InleidingHashimoto-thyreoïditis, in 1912 voor de eerste maal beschreven, is een chronische auto-immuunaandoening leidend tot destructie van de schildklier. Klinisch wordt de ziekte gekenmerkt door schildklierfalen en struma. In de meeste gevallen wordt de diagnose gesteld bij klachten en verschijnselen van hypothyreoïdie, maar in sommige gevallen is er sprake van euthyreoïdie of, in de acute fase, hyperthyreoïdie of thyreotoxicose (hashitoxicose). Het aantonen van antistoffen tegen de schildklier, zoals anti-thyreoïd-peroxidase (TPO) of anti-thyreoglobuline, bevestigt de diagnose Hashimoto-thyreoïditis. Deze antistoffen kunnen overigens ook worden gevonden bij gezonde mensen. Bij een Hashimoto-thyreoïditis kan als complicatie een encefalopathie optreden. In verband met haar neurologische toestand werd patiënte geïn-tubeerd en beademd. Bij aanvullend laboratoriumonderzoek was de serumconcentratie C-reactieve proteï-ne < 1 mg/l (referentiewaarde 0-10) en het leukocytenaantal 11,9Â10 9 /l (4,0-11,0). De elektrolytwaarden waren alle normaal, en de glucoseconcentratie bedroeg 12,9 mmol/l (nuchter 4,4-5,5 mmol/l). Deze normaliseerde spontaan binnen enkele uren. Onderzoek van de liquor toonde een verhoogd totaaleiwit van 880 mg/l (100-300), zonder aanwijzingen voor een infectie. Toxicologische screening van de urine toonde geen bijzonderheden. Bij beeldvorming van de hersenen door middel van CT en later MRI werden geen afwijkingen gezien. Patiënte werd vervolgens overgeplaatst naar de kinder-intensive care van ons ziekenhuis, waar zij aankwam in gesedeerde toestand met een EMVscore van 1-4-tube zonder tekenen van lateralisatie. Haar reflexen waren inmiddels symmetrisch goed opwekbaar zonder opvallende vertraagde relaxatie. SamenvattingEen meisje van 14 jaar werd op de afdeling Spoedeisende Hulp gezien nadat zij een epileptische aanval had doorgemaakt. In de maanden daarvoor had zij enkele episoden gehad met motorische uitvalsverschijnselen en eenmaal ook een gedaald bewustzijn. Omdat zij na de epileptische aanval niet goed herstelde, werd verdere diagnostiek ingezet waarbij een primaire hypothyreoïdie ten gevolge van de ziekte van Hashimoto werd vastgesteld. Bij een Hashimoto-thyreoïditis kan een scala aan neurologische verschijnselen optreden in het kader van een encefalopathie. De exacte ontstaanswijze van een Hashimoto-encefalopathie is onbekend, al zijn er sterke aanwijzingen dat het een auto-immuunaandoening betreft die over het algemeen goed en snel reageert op een behandeling met corticosteroïden. Tijdig herkennen van dit ziektebeeld voorkomt onnodige diagnostiek naar mogelijke andere aandoeningen.
