Cyclosporin A is a selective immunosuppressant, used in organ transplants to prevent graft rejection. Cyclosporin A can cause various side effects including gingival overgrowth. The aim of this work was to evaluate gingival overgrowth of rats treated daily with 10 mg/kg bodyweight of cyclosporin A for 60 days, as well as the regression after the interruption of treatment. All rats treated with cyclosporin A developed gingival overgrowth, with increased thickness of the epithelium, height and width of the connective tissue. The density of fibroblasts and collagen fibers also increased. Five to 90 days after the interruption of treatment with cyclosporin A, there was a progressive reduction of the gingival volume and of collagen fibers and fibroblast densities. The reduction was more pronounced in the initial periods and after 90 days did not return to the normal values.
We report the unusual simultaneous occurrence of lymphoepithelial cysts, cytomegalovirus (CMV) and mycobacterial infections in the intraparotid lymph nodes of a 52-year-old AIDS patient who died of disseminated mycobacteriosis. Although cytomegalovirosis is a common finding in the salivary glands of HIV patients, the association of CMV inclusions with lymphoepithelial cyst (LC) has not been previously reported. Parotid mycobacterial infection is an uncommon finding, despite its usual disseminated presentation in HIV patients. These data emphasize that in immunosuppressed patients, simultaneous diseases of the parotid gland may occur and should be considered for diagnosis and treatment.
La enfermedad de Behcet es un trastorno inflamatorio multisistémico vasculítico de vaso variable caracterizado por úlceras aftosas orales, genitales, recurrentes, panuveítis bilateral posterior, manifestaciones articulares y ocasionalmente compromiso del sistema nervioso central y grandes vasos . Las variadas manifestaciones clínicas y la falta de pruebas patognomónicas hacen de la enfermedad un reto diagnóstico cuya prevalencia en Colombia se desconoce. El diagnostico se basa en criterios diagnósticos propuestos ante la falta de una prueba Gold Standard. Con relación a la etiología se han estudiado diversas teorías, entre ellas las de origen inmunológico, especialmente por la existencia del HLA - B51, le siguen aquellas que responsabilizan a agentes infecciosos, daño endotelial, entre otros. Se presenta el caso de un paciente femenino de 35 años, con cuadro crónico y recurrente de aftas orales, presencia de lesiones dermatológicas y aparición de ulceras genitales. Descartándose causas inmunomediadas, infecciosas e inflamatorias. Considerándose un cuadro de aftosis bipolar recurrente y ante los síntomas dermatológicos, se propuso aplicar los criterios diagnósticos del International Team for the Revision of International Criteria for Behçet Disease, con una puntuación de 4, lo que avala el diagnostico de enfermedad de Behcet. El manejo se basó en la instauración de medidas terapéuticas a base de corticosteroides sistémicos e inmunosupresores, con adecuada respuesta clínica.
Introducción: El término “Síndrome de sobreposición” se refiere a una de las formas variantes de hepatitis autoinmune, en la cual se encuentran hallazgos clínicos y serológicos de hepatitis autoinmune con relación a las características de ciertas enfermedades hepáticas colestásicas como la colangitis biliar primaria (CBP) y la colangitis esclerosante primaria (CEP). Objetivo: El reconocimiento temprano de estas variantes es clínicamente importante desde el punto de vista del tratamiento y pronóstico. Metodología: Se expone un caso, correspondiente a un paciente femenino de 66 años de edad, con cuadro crónico de prurito generalizado, en ausencia de lesiones dermatológicas, con hipertransaminasemia y patrón colestásico, positividad para anticuerpos antimitocondriales y anti-musculo liso junto con elevación de las concentraciones de inmunoglobulina G (IgG). Conclusiones: El estudio histopatológico reportó actividad necroinflamatoria en ausencia de expansión fibrosa, lo que supone buen pronóstico, con instauración de medidas terapéuticas que incluyen ácido ursodexosicolico, corticosteroides sistémicos e inmunosupresores, con adecuada respuesta clínica.
