Observation : Le cas d'une patiente âgée de 19 ans, sans antécédents médicaux particuliers, présentant la dent 48 au sein de l'espace masticateur suite à une avulsion, a été décrit. Discussion : Cet événement survient surtout lorsque la dent est associée à un kyste, ou lorsque le décollement en direction linguale est important et que la dent glisse dans cet espace libre. Conclusion : Il est nécessaire de récupérer rapidement la dent afin d'éviter une infection ou sa migration plus profondément dans les tissus, ce qui entraîne-rait un geste chirurgical plus invasif.Abstract -Mandibular third molar complication: iatrogenic displacement into the pterygomandibular space. Introduction: Accidental displacement of the mandibular third molar into the pterygomandibular space is a rare surgical complication, especially when there is no underlying pathology. Observation: The case of a 19-year-old patient with no general health problems, presented a lower right third molar in the masticator space after its extraction, is reported. Discussion: The ectopic position is more often related to the presence of a cyst, or when the lingual subperiostal dissection is too important leading to a slip of the tooth into that free space. Conclusion : it is necessary to quickly retrieve it in order to avoid an infection or a migration deeper into the soft tissues, which would then require a more invasive surgical procedure. C A S C L I N I Q U E C A S C L I N I Q U Eopératoire survenue lors de l'avulsion des quatre troisièmes molaires incluses sous anesthésie locale. Ce praticien au moment de la luxation-avulsion de la dent 48 a vu la dent glisser et partir sous le lambeau en direction linguale jusque dans l'espace ptérygo-mandibulaire. Cette complication résultait donc d'un acte iatrogène et non d'une position ectopique associée à un kyste plus couramment rencontrée. Devant son impossibilité à localiser et à récupérer la dent, la décision a été prise de refermer le site. Une antibiothérapie (amoxicilline 3 grammes/jour pendant 7 jours) et une corticothérapie (prednisolone 60 milligrammes/jour pendant 3 jours) ont été mises en place, et le chirurgien-dentiste a pratiqué un bilan radiologique (orthopantomogramme, tomographie volumique à faisceau conique de la face ou CBCT -Cone Beam Computed Tomography) dans le but de déterminer la position précise de la dent. La patiente a consulté dans notre service une semaine après cet événement. L'interrogatoire n'a rapporté ni gêne ni douleur. L'examen clinique n'a pas retrouvé de limitation d'ouverture buccale/trismus, la prescription antibiotique ayant probablement permis d'éviter toute complication infectieuse.
