Our results indicate that laparoscopic surgery provides favourable outcomes even when used outside controlled trials and should be considered as a standard treatment for patients with colon cancer.
Background One critical aspect for improvement of health care delivery to the population is a continuous monitoring of results. In this context, an increasing number of clinical cancer registries has been implemented in Germany since 2013. Main aspects of these registries include quality control, assessment of clinical outcome, support of epidemiological registries and potential implementation into clinical follow up. The ADT (Cooperation of German Cancer Centres e.V.) aims to merge data from local cancer registries into one nation-wide registry. In this context, we were able to analyse for the first time data of > 100.000 patients with malignant diseases of the stomach and esophagus. Methods Data from all local cancer registries in Germany were requested in the context of the National German Cancer Conference 2018 hosted by the German Cancer Society (DKG). After preparation of data, analysis included demographic and treatment aspects, and esophageal/gastric/GOJ cancers were analysed separately. Results From 2000–2016, 101.559 patients with cancers of the esophagus and stomach have been reported, representing approximately one quarter of the expected cancer cases in this time frame in Germany. Data quality and completeness improved over time. Incidences of esophageal and GOJ cancers increased, whereas incidence of gastric cancer decreased. Median age differed between entities with highest respectively lowest age in gastric/esophageal cancer. Further, median age increased in all three tumor types in the study period. Treatment pattern changed over time with a distinct increase of the use of neoadjuvant therapy in all cancer types. Percentage of curative intended surgery increased over time, reaching roughly 60% in gastric cancer, about 50% in GOJ and esophageal adenocarcinoma, and slightly less then 25% in esophageal squamous cell carcinoma. Finally, analysis of tumor stage dependent treatment confirmed higher rates of curative intended surgical treatment in early tumor stages, and an increased use of neoadjuvant treatment or chemo/radiotherapy approaches in higher tumor stages. Conclusion Combined Clinical Cancer Registries provide the unique opportunity to assess clinical relevant data of a large population based cohort of patients for purposes such as quality control and outcome measurement. Further, subgroup analyses can provide valid information on clinically relevant questions such as outcome/quality of different treatments. Einführung Krebs ist nach Herz-Kreislauf-Erkrankungen die zweithäufigste Todesursache in Deutschland. Nach aktuellen Zahlen des Robert Koch-Instituts wurden im Jahr 2008 470 000 Neuerkrankungen registriert. Im selben Jahr starben 215 000 Menschen an Krebs [1]. Durch Initiierung des Nationalen Krebsplans 2008 durch das Bundesministerium für Gesundheit (BMG), die Deutsche Krebsgesellschaft e. V. (DKG), die Deutsche Krebshilfe e. V. (DKH) und die Arbeitsgemeinschaft deutscher Tumorzentren e. V. (ADT) wurde eine umfassende, interdisziplinäre und interprofessionelle Aufarbeitung des Themas Onkologie möglich. Mit seinen 4 Handlungsfeldern, 40 Zielen und über 100 Empfehlungen wurde auf breiter Basis sowohl die Früherkennung, die Qualitäts-sicherung, die Datensparsamkeit, v. a. aber auch die Patientenperspektive in den Mittelpunkt gestellt [2]. Das zunächst fertiggestellte Ziel 8 des Handlungsfeldes 2 (flächendeckende Implementierung regionaler klinischer Krebsregister) führte zum ersten fassbaren Ergebnis, dem Krebsfrüherken-nungs-und -registergesetz (KFRG § 65c SGB V) [3]. Bis Ende 2017 müssen in Deutschland flächendeckend in allen Bundesländern regionale klinische Krebsregister und eine Landesstelle bestimmt sein, eine Vielzahl spezifischer Kriterien nachgewiesen und alle Tumorentitäten vollzählig und vollständig erfasst werden, um von gesetzlichen und privaten Krankenkassen finanziert zu werden (Förderkriterien GKV Spitzenverband) [4, 5]. Um dies flächendeckend zu erreichen, müssen die bestehenden zertifizierten Organkrebszentren und Onkologischen Zentren der DKG sowie Onkologischen Spitzenzentren der DKH, alle Krankenhäuser und niedergelassenen Ärzte mit den regionalen Krebsregistern eng zusammenarbeiten. Grundlagen und Methoden Im KFRG § 65c SGB V sind folgende 8 Aufgaben klinischer Krebsregister festgelegt [3]: 1. die personenbezogene Erfassung der Daten aller in einem regional festgelegten Einzugsgebiet stationär und ambulant versorgten Patientinnen und Patienten über das Auftreten, die Behandlung und den Verlauf von bösarti-gen Neubildungen einschließlich ihrer Frühstadien sowie von gutartigen Tumoren des zentralen Nervensystems nach Kapitel II der Internationalen statistischen Klassifikation der Krankheiten und verwandter Gesundheitsprobleme (ICD) mit Ausnahme der Daten von Erkrankungsfällen, die an das Deutsche Kinderkrebsregister zu melden sind, 2. die Auswertung der erfassten klinischen Daten und die Rückmeldung der Auswertungsergebnisse an die einzelnen Leistungserbringer, 3. der Datenaustausch mit anderen regionalen klinischen Krebsregistern bei solchen Patientinnen und Patienten, bei denen Hauptwohnsitz und Behandlungsort in verschiedenen Einzugsgebieten liegen, sowie mit Auswertungsstellen der klinischen Krebsregistrierung auf Landesebene, 4. die Förderung der interdisziplinären, direkt patientenbezogenen Zusammenarbeit bei der Krebsbehandlung, 5. die Beteiligung an der einrichtungsund sektorenübergreifenden Qualitäts-sicherung des Gemeinsamen Bundesausschusses nach § 137a SGB V 6. die Zusammenarbeit mit...
