Die Operation der Mitralstenose ist heute vom chirurgisch-technischen Standpunkt kein Problem mehr. Mit zunehmender Erfahrung ist die Mortalität in den meisten Kliniken mit größerem Krankengut bereits unter 10 % gesunken. Auch die Indikationsstellung ist so fest umrissen, daß sie eigentlich nur noch unter dem Gesichtspunkt einer fraglichen Fixation des pulmonalen Hypertonus wirklich problematisch ist. (4). Nachdem etwa 3 000 bis 4 000 Fälle inzwischen in der ganzen Welt operiert sein dürften, wird jetzt die Frage, was aus diesen operierten Kranken auf lange Sicht wird, aktuell.Klinische Beobachtungen reichen schon bis zu 5 Jahren und mehr zurück und die Ergebnisse sind überwiegend günstig. Vergleichende und wiederholte Nachuntersuchungen mit dem Herzkatheter liegen aber nur sehr vereinzelt über größere Zeitabstände hin vor. Wir hatten bisher Gelegenheit, 3 von unseren 26 operierten Patienten mit Mitralstenosen zum dritten Mal mit dem Herzkatheterismus nach 2-2½ Jahren nachzuuntersuchen. Die einzelnen Werte wiesen gegenüber den Untersuchungen, die 3 Monate nach der Operation durchgeführt wurden, keine wesentlichen Veränderungen mehr auf. Es ist daraus zu schließen, daß die erweiterte Mitralklappe sich nicht erneut verengt hat. Dies ist neben den positiven klinischen Beobachtungen auch aus einigen in der Literatur berichteten postmortalen Untersuchungen operierter Mitralstenosen zu entnehmen.Bei diesen Fällen liegen jedoch keine entsprechenden Herzkatheteruntersuchungen vor. Wir glauben deshalb berechtigt zu sein, über einen von uns beobachteten, leider tragisch verlaufenden Fall, etwas ausführlicher berichten zu können. Es handelte sich dabei um eine 24 jährige Frau, die bei uns 2½ Jahre nach der Valvotomie an einer Pneumonie verstarb und die bereits zweimal, d. h. 3 Monate und 2 Jahre nach der Operation mit dem Herzkatheterismus nachuntersucht war.Ende 1951 kam die damals 22 jährige Frau Emma P. in unserer Klinik zur stationären Aufnahme. Abgesehen von mehreren Anginen -mit 10 Jahren wurde eine Tonsillektomie durchgeführt -war die Anamnese leer. 7 Jahre vorher wurde erstmalig Kurzluftigkeit bemerkt, die im Laufe der folgenden Jahren langsam zunahm. Seit 1950 war die Patientin fast ganz an das Haus gebunden, sie konnte selbst die leichteste Hausarbeit nicht mehr durchführen. Die Einweisung erfolgte wegen schwerster und akuter kardialer Dekompensation.Bei der stationären Aufnahme bestand ein bedrohliches Lungenoedem mit rötlich-schaumigem Sputum, Orthopnoe und hochgradiger Cyanose bei außerordentlich schlechtem peripherem Kreislauf. Trotz einer Tachykardie von über 140 Schl./min. war die Diagnose einer Mitralstenose auf Grund des typischen Auskultationsbefundes ohne Schwierigkeiten zu stellen. Unter entsprechender cardialer Therapie ließ sich das Bild langsam beherrschen. Nachdem der Kreislauf wieder als einigermaßen kompensiert anzusehen war, wurden weitere klinische Untersuchungen durchgeführt. Röntgenologisch wies das Herz eine typische Mitralkonfiguration auf, die Herztaille war
