SUMMARY BACKGROUND: The COVID-19 pandemic has affected the entire world, posing a serious threat to human health. T cells play a critical role in the cellular immune response against viral infections. We aimed to reveal the relationship between T cell subsets and disease severity. METHODS: 40 COVID-19 patients were randomly recruited in this cross-sectional study. All cases were confirmed by quantitative RT-PCR. Patients were divided into two equivalent groups, one severe and one nonsevere. Clinical, laboratory and flow cytometric data were obtained from both clinical groups and compared. RESULTS: Lymphocyte subsets, CD4+ and CD8+ T cells, memory CD4+ T cells, memory CD8+ T cells, naive CD4+ T cells, effector memory CD4+ T cells, central memory CD4+ T cells, and CD3+CD4+ CD25+ T cells were significantly lower in severe patients. The naive T cell/CD4 + EM T cell ratio, which is an indicator of the differentiation from naive T cells to memory cells, was relatively reduced in severe disease. Peripheral CD4+CD8+ double-positive T cells were notably lower in severe presentations of the disease (median DP T cells 11.12 µL vs 1.95 µL; p< 0.001). CONCLUSIONS: As disease severity increases in COVID-19 infection, the number of T cell subsets decreases significantly. Suppression of differentiation from naive T cells to effector memory T cells is the result of severe impairment in adaptive immune functions. Peripheral CD4+CD8+ double-positive T cells were significantly reduced in severe disease presentations and may be a useful marker to predict disease severity.
HCV genotiplerinin belirlenmesi antiviral tedavi seçiminde, tedavinin planlanması ve tedaviye yanıtın öngörülmesinde yol göstericidir. Bu çalışmada retrospektif olarak HCV genotip dağılımının belirlenmesi amaçlanmıştır. Çalışmada Ocak 2015-Aralık 2017 tarihleri arasında Sakarya Üniversitesi Tıbbi Mikrobiyoloji Laboratuvarına HCV genotip tayini amacıyla gönderilen 235 hastanın serum örneği çalışma grubunu oluşturdu. Örneklerden RNA izolasyonu üretici firmanın önerileriyle otomatik izolasyon sistemi (QIAsymphony, Qiagen, Almanya) ile yapıldı. Çalışma grubu; 105 (%44,7) erkek, 130 (%55,3) kadın olmak üzere toplam 235 hastadan oluştu. Genotip dağılımları; örneklerden 200’ü (%85,1) tip 1, 2’si (%0,9) tip 2, 20’si (%8,5) tip 3, 7’si (%3,0) tip 4 ve 6’sı (%2,6) mix tip (tip 1a+ tip 1b) idi. Genotip 1 içinde 13’ü (%5,5) tip 1a, 182’si (%77,4) tip 1b iken, 5 (%2,1) örnekte alt tip belirlenemedi. Kadınlarda genotip 1b görülme sıklığı erkeklere göre anlamlı olarak yüksekti (p=0,009). Genotip 1b saptanan hastaların yaş ortalaması diğer genotiplere sahip hastalara göre anlamlı olarak yüksekti (p<0,001). Yıllara göre tip1b dağılımında anlamlı bir değişiklik saptanamadı (p=0,376). Sonuç olarak çalışmamızda; bölgemizdeki örneklerin çoğunluğunda genotip 1, alt tipin ise tip 1b olduğu belirlenmiştir. Genotip 1’den sonra en sık, genotip 3 saptanmıştır. Klinik sürecin takibi ve antiviral tedavi seçiminde genotip belirlenmesinin yol gösterici olmasından dolayı bu çalışmaların HCV tedavisine önemli katkıları olacağı düşünülmektedir.
ÖzetSülükler ile tedavi (Hirudoterapi), yüzyıllar boyunca hastalar tarafından kullanılmış, enflamatuar hastalıklar, osteoartrit veya cerrahi sonrası gelişen komplikasyonlar üzerindeki muhtemel etkileri birçok araştırmacı tarafından da incelenmiş ve denenmiştir. Sülük tedavisinin kullanılabileceği endikasyonların başında plastik ve rekonstrüktif cerrahi operasyonlarında serbest ve pediküllü flep uygulamalarında post operatif gelişen venöz yetersizlikler gelir. Özellikle serbest flep uygulamalarında venöz problemler arteryel problemlere oranla çok daha sık görülmektedir. Hirudoterapi, venöz yetersizlik tedavisinde cerrahi eksplorasyona iyi bir alternatif oluşturmaktadır. Avrupa tıbbi sülük türlerinden olan Hirudo medicinalis, birçok hekim tarafından, daha küçük olması, yüzeysel kesik meydana getirmesi ve daha az kan emmesi nedeni ile özellikle tercih edilmektedir. Sülüklerin tükürük bezlerindeki salgılarında 100'ün üzerinde farklı özelliklere sahip biyoaktif maddeler vardır. Bu salgılar vazodilatasyona, analjeziye, antienflamasyona, kanda antikoagülasyonun arttırılmasına, bakteriyel çoğalımın engellenmesine, ödemin çözülmesine, mikrosirkülasyonun hızlanmasına, organ ve dokuların hasarlı vasküler yapılardaki permeabilitenin ve canlılığın tekrar artmasına, hipoksinin düzeltilmesine, kan basıncının düşürülmesine, ağrının azaltılmasına ve immünitenin tekrar arttırmasına etkili oldukları gösterilmiştir. Tekniği ucuz, etkili ve kullanımı kolaydır. ABD İlaç ve Gıda Dairesi (Food and Drug Administration of USA-FDA) sülüklerin satışına, genel amaçlı ve plastik cerrahi ve mikrocerrahide kullanımına 2004 yılında izin vermiştir. Türkiye' de de sülük uygulaması ile beraber onaltı