gegeben bei Fixateur-externe-Montagen, Infektionen, Schraubenfehllagen, allergischen Reaktionen auf Implantate, kritischer Weichteilbedeckung bzw. -kompromittierung durch auftragende Implantate und bei verheilten Frakturen am wachsenden Skelett. Eine individuelle Abw?gung der Risiken und Nutzen ist jedoch beim asymptomatischen Patienten sowie beim aufgrund von posttraumatischen Ver?nderungen symptomatischen Patienten erforderlich. Hier sollte bei der Entscheidung das initiale Verletzungsmuster mitber?cksichtigt werden. Wird die Indikation zur Materialentfernung nach einer Beckenringfraktur gestellt, so kann es sich um eine technisch anspruchsvolle Operation mit einer hohen Komplikationsrate handeln.
