We report two patients with Beare-Stevenson syndrome. This syndrome presents craniosynostosis with or without clover-leaf skull, craniofacial anomalies, cutis gyrata, acanthosis nigricans, prominent umbilical stump, furrowed palms and soles, genital and anal anomalies. Both female newborn patients presented at birth with craniofacial anomalies, variable cutis gyrata in forehead and preauricular regions, prominent umbilical stump and anogenital anomalies. Furrowed palms and soles were also observed. The radiologic examination showed a cloverleaf-form craniosynostosis. Chromosomes were normal. They were born with respiratory distress and were connected to mechanical ventilation for ventilatory support. Both of them died in 50 days after birth due to secondary complications. The molecular analysis of these patients identified the mutation Tyr375Cys in the FGFR2 gene.
RESUMO -A síndrome do desfiladeiro torácico neurogênica verdadeira é entidade rara que resulta da compressão ou estiramento do tronco inferior do plexo braquial por costela cervical, banda fibrosa ou processo transverso da sétima vértebra cervical alongado. Descrevemos os casos de duas mulheres (23 e 19 anos de idade) com história de dor em membro superior direito, fraqueza e atrofia progressiva da musculatura intrínseca da mão. Estudos eletrofisiológicos mostraram diminuição da amplitude do potencial de ação muscular composto do nervo mediano e diminuição da amplitude do potencial de ação do nervo ulnar sensitivo. As velocidades de condução nervosa motora e sensitiva foram normais em ambos os casos. Eletromiografia de agulha mostrou desinervação crônica da musculatura intrínseca da mão direita de ambas as pacientes. Investigação radiológica mostrou costelas cervicais em um caso e processos transversos da sétima vértebra cervical alongados no outro. São revisados os aspectos clínicos, eletrofisiológicos e tratamento da síndrome. We describe two cases of female patients (23 and 19 years old) with pain in the right superior limb and progressive muscular weakness and atrophy of the intrinsic muscles of hand. Electrodiagnostic studies showed reduced amplitude of compound muscle action potential of median nerve and decreased amplitude of ulnar sensory nerve action potential. Motor and sensory nerve conduction velocities were normal in both patients. Needle electromyography were findings compatible with chronic denervation in the intrinsic muscles of the right hand of both patients. Radiological investigation showed cervical ribs in one case and elongated transverse process of C7 in the other. RESUMO -A ossificação do ligamento longitudinal posterior (OLLP) é causa incomum de mielopatia compressiva na população caucasiana. É relatado o caso de um paciente do sexo masculino com um quadro de paraparesia espástica, cuja investigação radiológica mostrou OLLP. O raio-X de coluna cervical mostrou imagem laminar, vertical, com densidade óssea, posterior aos corpos vertebrais, que se estendia de C2 a T1. A tomografia computadorizada (TC) e a mielotomografia mostravam OLLP causando compressão medular ântero-posterior no segmento descrito. Na ressonância magnética, observou-se área de hiperintensidade em T2 no segmento C7-T1, compatível com mielomalácia. O paciente foi submetido a laminoplastia tipo "open-door", com melhora do quadro parético dos membros inferiores. A OLLP deve entrar no diagnóstico diferencial das mielopatias cervicais, sendo facilmente diagnosticada através de radiografias e TC da coluna cervical. São revisados os aspectos clínicos e radiológicos e o tratamento da OLLP. PALAVRAS-CHAVE: ossificação do ligamento longitudinal posterior, mielopatia cervical, tomografia computadorizada, ressonância magnética.Ossification of the posterior longitudinal ligament in the cervical spine: case report ABSTRACT -Ossification of the posterior longitudinal ligament (OPLL) is an uncommon cause of compressive myelopathy in the Caucasian population. A case of spastic paraparesis in a Caucasian man whose radiological investigation showed OPLL is presented. The radiographs of the cervical spine showed a strip of bony density posterior to the vertebral bodies, extending from C2 to T1. Computerized tomography (CT) and CT myelography showed OPLL at the same level. Magnetic resonance showed an area of increased signal on T2-weighted sequences at C7-T1 level suggestive of myelomalacia. The patient underwent an open-door laminoplasty (C2 to C7) with improvement of the paraparesis. OPLL should be included in the differential diagnosis of cervical myelopathy. It can be easily detected by plain radiographs and CT of the cervical spine. RESUMO: Descreve diversas causas genéticas de infertilidade masculina, as alterações cromossômicas e moleculares que podem estar presentes em pacientes com " oligo ou azoospermia idiopática". Faz, também, uma revisão das técnicas de diagnóstico pré-implantacional disponíveis atualmente. O estudo está centrado nos aspectos genéticos do fator masculino da infertilidade de casais. Sua importância consiste no dever ético de se analisar caso a caso, quando se opta por técnicas de injeção intra-citoplasmática de espermatozóides (ICSI), porque uma significativa fração dos indivíduos com " oligo ou azoospermia idiopática", apresentam alterações genéticas em seus espermatozóides ou outras células do epitélio germinativo. A probabilidade da criança apresentar doença genética é grande. Deste modo, a ICSI não é a opção mais indicada para casais que enfrentam este problema.PALAVRAS CHAVE: Infertilidade masculina; Genética; Anormalidades dos cromossomos sexuais. INTRODUÇÃOO fator masculino na infertilidade tem se tornado cada vez mais importante na investigação de casais inférteis. Tal fato representa uma queda considerável na qualidade do esperma entre indivíduos saudáveis e jovens nos últimos anos (Auger et al. 1995). Face a esta situação, novas opções terapêuticas têm surgido, que permitem o tratamento de casos de intensa oligospermia e azoospermia.Alguns investigadores (Vogt et al. 1995;Chandley et al. 1995) observaram que certos pacientes que apresentavam "oligo ou azoospermia idiopática" tinham alterações genéticas como microdeleções que, entre outras, localizavam-se no gene Yq 11. Estudos citogenéticos não são capazes de detectá-lo, mas estudos moleculares com a técnica de Reação em Cadeia da Polimerase (PCR), sim. Além disso, a ausência congênita de vasos deferentes foi identificada em formas leves, ou formas genitais puras de fibrose cística (Anguiano et al. 1992).Já que indivíduos portadores de anormalidades genéticas foram quase sempre estéreis no passado, atualmente, utilizando-se a injeção introcitoplasmática de espermatozoides (ICSI) com espermatozóides ou espermátides obtidos através de aspiração micro-epidimal (MESA) ou extração testicular (TESE), este problema pode ser minimizado, mas há risco considerável de transmissão de defeitos genéticos à prole. Um bom exemplo são as anormalidades ligadas ao cromossomo Y que são transmitidas para descendentes do sexo masculino.Devido a este fato, há muitas discussões em relação a adoção ou não destes meios terapêuticos em defeitos espermatogênicos idiopáticos. Pode-se decidir por não utilizá-las em determinados casais, pois a saúde e o bem-estar da futura criança é que deve orientar este impasse, apesar da constante pressão de grande parte dos casais. Este problema poderá ser solucionado com o advento de novas e revolucionárias técnicas de diagnóstico molecular pré-implantacional e a identificação de genes responsáveis por doenças genéticas pelo Projeto do Genoma Humano (HUGO).
