Uterine artery embolization (UAE) has been widely used for a number of years as an alternative mini-invasive method of treatment of symptomatic uterine fibroids. The technical aspects of the UFE are described in sufficient detail in various guidelines, recommendations and scientific articles. Along with low trauma and other benefits, UAE, like any other treatment, can be accompanied by some intra- and postinterventional complications. One of the intraoperative complications is uterine artery spasm. In most cases the spasm is eliminated medically and is considered to be a temporary and reverse phenomenon. Persistent spasm of the uterine artery can really prevent the adequate embolization of perifibroid arteries and subsequently cause ineffective treatment. The article provides a case of persistent spasm of the uterine artery at the final stage of its embolization for uterine leiomyoma. A set of intraoperative measures to eliminate this complication is described, which, in the end, allowed to properly complete the intervention. Таджикский научно-исследовательский институт акушерства, гинекологии и перинатологии, Душанбе, Республика Таджикистан Цель: изучение результатов внедрения новой методологии анализа критических случаев в работу акушерского стационара с целью профилактики возникновения смертельно опасных состояний в службе родовспоможения и улучшения оказания неотложной акушерской помощи. Материал и методы: проведён сравнительный анализ 20 случаев критических состояний -обменные карты, истории родов и новорождённых, карты анализа критических случаев (АКС) и интервью женщин: 14 случаев массивных акушерских кровотечений и 6 случаев тяжёлых гипертензивных нарушений с осложнениями. Результаты: в стране поддержана инициатива ВОЗ по анализу критических состояний в акушерстве, направленная на снижение материнской заболеваемости и смертности (2008). Экспертами обучены специалисты родовспоможения, в том числе из числа сотрудников ТНИИ АГиП. С использованием предложенной методологии изучен ряд критических случаев на заседании рабочей группы по АКС из числа специалистов ТНИИ АГиП. Каждая отдельная последующая сессия аудита включала в себя отчёт о реализации представленных ранее рекомендаций. Непосредственно исследование случая состояло из обсуждения доклада, интервью и выявления положительных моментов ведения пациентки, а также упущенных возможностей 20 случаев критических состояний -обменные карты, истории родов и новорождённых. Проведено изучение критических случаев, угрожающих жизни женщины в периоды беременности, родов и после родоразрешения, возникших в результате массивных акушерских кровотечений ( 14) и осложнений тяжёлой прэклампсии (6). Выяснилось, что положительными моментами оказания помощи в критических ситуациях является наличие необходимых препаратов и оборудования в учреждении, внедрение руководств и стандартов ведения акушерских кровотечений и гипертензивных осложнений, а также командная работа. Наряду с положительными моментами, выявлены упущенные возможности -недостаточное консультирование, дефекты антенатального и стационарного наблюдения, необходимость оптимизации существующих руководств в сфере неотложных состояний в акушерстве. На основании принятых решений участниками критических случаев и рабочей группой по АКС проведено 15 занятий, как для старшего, так и среднего медицинского персонала по различным аспектам оказания неотложной акушерской помощи, подготовлено 4 проекта приказов, в дальнейшем подписанных администрацией и направленных на улучшение организации работы и повышение эффективности качества оказываемых услуг; на основании полученного опыта внедрён ряд организационных мероприятий, направленных на профилактику возможных осложнений. Заключение: по результатам проведённых сессий аудита сделаны выводы относительно основных направлений, в которых деятельность должна быть улучшена: адаптирование некоторых руководств, приказов и стандартов для учреждения, создание ряда приказов, непрерывное повышение уровня знаний. отделение патологии беременности № 2, Таджикский нии акушерства, гинекологии и перинатологии, Душанбе, Республика Таджикистан В статье представлен случай благоприятного исхода беременности и родов у пациентки с большой миомой матки. Миома матки, особенно при значительных размерах, создаёт неблагоприятные условия для внутриутробного развития плода и его адекватного роста. Немаловажным фактором, помимо размеров, является её локализация относительно полости матки, места плацентации. В ходе гестационного процесса важно знать обо всех особенностях опухолевидного образования. Весьма значимой в подобного рода случаях является система перенаправления пациенток на более высокий уровень оказания перинатальной помощи, где есть необходимые условия для должного контроля за состоянием плода, характером и размерами опухоли, а также, наличие специалистов для проведения оперативного родоразрешения любой степени сложности и в любое время суток. Соответствующая терапия, качественное антенатальное наблюдение, стационарное лечение при возникших осложнениях, предоперационная госпитализация, а также оперативные вмешательства (кесарево сечение и консервативная миомэктомия) позволили сохранить беременность, сохранить орган и провести родоразрешение в доношенном сроке беременности жизнеспособным плодом.
