Dermatomyositis (DM) is an autoimmune connective tissue disorder with characteristic skin changes and involvement of different organs and systems, such as muscle, cardiovascular, respiratory and gastrointestinal systems. This condition is characterized by the presence of a myopathic syndrome with bilateral and symmetric weakness predominantly of extensor, proximal muscle groups, respiratory muscles and esophageal involvement, accompanied by characteristic skin disorders. The worldwide incidence is reported at 1 to 6 cases per million inhabitants; predominates in women, with 2.5:1 ratio; and has a bimodal distribution with two peaks, one from 5 to 14 years old and another from 45 to 64 years. The amyopathic form is present in 2 to 18% of all cases of dermatomyositis. Diagnosis is supported by alteration in muscle enzymes, as well as characteristic findings in magnetic resonance imaging (MRI), electromyography (EMG) and autoantibodies. The definitive diagnosis is determined by the histopathological study of both, skin biopsy and affected muscles. The purpose of this case is to review the diagnostic approach from a dermatological point of view, where characteristic lesions gave way to a thorough physical examination looking for signs of a myopathic syndrome; and where despite the elevation of muscle enzymes, findings in electromyography and MRI compatible with myositis, the muscle biopsy was negative. It was the punch biopsy that corroborated the diagnosis, even before the report of anti-MDA-5 antibodies (melanoma differentiation associated Gen 5) that has been linked to a characteristic phenotype of DM that has a higher prevalence of amyopathic disease and a worse prognosis.
Se desconocen los factores asociados a prurito urémico grave y el impacto sobre la calidad del sueño en pacientes con enfermedad renal crónica (ERC) en terapia de reemplazo renal (TRR) en México. Objetivo: Correlacionar características clínicas y bioquímicas con la gravedad del prurito urémico en pacientes con ERC en TRR. Material y métodos: Estudio descriptivo, transversal en el Hospital General de México de agosto a septiembre de 2018. Incluimos variables demográficas, bioquímicas y características del prurito urémico cuatro semanas previas a la evaluación. Se realizó estadística descriptiva y regresión logística binaria multivariada para identificar factores de riesgo para prurito grave, definido con escala visual análoga (EVA) > 6 puntos. Resultados: Se incluyeron 99 pacientes, 60.6% varones, edad promedio 44.9 ± 4.2. Variables asociadas a prurito grave: edad (42.4 vs 49.7, p = 0.021), IMC (23.8 vs 25.9 kg/m 2 , p = 0.019). Los factores asociados a prurito grave obtenidos en el análisis de regresión multivariado fueron IMC ≥ 24 (OR = 3.37 [IC95% 1.008-11.29, p = 0.049]), prurito diseminado (OR = 17.2 [IC95% 1.039-286.6, p = 0.047]), despertares (OR = 10.9 [IC95% 2.21-53.87, p = 0.003]). No se observó asociación con variables bioquímicas ni modalidad de TRR. Conclusiones: El prurito urémico grave tuvo asociación con mayor IMC, edad, presentación diseminada y despertares.
El mieloma múltiple (MM) es una enfermedad maligna de células plasmáticas de linfocitos; B. Bluefarb estimó que aproximadamente el 15% de los pacientes con MM desarrollarán alguna forma de amiloidosis. El término amiloidosis hace referencia a la acumulación extracelular de un material amiloide, debido a un mal plegamiento de las proteínas autólogas. El subtipo de amiloidosis de cadenas ligeras lambda (AL) es el más común en amiloidosis cutánea asociada con MM. En México, la frecuencia de amiloidosis cutánea en la consulta dermatológica es de aproximadamente 0.5% y predomina en mujeres. La infiltración de tejido por estos depósitos de amiloide se produce de forma localizada o sistémica. Cuando la amiloidosis afecta la piel, puede ser como una manifestación de amiloidosis sistémica, o una amiloidosis cutánea localizada. Las enfermedades amiloides cutáneas pueden tener manifestaciones heterogéneas, dependiendo de la ubicación del depósito amiloide dentro de la dermis o la epidermis. El diagnóstico de amiloidosis se realiza histológicamente mediante la demostración del depósito de amiloide. El tratamiento de la amiloidosis va enfocado a cada tipo. Los pacientes con mieloma asociado con amiloidosis AL con afectación de la piel tienen el peor pronóstico.