uygulamayı kapsayan "Geleneksel ve Tamamlayıcı Tıp Uygulamaları Yönetmeliği" 2014' de yürürlüğe girmiştir.Geleneksel tıpta uzun bir geçmişi olan sülük tedavisini uygulaması kolay, yan etkileri az ve uygun maliyettedir. Son yıllarda hem ülkemizde hem de dünyada modern tıp alanında yerini almaya başladığı görülmekte ve tedavilerde etkinliği arttırdığı görüldükçe kullanım alanları genişleyecektir. Amaç: COVID-19 hastalığında, T lenfositlerin sayısal olarak azaldığı, hem CD4+ hem de CD8+’in azalabildiği, bazen de CD8+ düzeyinin anlamlı arttığı görülmüştür. Virüse spesifik CD8+ T-hücrelerin Efektör bellek (TEM) veya RA+ Terminal diferansiye efektör bellek (TEMRA) hücrelerinin olduğu düşünülmektedir. Ancak bu hücrelerin özellikleri, özellikle SARS-CoV-2 enfeksiyonu veya COVID-19 hastalığının patogenezindeki rolleri belirsizdir. Bu araştırmada, SARS-CoV-2 PCR pozitifliği ile COVID -19 hastalığı tanısı alan çocuk hastalarda tanı sırasında yardımcı T, sitotoksik T hücreleri ve alt tiplerinde gözlenen değişiklikleri akım hücre ölçer ile irdelemeyi amaçladık. Yöntem: COVID-19 tanısı alan, 0-18 yaş arası 22 çocuğa flow sitometri ile; T Yardımcı Hücresi (TH), T Sitotoksik Hücresi (TC), T Naif Hücreleri (TN), Santral bellek (TCM), Efektör bellek (TEM), RA+ Terminal diferansiye efektör bellek (TEMRA) ve Son timik göçmen T hücreleri (RTEs) çalışılmıştır. Bulgular: T hücre sayıları tüm yaş gruplarında normal bulundu. CD4/CD8 oranı 5 yaş altı ve 16 yaş üstü grupta arttı. CD4+T hücreler içinde TCM 16 yaş üstü grupta azalırken, TEM tüm yaş gruplarında azaldı. RTEs, 16 yaş üstü grup hariç diğerlerinde azaldı. Naif CD8+T hücreleri (TN) tüm yaş gruplarında yüksek bulundu. Sonuç: Düşük sayıda saptanan CD4+ ve CD8+ lenfosit sayısı, 2019 Coronavirus hastalığında (COVID-19) ayırt edici bir laboratuvar bulgusu olarak bildirilmiştir. Yeterli sayıda naif T hücresine sahip olmak, bağışıklık sisteminin bilinmeyen patojenlere sürekli yanıt vermesi için gereklidir. Çocuklarda bu hücrelerin normalden yüksek olduğu bu çalışma tespit edilmiştir.
The presence of chronic pelvic pain disease with characteristic urinary symptoms and sexual dysfunction for at least 3 months in the last 6 months, without bacterial infection of the urinary tract, is called chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). 1 Its prevalence in the population is 8.2% (2.2-9.7%). 2 The National Institutes of Health (NIH) published the NIH Chronic Prostatitis Symptom Index (NIH-CPSI) which assesses symptoms in 3 main dimensions (pain, voiding, and quality of life) (Table 1). 2,3 The treatment of CP/CPPS may involve various medications, including alpha-blockers, tricyclic antidepressants, quercetin, antibiotics, pregabalin and sildenafil-if erectile dysfunction is present, as well as physiotherapy/pelvic floor relaxation therapies. 4 Various studies on different subgroups of CP/CPPS have not provided high-level evidence on treatment modalities, largely owing to the fact that the pathophysiology of CP/CPPS is still not fully understood. 2,4
Objective: During viral infections, antibody production of B cells are critical for protective immunity. It is known that the COVID-19 disease has a milder course in children. It is crucial to evaluate the causes of this situation from a pediatrician's perspective to determine the treatment goals of the disease. We aimed to examine the flow cytometric changes in B cells and subtypes observed in children diagnosed with the COVID-19 infection.Materials and Methods: This is a prospective cohort study including 22 children aged 0-18 who had been diagnosed with COVID-19. CD19 + B cells, CD27 -IgD + naive B, CD21 low immature B, CD21 low CD-38 low active B, CD27 -IgDdouble-negative B, CD27non-memory B, CD27 + memory B, CD27 + IgDswitched memory B, and CD27 + IgD + non-switched memory B cells were studied using flow cytometry.Results: B cells counts decreased as a percentage in the 2-5 years age group and the 10-16 age group as an absolute number. Naive and non-memory B cell frequencies increased in the 5-10 years old and over 16 years old groups. Double negative B cells were normal in all age groups. Non-memory B cells increased in the 5-10 and over 16 years old groups, whereas memory B cells decreased. In all groups, switched memory B cells decreased. Non-switched memory B cell counts were within reference ranges in all groups except for the over 16 years group. Conclusion:Although the decrease in B cell count is associated with the severity of the disease, naive B cell subgroups did not decrease in the pediatric patients included in the study. All groups showed increased switched memory B cell counts, in accordance with the literature. Unlike adults, naive B cells, non-switched memory B cells, and double-negative B cells were normal in children.
