Резюме. Цель работы-оценка вероятности тяжелого течения заболевания и потребности в кислородной поддержке у пациентов с вирусными пневмониями, предположительно вызванными вирусом COVID-19, при поступлении пациента в организацию здравоохранения для решения вопроса о перераспределении потоков пациентов в организациях здравоохранения. Материал и методы. Было проведено динамическое наблюдения за 81 пациентом с легким и среднетяжелым течением вирусной пневмонии, а также 43 пациентами с тяжелым течением, потребовавшим кислородной поддержки и интенсивной терапии. Результаты. У пациентов с тяжелым течением вирусной пневмонии выявлены относительная и абсолютная лимфопения, относительный и абсолютный нейтрофилез со сдвигом лейкоцитарной формулы влево, повышение уровня СОЭ, гипопротеинемия, гипергликемия, повышение уровней АСТ, АЛТ, креатинфосфокиназы-МВ, ЛДГ, мочевины, креатинина, электролитные нарушения) по сравнению с пациентами с легким и среднетяжелым течением заболевания (p<0,05). Возраст пациентов с тяжелым течением заболевания 45 лет и старше (p<0,05). Кроме того, у пациентов с двусторонним поражением легких чаще наблюдалось тяжелое течение заболевания (p<0,05). На основании установленных данных была разработана шкала оценки вероятности течения вирусной пневмонии, которая позволяет спрогнозировать развитие тяжелого течения заболевания с необходимостью интенсивной терапии и продолжительной кислородной поддержки в виде инсуфляции кислорода через лицевую маску или ИВЛ в течение первых 6 суток от поступления в больницу (or-133,1, чувствительность-92,11; специфичность-91,25; p<0,0001). Заключение. Шкала оценки вероятности тяжелого течения заболевания позволит выявить тяжелых пациентов, перераспределять потоки пациентов между организациями здравоохранения, оценить необходимость свободных мест в «кислородных палатах» и/или отделении интенсивной терапии.
