Background: Morbidly obese patients develop more atelectasis during general anesthesia than non-obese ones, and these atelectasis
Purpose:To compare the effect of parenteral versus enteral nutritional support in severe acute pancreatitis, with respect to efficacy, safety, morbidity, mortality and length of hospitalization. Methods: The study was comprised of 31 patients, divided into a parenteral group (n=16) and an enteral group (n=15), who met severity criteria for abdominal tomography (Balthazar classes C, D, and E). The patients were compared by demographics, disease etiology, antibiotic prophylaxis, use or not of somatostatin, nutritional support, complications and disease progression. Results: There was no statistical difference in the average duration of nutritional support, somatostatin, or antibiotics in the two groups. Imipenem was the drug of choice for prophylaxis of pancreatic infections in both groups. More complications occurred in the parenteral group, although the difference was not statistically significant (p=0.10). Infectious complications, such as catheter sepsis and infections of the pancreatic tissue, were significantly more frequent in the parenteral group (p=0.006). There was no difference in average length of hospitalization in the two groups. There were three deaths in the parenteral group and none in the enteral group. Conclusion: Enteral nutritional support is associated with fewer septic complications compared to parenteral nutritional support. Key words: Nutritional Support. Parenteral Nutrition. RESUMO Objetivo:Comparar o efeito do suporte nutricional parenteral versus enteral, em pancreatite aguda grave, com relação à eficácia, à segurança, à morbi-mortalidade e ao tempo de internação. Métodos: Foram estudados 31 pacientes distribuídos em grupo parenteral (n=16), no período de 1995 a 1998 e grupo enteral (n=15), no período de 1999 a 2002, que preencheram os critérios de gravidade pela tomografia de abdome (Balthazar C,D,E). Os pacientes foram comparados quanto aos dados demográficos, etiologia, antibioticoprofilaxia, somatostatina, suporte nutricional, complicações e evolução. Resultados: A maioria dos pacientes era Balthazar E, principalmente no grupo enteral, porém sem significado estatístico (p=0,21). Também não houve diferença estatística nos dois grupos em relação ao tempo médio de uso de suporte nutricional, somatostatina e antibiótico. O imipenem foi a droga de escolha para profilaxia da infecção pancreática nos dois grupos. Houve mais complicações gerais no grupo parenteral, sem significado estatístico (p=0,10). As complicações infecciosas do tipo sépsis do cateter e infecção do tecido pancreático foram mais frequentes no grupo parenteral, com significância estatística (p=0,06). Não houve diferença na média de internação nos dois grupos. Houve três óbitos no grupo parenteral e nenhum no enteral. OBJETIVO: Analisar os principais aspectos técnicos e clínicos referentes às ressecções hepáticas realizadas em um período de cinco anos (1994 a 1998). MÉTODO: Estudo retrospectivo de 21 ressecções hepáticas, com ênfase no tipo de ressecção utilizado, o preparo pré-operatório e as complicações do período pós-operatório. Ademais, estabelecer correlações com os dados clínicos e epidemiológicos, bem como os exames complementares mais solicitados, em especial, o exame histopatológico. RESULTADOS: Foram realizadas oito ressecções locais atípicas, quatro segmentectomias, quatro hepatectomias esquerdas, duas hepatectomias direitas, uma lobectomia esquerda, uma trissegmentectomia esquerda e uma trissegmentectomia direita. As ressecções foram indicadas para tratamento de tumores malignos primários do fígado em nove pacientes; tumores benignos em seis pacientes; miscelânea em três pacientes; metástases hepáticas em dois e tumor de via biliar em um paciente. As complicações pós-operatórias ocorreram em sete pacientes (33%), sendo as mais freqüentes o abscesso subfrênico e peritonite e a mortalidade operatória foi de 9,5%. CONCLUSÕES: A cirurgia hepática tem se tornado cada vez mais factível e as complicações pós-operatórias, sob maior controle clínico, têm diminuído bastante a mortalidade.
OBJETIVO: Estudar o efeito do dreno de Penrose na cavidade peritoneal de ratos. MÉTODO: Foram selecionados 30 ratos Wistar do mesmo sexo,divididos em três grupos de 10 animais, todos submetidos à introdução e fixação de dreno (Penrose) na cavidade peritoneal. Os períodos de observação , após os quais os animais foram mortos para estudo foram: grupo I - 24 horas; grupo II - sete dias; grupo III - 14 dias. RESULTADOS: Foi observada a formação de aderências nos animais dos três grupos, sendo que no grupo I, as aderências foram tênues em seis animais, moderadas em dois e intensas em dois. No grupo II, foi observada aderência tênue em um animal, moderada em cinco e intensas em quatro. CONCLUSÕES: A presença de dreno de Penrose na cavidade peritoneal de ratos induz à formação de aderências.
Introduction: Obesity is a worldwide epidemic and people with super obesity (BMI > 50 kg/m²) have the worse prognosis, related to physical complications but also to mental illnesses, physical and verbal abuse, as well as social exclusion. Objective: To determine the profile of super obese patients treated in a referential bariatric surgery service. Methods: Retrospective analysis of 56 medical records of patients treated at the HUUFMA’s Bariatric and Metabolic Surgery Service multidisciplinary ambulatory care in 2017-2019. Incomplete records or with body mass index (BMI) < 50 kg/m² were excluded. Results are presented in mean and standard deviation. Chi-square test was used for association analysis, Shapiro-Wilks test for normality and significance was considered for p<0.05. Results: Prevalence in sample of females (80.3%), brown skin (67.8%), average age of 37.8 ± 9.8 years, and 49.1% graduated high school. The average weight and BMI were that of 136.6 ± 12.4 kg and 59 ± 6.5 kg/m², respectively, with a patient standing out with a BMI of 71 kg/m². Among the comorbidities we found obstructive sleep apnea in 100%, insomnia in 98.2%, joint problems in 53.5%, anxiety in 50%, hepatic steatosis and systemic arterial hypertension with 48.2% each, dyslipidemia in 42.8%, intestinal constipation in 32.1% and type 2 diabetes mellitus in 17.8%. There was a significant relation (p<0.05) between schooling and weight. An impact on mental health was observed, with 37.5% of the patients exhibiting depression, 44.6% binge eating and 17.6% suicidal thoughts and/or self-mutilation. Conclusion: The super obese patients’ profile is that of a young adult woman, with average schooling, various comorbidities and with significant impact on mental health.
