ZusammenfassungDie 2009 geschlossene "Pflege-Transparenzvereinbarung ambulant" (PTVA) soll die Qualität der durch Pflegedienste erbrachten Leistungen transparent und vergleichbar darlegen sowie deren schrittweise Verbesserung gewährleisten. In diesem Artikel werden Anspruch und Realität der PTVA gegenübergestellt. Anhand eines Rechenbeispiels wird die Funktionsweise der PTVA verdeutlicht und analysiert, welche Anreizstrukturen von diesem Instrument der Qualitäts bericht erstattung (QBE) ausgehen. Zwar besitzt QBE das Potenzial zur Verbesserung der Versorgungsqualität, von den Qualitätskriterien, Bewertungssystemen und Darstellungsformen der PTVA jedoch gehen derzeit weder Impulse für Transparenz noch Anreize für pflegerische Spitzenleistungen aus. Vertreter der Zielgruppen sind in die weitere Entwicklung und Evaluation der Kriterien und Berichtsinstrumente einzubeziehen und es sind lebensnahe Formen der Daten aufbereitung zu wählen. Die PTVA ist um Exzellenzstandards der Ergebnis-und Lebens qualität zu ergänzen. Es sind diffe renzierte Bewertungsstufen einzuziehen und -für den Fall mangelhafter Versorgungsqualität -so genannte k.o.-Kriterien einzuführen. Von einer leichtfertigen Berechnung von Mittelwerten ist abzusehen. Darüber hinaus sollte eine aussagekräftige Fallzahl in die Begutachtung der Pflegedienste einbezogen werden. Schließlich sollten die Beson derheiten der ambulanten Versorgung (z. B. Wahlleistungskatalog) berücksichtigt werden.
SummaryThe German federal public reporting system on the quality of home care entiteled "Pflege-Transparenz vereinbarung ambulant" (PTVA) is meant to reveal excellence and improve performance. But does the PTVA meet these demands? In this article the German reporting system on the quality of home care is introduced and assessed using a numeric example. Incentives resulting from the PTVA are analysed. Although reports on quality may generally advance quality of healthcare, the PTVA does not evoke quality improvement. Moreover, neither criteria, nor assessment framework or display format from the PTVA motivate nursing care providers to excellence. Instead resource adjustment from people-intensive care to administrative structures is triggered. Data suggested that the recorded mean of quality assessment results improve while the quality of nursing home care may coincidentally drop (adverse effect). To improve the public reporting system users have to be involved in the development process of the PTVA. Furthermore, the PTVA should focus on parameters of outcome quality and quality of life. Finally, the instrument's scales and rating system have to be adapted as well.
