A primeira mega onda destrutiva sobre a psiquiatria veio com a CID-10 e a DSM-IV, nos anos 1990 (cujo "abalo sísmico" se deu pouco antes, nos anos 1980, com a entrada da CID-9 e a do DSM III -R).O estrago foi avassalador, atingindo em cheio a nosografia e a psicopatologia, até então ditadas pela etimologia, pela história, pela doutrina, pelos livrostextos. Estes últimos foram varridos da face da psiquiatria, que ficou encharcada pelos catalogões CID e DSM, as "bíblias" dos psiquiatras modernos. Bumke, Bleuler, Kraepelin, Kretschmer, Jaspers, Ey, Mayer-Gross, juntamente com centenas de outros grandes, morreram afogados pela imensa onda.A segunda tsunami dá-se com a primazia do resultado do exame subsidiário sobre o exame clínico.Essa onda arrasou o raciocínio clínico. Não há mais valorização do exame direto do paciente. A emissão de pósitrons, a ressonância magnética, o expertsystem e outros assemelhados, dizimaram a semiologia psiquiátrica, vieram os psiquiatras cibernéticos de última geração, que, diante do ser humano e suas infinitas combinações bio-psicossocio-religiosoculturais, acreditam mais na especificidade e nos resultados da máquina, do que na própria intuição, sensopercepção, raciocínio lógico e inteligência.Essa onda gigante matou a soberania clínica. Agora a eficiência está na máquina... e, conseqüentemente, a deficiência, no psiquiatra.A terceira tsunami é a vinda desses imbecis (in, negação; bacillum, bastão, in bacillum, sem o bastão, o apoio da inteligência) inventários ou escalas de medição, previamente fabricados, que o psiquiatra pede para o paciente marcar com um "x", se falso ou verdadeiro ou algo que o valha.Essa onda destrói, completamente, qualquer possibilidade de arte dentro da psiquiatria. Nivela tudo pelo mais baixo grau de primariedade. Esses ditos instrumentos parecem aqueles testezinhos de revistas populares para meninas adolescentes, os quais, ao final, de 0 a 5 pontos, de 5 a 10 e de 10 a 15, dizem se o namorado gosta dela ou se é do tipo espontâneo, inibido ou atirado.O momento é delicado, pois as ondas geraram montanhas de entulho, e nelas as pragas disseminam-se com facilidade. E já tem várias que vicejam à tripaforra, por exemplo, a doença de Alzheimer, que na escumalha boiante é tida como doença senil, quando, em verdade, é pré-senil, descrita por Alois Alzheimer, em 1907. O leitor não entendeu a gravidade desse fato ou acha isso um absurdo?São as tsunamis arrasadoras de doutrina, que a quase todos leva de roldão! As três tsunamis da Psiquiatria
Prezados Editores, Lemos, com atenção, o artigo "Inimputabilidade: estudo dos internos do IPFMC", publicado na Rev Psiquiatr RS, v. 29, n. 3, set/dez 2007, páginas 286-293, e sentimos uma certa decepção: a ausência da epilepsia como diagnóstico! Impossível não haver pacientes epilépticos se o instituto recebeu criminosos que praticaram crimes contra pessoa e caíram na inimputabilidade. São 381 doentes mentais assassinos segundo a pesquisa. Dirão os articulistas: "usamos os critérios da Classificação Estatística Internacional de Doenças (CID-10)". E usaram, sim, esse critério semimédico, desvirtuador de diagnósticos, livro-texto dos psiquiatras modernos, infrutífero à psiquiatria forense de qualidade.Afirmamos, categoricamente, sem ter examinado um só paciente da pesquisa que selecionou 381 "inimputáveis que praticaram crimes contra a pessoa", que deve haver alguns que praticaram crimes com seis características dentre as nove catalogadas para crimes violentos praticados por epilépticos, a saber: 1 -ausência de motivos plausíveis; 2 -ausência de premeditação; 3 -instantaneidade da ação; 4 -ferocidade na execução; 5 -multiplicidade de golpes; 6 -ausência de dissimulação; 7 -ausência de remorso; 8 -ausência de cúmplice; 9 -amnésia ou reminiscências mnêmicas confusas sobre o crime. E se há, é imprescindível respeito aos mestres da psiquiatria forense, os quais se dedicaram profundamente ao estudo dos crimes violentos dos epilépticos, a começar por Henry Legrand du Saulle, o primeiro a descrever a lista acima, e seus seguidores, Richard von Krafft-Ebing, Henry Maudsley, Cesare Lombroso, Eugenio Tanzi, Carlo Ferrio. Entre os brasileiros, os imortais Afrânio Peixoto, Antonio Carlos Pacheco e Silva, José Alves Garcia e Hélio Gomes. Dos presentes, dirá o futuro.Talvez alguém pondere: os nomes citados são referências demasiado antigas. Independentemente disso, ainda são nomes de respaldo até que venham outros e façam livros melhores do que os tratados que deixaram, os quais guardam ensinamentos perenes para a psiquiatria forense de qualidade.A psiquiatria clínica ordinária (no sentido de vezeira, a mais praticada), dominante dos tempos atuais, tem seus preceitos fundamentados na CID, que não tem sequer uma linha de psicopatologia, e assim permite que se diga que a psiquiatria atual, totalmente carente de estudos nessa área, é ordinária (no sentido de ruim).Epi, "em cima", lepsis, "abater", epilepsis, "o diabo vem por cima e abate" o energoumenos ("possuído pelo diabo"), mal sagrado, mal demoníaco, o único que nunca mudou de nome desde que foi cunhado, que consta do Código de Hamurabi (em que se verifica a primeira referência psiquiátrico-forense da história). E não será essa decadente CID, que muda o nome das doenças a cada revisão (já são 10), que destruirá a epilepsia na psiquiatria, ainda que este catalogão de doenças tenha seduzido a maioria dos psiquiatras modernos. São apenas os tempos psiquiátricos atuais, dominados pelos fabricantes de remédios, pelos diagnósticos que interessam a esses conglomerados industriais. A...
