La psicosis orgánica hace referencia a un grupo de enfermedades clasificadas en la actualidad como “trastorno psicótico debido a otra afección médica”, a diferencia de la ausencia de causalidad orgánica específica que hay en los trastornos primarios. Se trata de cuadros secundarios a procesos de larga duración, que es necesario reevaluar con frecuencia debido a la asociación entre el deterioro relacionado con la edad, la comorbilidad y la propia evolución de la psicosis. Presentamos el caso de una paciente con hemorragia subaracnoidea secundaria a ruptura de malformación arteriovenosa, que padeció de cambio leve en el comportamiento y disminución en las funciones cognitivas con posterior evolución a cuadros disociativos, episodios de hipersomnia y desconexión del medio, mediante la valoración conjunta de especialidades se llego al diagnóstico de psicosis orgánica tipo trastorno esquizofreniforme.
