Male epileptic patients frequently complain of sexual dysfunction, particularly impotence and loss of libido. Epilepsy itself, antiepileptic drugs (AEDs), and psychosocial factors are believed to contribute to impaired sexuality. We studied luteinizing hormone (LH) pulsatile secretion, gonadotropin, and prolactin (PRL) responses to LH-releasing hormone (LHRH) and thyrotropin-releasing hormone (TRH) in 37 adult male epileptic patients receiving AED monotherapy who were seizure-free and had normal EEGs. Sexuality was assessed by psychological interview. Impotence was diagnosed in 8 patients (in 2 combined with loss of sexual desire). The occurrence of hyposexuality (approximately 20%) was independent of epilepsy syndrome or AED. No change in total testosterone (T) level was observed. Free T (fT) and dihydrotestosterone (DHT) levels were lower and sex hormone binding globulin (SHBG) levels were higher in epileptic subjects than in healthy controls, but a statistically significant difference was not observed between hypo- and normosexual patients. In impotent epileptic patients, estradiol (E2) levels were significantly increased as compared with those of patients with preserved sexuality and of healthy controls. The unbalanced relation between androgen and E2 levels was emphasized by decreased T/E2, fT/E2, and DHT/E2 ratios obtained in hyposexual epileptic patients. In this group, LHRH induced blunted LH peaks. No changes were noted in LH pulsatility features. These findings of higher E2 levels and of decreased LH response to LHRH administration in some epileptic patients with impaired sexuality, may suggest they have subclinical hypogonadotropic hypogonadism.
We studied the effect of alprazolam (APZ) in 12 healthy volunteers on the psychological stress-induced activation of emotion and on the pituitary-adrenal, adrenomedullary and sympathoneuronal systems. After 3 days of placebo or APZ (1 mg/day orally) administration, we examined plasma levels of adre-nocorticotropic hormone, cortisol, L-3,4 dihydroxyphenylalanine, dopamine, norepinephrine (NE), epinephrine, metanephrine, normetanephrine, homovanillic acid, vanillylmandelic acid, 3-methoxy-4-hydroxy phenylglycol, urinary levels of cortisol and catecholamines, circulatory responses and state anxiety levels in subjects undergoing psychological stress based on viewing horror, violence, danger and war film clips. Film viewing produced modest rises of state anxiety levels, of plasma NE concentration and of diastolic blood pressure in both the placebo and drug groups. APZ significantly reduced anxiety levels at the beginning of the experimental session and caused a decrease of noradrenergic and dopaminergic neurotransmitter and cortisol concentrations. Our data suggest that APZ reduced anxiety related to the expectation of the event, while the circuitry between structures responsible for anxiety and peripheral sympathoneural function was still found to be partly sensitive to film viewing.
Migration flows are part of human history. The process of globalization, if on the one hand it seems to favour the movement of human beings, on the other hand it is creating the conditions for the recovery of migratory flows, especially within some areas of the world and, in part, directed towards advanced development countries. This creates problems of acceptance on the part of the host with respect to the customs and habits of the guest. Resistance to reception, used for political ends by populist parties, has deep roots that have to do with psychological and social factors: defence mechanisms, stereotypes and prejudices. The article stresses the importance of training in overcoming these obstacles to building a society that is first multicultural and then intercultural.
The issue of leadership is one of the most topical. The focus of the research is on identifying the characteristics a person must possess to be a leader, where the term indicates anyone who has a role in coordinating and guiding a group of people. In a globalised world in which production processes are carried out by teams of people, the possibility of identifying people with leadership skills or, better still, of training people to be leaders acquires fundamental importance. The article attempts to identify the intellectual, personality, emotional and relational characteristics of a leader and suggest ways to 'educate' for leadership, highlighting the centrality of psychology in the study, identification and enhancement of these characteristics.
The ageing of the population is beginning to be an economic, social and health matter even for countries that do not belong to the advanced developed economies, but where the phenomenon can take on an impressive size that risks affecting the entire globe. It is clear that intervening downstream of the problem with forms of economic subsidy and health aid is not a winning strategy. It is necessary to work on prevention by limiting, as much as possible, the periods of non-self-sufficiency of older people that represent high health and social costs. Active ageing seems to be particularly useful in accompanying people towards a long, mostly self-sufficient and value-added old age, whereby older people can continue to be active members of the society in which they live. The system of higher education is able to perform a function of extreme importance with respect to the possibility, for the elderly population of a country, to actively live their old age: developing the activities of Life Long Learning, encouraging the birth of universities of the third age, intervene in the design, implementation and management of permanent training centres for the elderly are activities in which the university institutions can and must engage.
