Zusammenfassung. Erstmalig wird bei der progressiven Muskeldystrophie (PMD) Erb eine Untersuchungsmethode beschrieben, mit deren Hilfe gleichzeitig ein Enzymverlust aus Skelett-, Herzmuskel- und Leberzellen beim Typ Duchenne (Typ IIIa) aufgezeigt wird. Der Enzymverlust aus Blutzellen und glatter Muskulatur in das Blutserum kann beim Duchenne-Typ nicht ausgeschlossen werden. Als Zeichen eines Herzmuskelschadens werden angesehen: Im Serum HBDH/LDH >0,8, MK/CK >0,5, ALD/CK ≦0,3, LDH/MDH < Normwert, GOT/GPT ≧1,0, NADPH-GSSGR/NADH-GSSGR im Bereich der Norm; simultane Verschlimmerung im EKG. Als Zeichen eines Skelettmuskelschadens werden im Serum gefunden: Hohe CK-Aktivitäten, MK/CK ≦0,5, ALD/CK ≧0,3, GOT/GPT ≧1,0, LDH/MDH, HBDH/LDH, NADPH-GSSGR/NADH-GSSGR im Bereich der Norm, keine Verschlimmerung des EKG-Befundes. Befunde, die für einen Enzymaustritt aus Leberzellen sprechen, sind: Im Serum GOT/GPT <1,0, HBDH/LDH ≦0,4-0,6, NADPH-GSSGR/NADH-GSSGR <50, LDH/MDH > Normwert, erhöhte SDH-, GLDH-, NADPH- bzw. NADH-spezifische Aktivitäten, keine Verschlimmerung des EKG-Befundes. Befunde, die für einen Enzymaustritt aus Blutzellen sprechen, sind im Serum: LDH/GOT > 12,5, HBDH/LDH ≦0,8 erhöhte Aktivitäten der NADPH- und NADH-GSSGR, normale Aktivitäten von SDH und GLDH, GOT/GPT ≧1,0. Die schubweis verlaufende, in der kalten Jahreszeit sich verschlimmernde Krankheit von Skelett- und Herzmuskel sowie der schubweise Enzymaustritt aus Leber- und wahrscheinlich auch Blutzellen, kann durch drei Therapieformen (Digitoxin + Dianabol^®, K-Mg-Asparaginat, Dianabol^® + Vitamin E) weder verhütet noch deutlich gebessert werden. Ein unbehandelter Patient zeigt ähnliche Befunde der Organbeteiligung. Bei der Diskussion der Ergebnisse wird festgestellt, dass der bei der PMD postulierte, genetisch festgelegte Defekt im Elektronen- und Energietransport der dystrophischen Zelle durch obige Therapieformen weder umgangen noch repariert werden kann.
