Effizienz und Toleranz parenteral verabreichter Fettemulsionen hängen eng mit dem Schicksal der einzelnen verabreichten Fett-partikel nach Applikation in den Kreislauf zusammen. – Quantitativ im Hintergrund, jedoch beachtenswert ist die unerwünschte Phagozytose durch das RES unter Beeinträchtigung der Abwehrlage. Bei der eigentlichen »Klärung« werden noch extrazellulär Fettsäuren freigesetzt, die zum kleineren Teil verbrannt, zum gröβeren unter Energieverbrauch zu Speicherfett reverestert werden. Zur Vermeidung eines »Überladungssysndroms« wegen der leicht möglichen Sättigung dieser Mechanismen sind Dosierungshöchstgrenzen einzuhalten (30% beim normalen Erwachsenen, 50% bei Säuglingen, maximale Infusionsrate 150 mg/kg KG/h). – Die zu erwartende Effizienz parenteral verabreichter Fettemulsionen läβt sich nach Parametern des Einzelfalls in etwa abschätzen. Die wichtigsten darunter betreffen Umfang und unterschiedlich intrazellulär aktivierte Heranziehung des körpereigenen Fettgewebes im Verhältnis zu Verbrennungskapazität bzw. Verbrauch. Dadurch ergeben sich dieselben Indikationen sowohl vom Standpunkt der Verabreichung essentieller Fettsäuren als auch der Fettemulsionen als Kalorienquelle (Abbildung 7a und b).
