ResumenObjetivo. Obtener datos actuales de la angioplastia coronaria (ATC) en la Argentina y factores asociados a morbimortalidad hospitalaria, en pacientes con infarto agudo de miocardio (IAM) incluidos en el registro RAdAC (Registro Argentino de Angioplastia Coronaria). Material y métodos. Desde mayo de 2010 a noviembre de 2010, 67 centros incluyeron 1.905 pacientes tratados con ATC; de éstos, 752 presentaban IAM. Todos los pacientes fueron enrolados en forma consecutiva, previa fi rma del consentimiento informado, y los datos demográfi cos, clínicos y angiográfi cos recolectados en un formulario electrónico online.
The "accordion effect" is a mechanical remodeling of the arterial wall produced by endoluminal introduction of stiff guidewires, leading to vessel wall shortening and development of multiple eccentric constrictions. This phenomenon is unresponsive to vasodilators and may lead to unnecessary stenting of the pseudo-lesions. It must be suspected with the development of multiple lesions and when a tortuous artery assumes a linear shape after advancing distally a stiff wire. In the situation of a complete occlusion of the artery the diagnosis may be challenging. Stenting in this scenario can be demanding because of the difficulty to appropriately size the length of the stent to the true lesion. We present a case of an unrecognized accordion phenomenon in an occluded coronary artery with a long dissection treated with two stents that after retrieving the wire were insufficient to cover the whole dissection.
This group of diabetic patients was older, and with a higher prevalence of 2 or more vessel disease. Cardiogenic shock on admission was the only independent predictor of in-hospital death and along with age and angiotensin-converting enzyme inhibitor use, an independent predictor of mortality during long term follow-up.
Aunque la angiografía coronaria (AC) continúa siendo el patrón oro para el diagnóstico de la enfermedad coronaria en vasos coronarios epicárdicos (VCE), la cuantifi cación de una estenosis coronaria puede ser limitada, especialmente cuando se trata de lesiones moderadas que comprometen la luz del vaso entre el 40-70%. La AC carece de la posibilidad de obtener una evaluación funcional (valoración de isquemia miocárdica) correspondiente a un territorio coronario determinado, siendo la presencia de isquemia miocárdica el factor pronóstico más importante de eventos en el seguimiento clínico alejado. Muchas veces se necesitan exámenes complementarios no invasivos para tener esta información (estudios de perfusión con SPECT o ecocardiografía de estrés) que obligan a diferir el procedimiento en el caso de una eventual revascularización. Los trabajos de Gould et al. describieron en forma experimental la relación entre la severidad anatómica de la estenosis y la resistencia (R) que ésta induce al fl ujo sanguíneo. En este modelo se acepta que hace falta una reducción mayor del 75% de la sección de corte del vaso (equivalente al 50% de estenosis por diámetro) para que se produzca una disminución del fl ujo sanguíneo coronario capaz de producir isquemia miocárdica con el esfuerzo. Estos datos se han trasladado a la práctica clínica, donde se ha generalizado el concepto de estenosis coronaria signifi cativa a aquella que compromete la luz del vaso más allá del 50% si se considera el diámetro, o más del 70% si se considera el área, en comparación con un segmento de referencia. La AC produce una silueta del vaso en estudio (luminograma), que no permite obtener detalles de defectos intraluminales o brindar información sobre las características de
