of 12 M (3-84 M). Children with bloody diarrhoea acquired tolerance earlier, median age 8 M (5-23 M), than the group with cutaneous symptoms, median age 14 M (4-84 M), p = 0.005.It's safe and beneficial, both for children and parents, to perform OFC from 6 to 12 M.
Background The purpose of this study was to compare the clinical symptoms and pathological consequences of Helicobacter pylori (H. pylori) infection between children and adults and determine the levels of expression of FOX3P and IL‐17A to examine the Th17/Treg balance. Methods Forty pediatric and 40 adult patients who were followed up at the Pediatric Gastroenterology and Internal Medicine Gastroenterology Departments were enrolled in the study. In our case‐control study, gastric tissue specimens were evaluated using the updated Sydney system, and the number of cells expressing FOXP3/IL‐17A (Treg and Th17 cell markers) was analyzed immunohistochemically. In addition, each case was evaluated using a clinical follow‐up questionnaire. Results Clinical signs and symptoms of children and adults were similar. IL‐17A and FOXP3 levels were significantly higher in children and adults with H. pylori (+) than in those without H. pylori (−) (p < .001). In patients with H. pylori (+), the mean FOXP3 level was significantly higher, whereas the mean IL‐17A level was significantly lower in children than in adults (p < 0001 for both groups). In children with H. pylori (+), bacterial density was negatively correlated with IL‐17A level and positively correlated with FOXP3 level. In adults with H. pylori (+), there was a statistically significant, highly positive correlation between bacterial density and levels of IL‐17A and FOXP3. Conclusions Treg cells are suggested to more predominant in children than in adults, IL‐17A levels decrease as H. pylori bacterial density increases. In conclusion, immune responses incline toward Treg, which increases the susceptibility to persistent infections.
Helicobacter pylori (H.pylori), toplumda en sık görülen patojenlerden biri olup, tüm dünya nüfusunun yarısından fazlasını etkilemektedir (1). Gastrit ve peptik ülserin oluşumunda önemli bir etken olup uzun dönemde lenfoma ve adenokarsinoma yol açmaktadır (2). Ağır klinik tablo genelde erişkinlerde görülmekle beraber enfeksiyon çocukluk çağında kazanılmakta, bu da erken yaşta tanının önemini arttırmaktadır (3). Ancak çocukluk çağında H. pylori enfeksiyonunun, genelde karın ağrısı, dispepsi, bulantı gibi özgün olmayan semptomlarla kendini ortaya koyması tanı koymayı güçleştirmektedir. Bu araştırmada, H.pylori ile enfekte olan pediatrik hastaların klinik, laboratuvar, endoskopik ve histopatolojik bulguları retrospektif olarak değerlendirilmiştir. Bu çalışma ile H. Bu da erken yaşta tanının önemini arttırmaktadır. Bu araştırmanın amacı, çocuklarda H.pylori enfeksiyonunu yaş, cinsiyet, tanı testleri, klinik ve histopatolojik bulgularla değerlendirmektir. Gereç ve yöntem: Merkezimizde üst gastrointestinal sistem (ÜGS) endoskopisi yapılan ve biyopsi örneklerinde H. pylori saptanan çocuk hastaların yaşları, cinsiyetleri, geliş şikayetleri, fekal H.pylori antijen sonuçları ve histopatolojik bulguları retrospektif olarak değerlendirildi. Bulgular: Çalışmaya alınan 148 hastanın 111'i (%75) kız, 37'si (%25) erkekti. Yaş ortalaması 14.2 yıl (1-18 yıl, ortanca 15 yıl) idi. Hastaların %60,1'i karın ağrısı, %9,5'i bulantı, %2'si ise retrosternal yanma şikayeti ile başvurdu. H. pylori ve inflamasyon yoğunluğu arasında pozitif korelasyon bulundu (p<0.001). Fekal H.pylori antijen testi duyarlılığı %21,4 bulundu. Sonuç: Çocuklarda H.pylori enfeksiyonu, karın ağrısı ve bulantı gibi nonspesifik bulgularla görülebilmektedir. Tanı için invaziv yöntemlere ihtiyaç vardır.
