Despite the technological advances over the last years, the treatment of low rectal cancer (extraperitoneal) remains as a surgical challenge. Currently, the abdominoperineal resection is still the standard surgical treatment for most rectal lesions within 5 cm from the anal verge with well-known physical and psychological consequences. On the other hand, the introduction of intersphincteric resection in clinical practice has led to the possibility of sphincter preservation allied to satisfactory oncologic and functional outcomes in well selected cases. The present paper describes the technical steps of an intersphincteric resection for the treatment of extraperitoneal low rectal cancer.resumo: Apesar dos avanços tecnológicos dos últimos anos, o tratamento do câncer de reto inferior (extraperitoneal) continua sendo desafiador. Atualmente, a ressecção abdômino-perineal ainda é o tratamento padrão para lesões retais situadas a uma distância menor que 5 cm da margem anal, sendo bem conhecidas suas sequelas físicas e psicológicas. Em contrapartida, a introdução da ressecção interesfincteriana na prática clínica trouxe à tona a possibilidade da preservação anal aliada à obtenção de resultados oncológicos e funcionais satisfatórios em casos bem selecionados. O presente estudo tem como objetivo descrever a técnica operatória da ressecção interesfincteriana no tratamento do câncer de reto extraperitoneal.Palavras-chave: cirurgia; neoplasias; reto; canal anal.
RESUMO: Neste trabalho relatamos o caso de um paciente portador de síndrome de Lynch (HNPCC) que desenvolveu câncer retal metacrônico em curto intervalo de tempo após tratamento do tumor primário (câncer de cólon direito INTRODUÇÃONeoplasia maligna mais comum do tubo digestivo, o câncer colorretal (CCR) teve sua incidência aumentada nos últimos anos, principalmente em áreas consideradas de baixo risco (países em desenvolvimento). Aproximadamente 80% dos pacientes desenvolvem o câncer colorretal (CCR) de forma esporádica, enquanto que em 20% há uma suscetibilidade hereditária à neoplasia. Estima-se em cerca de meio milhão o número de óbitos por esta doença a cada ano, com tendência a queda na mortalidade nos últimos anos 1 . No Brasil, o CCR é a quinta neoplasia maligna mais diagnosticada (ocupando segundo lugar na região Sudeste) e a quarta causa de morte por câncer 2-3 . São condições hereditárias freqüentemente associadas a CCR: a polipose adenomatosa familiar (PAF) e o cân-cer colorretal hereditário não polipose (HNPCC), também denominada síndrome de Lynch [4][5] . Das formas hereditárias, o câncer colo-retal hereditário não polipose (HNPCC) é a mais comum, sendo responsável por 20 -30% destes, o que equivale a 3% a 5% de todas as neoplasias colorretais 1 . Classicamente como é conhecida, a síndrome de Lynch I e II é uma doença autossômica dominante sendo cinco vezes mais freqüente que a polipose adenomatosa familiar (PAF) 2 e é decorrente de mutações em genes responsáveis pelo reparo do DNA. O envolvimento colônico isolado ocorre na síndrome Lynch I, enquanto no subtipo II ocorre câncer colorretal associado a neoplasias em outros órgãos, como de endométrio, ová-rio, mama, estômago, intestino delgado,sistema hepatobiliar, pâncreas, ureter e de pelve renal 3 . Introduction Postoperative nausea and vomiting (PONV) is a common complication after general anesthesia. Pregabalin may reduce its incidence. The objective of this study was to evaluate the adjuvant antiemetic effect of pregabalin in the first 24 hours after surgery in patients undergoing breast reconstruction after bariatric surgery. Materials and Methods This prospective, randomized, double-blind study had 52 female patients aged between 18 and 64 years with physical status 1-2 of the American Society of Anesthesiologists who underwent breast reconstruction after bariatric surgery. The patients were divided into two groups. Patients in the control group received placebo, and those in the pregabalin group received 75 mg of pregabalin 2 hours before surgery and 75 mg 12 hours later. All patients received 4 mg dexamethasone and 4 mg ondansetron. The incidence of PONV was evaluated in the immediate postoperative period and 6 hours, 12 hours, and 24 hours later. The need for rescue doses of antiemetics and adverse events in the first 24 hours were recorded. Results The groups were homogeneous in clinical and treatment variables. There was no significant difference in the incidence of PONV over time in the control group (P = 0.71/no occurrence) or in the pregabalin group (P = 0.11/P = 0.26). There was no significant difference in the need for rescue antiemetic dose (P = 0.40) or in the incidence of adverse events (P = 0.51) between groups. Conclusion The administration of pregabalin in the first 24 hours after surgery did not significantly reduce PONV in patients undergoing breast reconstruction after bariatric surgery.
