Lymphoscintigraphy was performed in 128 patients with primary lymphedema, in 91 patients with secondary lymphedema, and in 19 healthy volunteers. A total of 457 extremities were examined. Technetium-99m-labeled human serum albumin was injected subcutaneously, and passive muscular exercise was standardized to reduce the variability of lymphatic function. The scintigrams were analyzed for visualization of lymph vessels and lymph nodes, dilatation of lymphatic vessels, existence of collateral vessels, and dermal backflow. With this qualitative interpretation alone, the diagnosis of lymphedema was established in 216 of 308 extremities (70.1%). Quantitative parameters derived from clearance data showed abnormal lymphatic function in all 308 extremities. Whereas qualitative lymphoscintigraphy allows the characterization of lymphatic morphology, quantitative lymphoscintigraphy is very accurate in detection of incipient lymphedema.
Management of Lymphoedema is extremely variable in the medical profession, ranging from aggressive diagnostic procedures, which may aggravate the disease (lymphography by oily contrast mediums) in combination with therapeutic nihilism to precipitous operations. Scattered between these two contraversial extreme positions are quite a number of various conservative methods - some of which being in conflict with the anatomy and physiology of the lymph vascular system and the pathology of Lymphoedema. Lymphoedema, if left untreated, or is treated inadequately, may lead to invalidity and to death if Stewart-Treves Syndrome develops. Unnecessary surgery may have disastrous consequences. The paper describes how Lymphoedema can be treated with success, free of any side effects, by complex physiotherapy.
Interstitial lymphangiography performed with intradermal injection of a new nonionic dimeric contrast agent was used to examine 34 normal lower extremities and 35 lower extremities with proved lymphedema. An infusion pump was used to inject 2 mL of the contrast agent into the intradermal interdigital space at a rate of 0.1 mL/min. In all normal extremities, the injection deposit appeared homogeneous, and one to five lymph vessels could be identified. Diffusion of contrast agent into perilymphatic tissue occurred 30-40 cm proximal to the injection site; inguinal lymph nodes were, therefore, not routinely visualized. The findings at interstitial lymphangiography were abnormal in all extremities with advanced lymphedema. The results indicate that interstitial lymphangiography with the use of a nonionic dimeric contrast agent allows morphologic assessment of lymphatic vessels and facilitates the diagnosis of lymphatic obstructive disease.
ZusammenfassungFragestellung: Die Lymphszintigraphie wird fast ausschließlich als qualitative Untersuchung durchgeführt. Diese Tatsache erfüllt nicht die lymphologische Forderung nach einer Funktionsdiagnostik zur Abklärung lymphostatischer Ödeme der Extremitäten. Eine zusätzliche quantitative Untersuchung ist deshalb unbedingt erforderlich. In diesem Zusammenhang wurde die Notwendigkeit einer Schwächungskorrektur und Methoden der Lymphknotentiefenbestimmung sowie die Verbesserung der körperlichen Belastung während der Untersuchung bestimmt und ein standardisiertes Untersuchungsprotokoll erarbeitet. Methoden: Subkutane Injektion (in Fuß oder Hand) von 37 MBq 99mTc-Nanokolloid bei 924 Patienten. Kontinuierliche Erfassung des Uptakes in den regionalen Lymphknoten während körperlicher Belastung (1 h) mit unterschiedlichen Belastungsverfahren. Bei der Uptakeberechnung wird eine Schwächungskorrektur durchgeführt; die Ergebnisse nach Lymphknotentiefenbestimmung mit SPECT (Single Photon Emission Computed Tomography) werden im Vergleich mit der sonographischen Tiefenbestimmung sowie mit Uptakeberechnung ohne Schwächungskorrektur diskutiert. Ergebnisse: Der Vergleich der Belastungsformen zeigt, dass nur das kontrollierte Gehen eine zuverlässige Belastung gewährleistet. Ein Vergleich der Uptakewerte nach sonographischer Lymphknotentiefenbestimmung gegenüber der SPECT ergibt beträchtliche Fehler bei der sonographischen Methode. Der Verzicht auf eine Schwächungskorrektur schließt eine quantitative Untersuchung aus. Die genaueren Messverfahren eröffnen neue pathophysiologische Aspekte beim Lipödem und Lipolymphödem. Schlussfolgerungen: Kontrolliertes Gehen und tiefenkorrigierte Lymphknotenuptakewerte sind unverzichtbare Voraussetzungen für zuverlässige Ergebnisse bei der Funktionslymphszintigraphie.