Introduction:Although severe forms of the disease are seen in adults, the Helicobacter pylori (H. pylori) infection is usually acquired during childhood. Therefore, prompt diagnosis of H. pylori infection in childhood is of prime importance. In the present study, we aimed to identify the sensitivity and specificity of 13C urea breath test (UBT) in the diagnosis of H. pylori infection and to assess the severity of the disease with delta over baseline (DOB) values. Materials and Methods: UBT was administered to 200 children who presented with epigastric pain and/or nausea. Esophagogastroduodenoscopy (EGD) was performed in patients. UBT results were compared with histopathological findings. In the Area Under the ROC Curve analysis, a cutoff DOB value was found for the severity of H. pylori infection. Results: 193 children with a mean age of 13.50±2.98 years were included in the analysis. Of these, 71 (36.8%) patients had a positive UBT and 122 (63.2%) had a negative UBT. EGD was performed in 60 out of 71 patients with positive UBT and in 30 out of 122 patients with negative UBT. The sensitivity and specificity of UBT were 85.1% and 100%. DOB was found to be a significant predictor of moderate/ marked H. pylori density at a cutoff value of 6%. Conclusions: The positive correlation detected between the level of DOB values and the density of H. pylori and inflammatory activity can be an advantage for detecting the severity of disease Öz Giriş: Yetişkinlerde ağır formları görülmekle birlikte, Helicobacter pylori (H. pylori) enfeksiyonu genellikle çocukluk çağında kazanılır. Bu nedenle, çocukluk çağında H. pylori enfeksiyonunun erken tanısı çok önemlidir. Bu çalışmada, H. pylori enfeksiyonu tanısında 13C üre nefes testinin (ÜNT) duyarlılığını ve özgüllüğünü bulmayı, delta değeri (DOB) ile hastalığın şiddetini değerlendirmeyi amaçladık. Gereç ve Yöntem: Epigastrik ağrı ve/veya bulantı şikayeti ile başvuran 200 çocuğa UBT uygulandı. Hastalara özofagogastroduodenoskopi (ÖGD) yapıldı. ÜNT sonuçları histopatolojik bulgularla karşılaştırıldı. ROC eğrisi analizi ile H. pylori enfeksiyonunun şiddeti için bir belirleyici DOB değeri bulundu. Bulgular: Yaş ortalaması 13,50±2,98 yıl olan 193 hasta araştırmaya alındı. Bu hastaların 71'i (%36,8) ÜNT pozitif, 122'si (%63,2) negatif olarak tespit edildi. ÜNT pozitif olan 71 hastanın 60'ına, ÜNT negatif olan 122 hastanın 30'una ÖGD yapıldı. ÜNT'nin duyarlılığı %85,1, özgüllüğü %100 tespit edildi. Orta/belirgin H. pylori yoğunluğu için DOB kesim değeri %6 olarak bulundu. Sonuç: DOB değerleri ile H. pylori yoğunluğu ve enflamatuar aktivite arasındaki pozitif korelasyon, hastalığın şiddetini tespit etmek için bir avantaj olabilir. Aim: This study aimed to present clinical and laboratory features of 41 children with Wilson's disease. Material and Methods: The medical records of all of the patients who had got a diagnosis of Wilson's disease between 2001 June and 2005 March in Ankara Dr. Sami Ulus Training& Research Hospital, Turkey. Demographic, clinical and biochemical information was obtained from the patients' records. Findings were retrospectively analyzed by the SPSS Windows 16.0 (SPSS Inc. IL, USA) statistical software. Results: A total of 41 patients had got the diagnosis of Wilson's disease: 24 boys and 17 girls, between in the range of 3-14 years old. The mean age of patients was 9.05±2.84 years. Kayser-Fleischer rings were observed in 24 patients. Urinary copper excretion in 24-hours urine was high in 39 of 40 patients. Serum ceruloplasmin levels were found low in 35 of 40 patients. Thirteen of patients were diagnosed after the family screening. Pathologic brain MR findings were detected in 4 of 8 patients without neurological system complaints or physical examination findings. Conclusion: Especially in societies that consanguineous marriages are so common, Wilson's disease should be considered in differential diagnosis of chronic liver diseases, prolonged hypertransaminasemia, and degenerative brain disorders of unknown origin. In our study, it has been shown that Wilson's disease diagnosis and follow-up preserved the value of classical diagnostic methods and it has been shown that neuroimaging may be useful for early detection of neurological involvement even if neurological findings do not occur.
