Im Tierversuch und bei freiwilligen Versuchspersonen wurde die Eignung von Äthylalkohol für die parenterale Ernährung geprüft. Als wesentlichster Nachteil erwies sich die verhältnismäßig geringe Umsatzkapazität für Äthanol. Im Tierversuch war lediglich bei niedriger Dosierung (0,2 g/kg ■ h) ein Fließgleichgewicht zu erreichen. Bei Versuchspersonen gelang dies bei einer Dosierung von 0,125 g/kg · h nicht. Nach den vorliegenden Untersuchungen bestehen keine Anhaltspunkte dafür, daß Äthanol zur Entstehung einer Laktatazidose beitragen könnte. Lediglich präfinal ist bei Ratten unter Alkoholinfusion eine Erhöhung der Laktatkonzentration festzustellen, welche jedoch auch auf einen Kreislaufkollaps zurückgeführt werden könnte. Auch die Wirkung von Äthanol auf den Purinstoffwechsel scheint nicht von wesentlicher Bedeutung zu sein. Weder im Tierversuch noch bei Versuchspersonen ließen sich starker ausgeprägte Effekte nachweisen. Eine für den Stoffwechsel von Äthanol charakteristische Wirkung scheint die starke Hemmung des Galaktoseabbaus zu sein. Trotz ähnlicher Umsatzmechanismen wird bei freiwilligen Versuchspersonen der Galaktoseabbau durch gleichzeitige Verabreichung von Sorbit oder von Xylit nicht beeinträchtigt. Diese Ergebmsse würden darauf hindeuten, daβ die Reduktion von NAD im Zytoplasma der Leberparenchymzellen durch Sorbitdehydrogenase eine andere Wertigkeit hat als diejenige durch Alkoholdehydrogenase. Nach den vorliegenden Ergebnissen verursacht Alkohol keine starker ausgeprägten akuten Stoffwechseleffekte. Die geringe Umsatzkapazität sollte allerdings neben der Hepatotoxizität zur Zurückhaltung bei seiner Verwendung in der Infusionstherapie mahnen.
Bei freiwilligen Versuchspersonen und bei Versuchstieren wurde die Wirkung von Fruktose, Xylit, Sorbit und Glyzerin auf die Harnsäurebildung untersucht. Die Tierexperimente wurden unter Verwendung des Urikaseinhibitors Oxonat durchgeführt. Während vierstündiger Infusionen verursacht bei freiwilligen Versuchspersonen Xylit in hoher Dosierung (0,5–2,0 g/kg Körpergewicht pro Stunde) den intensivsten Anstieg der Harnsäurekonzentration. Fruktose und auch Sorbit sind deutlich weniger wirksam. Der Effekt dieser beiden Substanzen ist besonders ausgeprägt zu Beginn der Infusionen. 48stündige Dauerinfusionen in niedriger Dosierung (0,25 g/kg pro Stunde) haben deutlich andere Wirkungen als die hochdosierten Kurzzeitinfusionen. Die Sorbitinfusionen sind unter diesen Bedingungen ohne Effekt auf die Konzentration der Seru-harnsäure. Fruktose führt lediglich zu einem initialen Anstieg, und Xylit verursacht eine kontinuierliche Erhöhung der Harnsäurekonzentration. Bei den Tierversuchen sind zwei verschiedenartige Wirkungen abzugrenzen. Fruktose und Glyzerin verusachen bei oxonatbehandelten Ratten einen initialen raschen Anstieg der Harnsäurekonzentration in der ersten Stunde des Experiments. Erst in der dritten Stunde der Dauerinfusion erfolgt dann im Tierversuch eine weitere Erhöhung der Harnsäurekonzentration. Während der Xylitinfusionen erfolgt hingegen ein kontinuierlicher Anstieg der Harnsäurekonzentration über den gesamten Zeitraum des Versuchs. Aus diesen Ergebnissen ist abzuleiten, daβ Fruktose und Glyzerin im Tierversuch über zwei verschiedene Mechanismen die Harnsäuresynthese beeinflussen: über einen raschen Effekt und über eine Spätwirkung. Es wird für möglich gehalten, daβ die Spätwirkung von Fruktose und Glyzerin mit der Wirkung von Xylit identisch ist. Diese Ansicht wird durch die Ergebnisse der Frierstoppuntersuchungen mit Lebergewebe unterstützt. Lediglich Fruktose (und Glyzerin) verursachen bei der Ratte bei rascher Infusion eine deutliche Abnahme der Konzentration von ATP und von Adeninnukleotiden. Auch bei hochdosierten Dauerinfusionen von Fruktose kommt es wieder zu einer weitgehenden Normalisierung dieser Werte. Xylit (und auch Sorbit) verursacht hingegen nur unwesentliche Veränderungen der Adeninnukleotidkonzentrationen. Es gibt einerseits deutliche Parallelen zwischen den Wirkungen der Glukoseaustauschstoffe auf den Purinstoffwechsel im Tierversuch und beim Menschen. Es sind jedoch auch gewisse Differenzen vorhanden, welche durch weitere Untersucñungen geklärt werden soilten.
