Background and Purpose. Several studies have demonstrated that patients with knee injury scored within a normal range during one-legged hop tests yet showed quadriceps femoris muscle weakness with non–weight-bearing isokinetic testing. This study evaluated lower-extremity kinetics while subjects performed a single-leg vertical jump (VJ) and a lateral step-up (LSU) in an attempt to explain this phenomenon. Subjects and Methods. Using a motion analysis and force platform system, hip, knee, and ankle extension moments of 20 subjects with anterior cruciate ligament (ACL) reconstructions and 20 matched subjects were measured while they performed an LSU and a VJ. Results. An analysis of variance revealed that the knee extension moment of the ACL-reconstructed extremity was lower than that of the uninjured and matched extremities during the LSU, VJ take-off, and VJ landing. However, there was no difference in summated extension moment (hip + knee + ankle) among extremities during the LSU and VJ take-off. The summated extension moment of the ACL-reconstructed extremity during VJ landing was less than that of the uninvolved and matched extremities. Conclusions and Discussion. These results suggest that the hip or ankle extensors may compensate for the knee extension moment deficit. The decrease in summated extension moment in the ACL-reconstructed extremity during VJ landing represents inadequate attenuation of landing forces, which may expose the skeleton and joint structures to injury.

Effect of patellar taping on knee kinetics of patients with patellofemoral pain syndrome [Journal of Orthopaedic and Sports Physical Therapy 1999;29 (11):661-7] Onderzoeksopzet. Herhaalde metingen bij een groep patie¨nten met een patellofemoraal pijnsyndroom.Doel. Nagaan van het effect van het tapen volgens McConnell op de verticale spronghoogte en het moment en de kracht van de knie-extensie van het aangedane been tijdens recht omhoog springen en zijdelings op een verhoging stappen.Achtergrond. Tapen van de patella wordt door therapeuten toegepast ten einde de functie van de knie-extensoren te maximaliseren tijdens sportactiviteiten en bij revalidatie van patie¨nten met een patellofemoraal pijnsyndroom. Er zijn geen studies gevonden waarin het effect is onderzocht van tapen van de patella op de spronghoogte en het moment en de kracht van de knieextensoren tijdens een maximale verticale hoogtesprong of zijdelings op een verhoging stappen.Methoden en metingen. Veertien vrouwen (van gemiddeld 24,4 ± 5,8 jaar oud) met eenzijdige patellofemorale pijn voerden met e´e´n been een verticale sprong en een zijdelingse opstap uit, bij vier testsituaties: 1 aangedane been met patella-tape, 2 aangedane been met placebotape, 3 aangedane been zonder tape en 4 niet-aangedane been. Bij elke testsituatie werd het maximale moment van de knie-extensoren, de kniekracht en de verticale spronghoogte gemeten met behulp van een krachtenplatform en een bewegingsanalysesysteem.Resultaten. Variantieanalyse en post hoc-analyses brachten een belangrijk effect ten aanzien van de knieconditie aan het licht. De testsituatie met de patella-tape resulteerde in een significant (p < 0,01) groter extensorenmoment en een grotere kracht van de knie dan zonder tape en met de placebo-tape:1. met patella-tape: moment 1,57 ± 0,32 N·m/kg en kracht 3,47 ± 0,67 W/kg; 2. zonder tape: 1,31 ± 0,39 N·m/kg en 2,79 ± 1,21 W/ kg; 3. met placebo-tape: 1,33 ± 0,30 N·m/kg en 2,70 ± 0,99 W/kg.Additionele analyses toonden dat de verticale sprongkracht in het niet-aangedane been significant groter was dan in het been met de patella-tape, met de placebo-tape en zonder tape:1. niet-aangedane been: 25,69 ± 2,66 cm; 2. aangedane been met patella-tape: 23,33 ± 4,22 cm; 3. aangedane been met placebo-tape: 23,08 ± 4,20 cm; 4. aangedane been zonder tape: 23,45 ± 4,54 cm.De verticale spronghoogte met de patella-tape verschilde niet van die met de placebo-tape of van die zonder tape.Conclusie. Deze resultaten suggereren dat de patellatape het knie-extensorenmoment en de kracht tijdens gewichtdragende activiteiten kan verbeteren, vergeleken met de situatie zonder tape.

In een poging dit fenomeen te verklaren evalueerden wij de kinematica van de onderste extremiteit, terwijl de proefpersonen een eenbenige verticale sprong (VS) en een zijdelingse opstap (ZOS) uitvoerden.Proefpersonen en methoden. Met behulp van een bewegingsanalysesysteem en een krachtenplatform werden bij 20 proefpersonen met een voorste-kruisband(VKB)-reconstructie en 20 vergelijkbare proefpersonen de extensiekrachten van heup, knie en enkel gemeten terwijl zij een ZOS en een VS-sprong uitvoerden.Resultaten. Een variantieanalyse bracht aan het licht dat de knie-extensiekracht van de extremiteit met de gereconstrueerde VKB kleiner was dan die van de nietaangedane en controle-extremiteiten tijdens de ZOS, de VSafzet en de VS-landing. De gesummeerde extensiekracht van de extremiteit met de VKB-reconstructie was tijdens de VS-landing minder dan die bij de niet-aangedane en controle-extremiteiten.Beschouwing en conclusie. De onderzoeksresultaten suggereren dat de heup-of enkelextensoren mogelijk het tekort aan knie-extensiekracht compenseren. De afname van de gesummeerde extensiekracht in de extremiteit met de VKB-reconstructie tijdens de VS-landing is een uiting van inadequate afzwakking van de landingskrachten, die een blessurerisico voor de gewrichtsstructuren met zich meebrengt. BeschouwingVoorzover ons bekend is er slechts e´e´n onderzoek waarin de kinetica en kinematica werden onderzocht tijdens functionele activiteiten van personen met een VKB-reconstructie. Daarom is het moeilijk om onze bevindingen te vergelijken met die van onderzoekers die andere metingen hebben gebruikt. Onze bevinding van een verminderde knie-extensiekracht tijdens alle drie testactiviteiten lijkt op het eerste gezicht te wijten aan spierzwakte van de m. quadriceps femoris. Zo was de gemiddelde knie-extensiekracht tijden de VS-landing 1,30 N·m/kg voor de proefpersonen met een VKB-reconstructie, terwijl die voor de controlepersonen 2,09 N·m/kg was.Bij belaste activiteiten van de onderste extremiteit kan echter het neuromusculaire systeem ook de heupextensoren en de m. soleus inschakelen om de knie-extensie te ondersteunen. Het is dus mogelijk dat de afname in knieextensiekracht die we vonden, niet alleen te wijten was aan zwakte van de m. quadriceps femoris maar ook aan het inschakelen van andere spiergroepen, naast de m. quadriceps femoris, bij het strekken van de knie (d.w.z. de heupextensoren en/of de m. soleus).
