Einleitung und FragestellungDie pathologischen Veränderungen, welche bei intracardialen Operationen während der Anwendung eines Pumpoxygenators auftreten können, sind in den letzten Jahren häufig Gegenstand ausgedehnter Untersuchungen gewesen. Die Berücksichtigung der hierbei gewonnenen Erkenntnisse hat zusammen mit der Weiterentwicklung leistungs fähiger Herz-Lungen-Maschinen dazu geführt, daß man heute während einer künst lichen Perfusion weitgehend physiologische Verhältnisse aufrechterhalten kann. Eine in solcher Weise durchgeführte Perfusion gewährleistet darüber hinaus, daß auch nach einer künstlichen Durchströmung die meisten Organ-und Stoffwechselfunktionen unverändert ablaufen. Die postoperativ durchgeführten Untersuchungen lassen bisher aber noch manche Fragestellung offen. So ist auch wenig darüber bekannt, inwieweit die Lunge, nachdem sie während der extrakorporalen Zirkulation weit gehend aus dem allgemeinen Kreislauf ausgeschaltet war und nur noch über das Bronchialarteriensystem versorgt wurde, nach der Perfusion in der Lage ist, den Gasaustausch wieder in normaler Weise zu übernehmen.Postoperative Lungenkomplikationen waren in der Anfangszeit der Verwendung extrakorporaler Zirkulation ein recht häufiges Ereignis. Sie sind seltener geworden, seit man gelernt hat, eine Überfüllung des kleinen Kreislaufs während des totalen bypass und insbesondere nach Aufhebung eines induzierten Herzstillstandes zu ver meiden. Das Fehlen einer klinisch erfaßbaren Lungenkomplikation bedeutet jedoch noch nicht, daß die Funktion der Lunge völlig unbeeinträchtigt sein muß, also der pulmonale Sauerstoff-und Kohlensäureaustausch wirklich ungestört abläuft.Einen Hinweis darauf, daß Störungen des Gasaustausches nach extrakorporaler Zirkulation ohne röntgenologisch faßbare Veränderungen der Lunge vorhanden sein können, gaben uns Untersuchungen über das Verhalten des arteriellen Sauerstoffund Kohlensäuredrucks in den ersten postoperativen Tagen. Da sich hierbei in einem hohen Prozentsatz der Untersuchten pathologische Blutgas werte fanden, ergab sich die Fragestellung, welche Teilfunktionen der Lunge hierbei ursächlich beteiligt waren. Wir führten deshalb bei einer Reihe von Patienten vor und nach extrakorporaler Zirkulation eine eingehende Lungenfunktionsprüfung mit spezieller Methodik durch. Von den hierbei gefundenen, z. T. erheblichen Störungen konnte jedoch nicht ein deutig gesagt werden, ob sie allein Folge der künstlichen Perfusion waren oder ob sie z. T. auch auf postoperativen Veränderungen beruhten, wie sie nach jeder Thorakoto mie auftreten können.
