Heart failure is a pathological condition with a poor prognosis, caused not only by heart disease, but it is also considered as the final stage of various diseases. An elderly causes natural changes that contribute to reduced heart performance. Despite the innovative methods of treatment, the number of these patients is growing rapidly. Experts predict that one out of five people will manifest symptoms of heart failure at some point in their lives, so heart failure should be perceived as a 21st century's epidemic with a component not only health but also economic. Rehospitalisation’s due to exacerbations of the disease are a serious problem, and each subsequent one worsens the quality of life of these patients, including palliative care. Aim. The aim of this study was to assess the quality of life of patients with heart failure in the cardiology ward and second internal department of the Provincial Specialist Hospital in Włocławek and the health clinic MEDICOR PLUS – Cardiologists in Włocławek. Material and methods. 105 patients with heart failure participated in the study and it was carried out by means of a diagnostic survey. As a research tool, a standardized questionnaire was used, in the Polish language version, concerning health EQ-5D-3L and a self- constructions questionnaire covering sociodemographic characteristics of the research group and questions enabling the assessment of health behaviour and analysis of the willingness to improve the respondents quality of life. The obtained data were subjected to statistical analysis. Results. There hospitalization number significantly correlate with the quality of life parameters, which support the thesis of the study. The study did not show a significant relationship between the compliance and hospitalizations due to exacerbation of heart failure, which does not support the thesis of the study. The study did not show a statistically significant relationship between the reduced number of hospitalizations and knowledge about heart failure and proper health behaviours. Conclusions. As many as 75% of the patients with heart failure show high interest in the medical institution's activities, associations working for the quality of life improvement and the nurse advices, which support the thesis of the study.
Patient death is an unavoidable element in the work of nurses. It is associated with experiencing various emotions which affects their value system and personal life. The aim. To examine the attitudes of nurses towards dying patients, to present their emotions, anxiety and experiences related to patients' deaths and to assess the impact on the personal life of the respondents. Methods and research material. The results of own research based on the questionnaire as well as the standardized Mini-COPE tool. Results. Gender, marital status and education do not differentiate the attitudes of nurses towards the dying and death of the patient. On the other hand, age and seniority turned out to be a factor that significantly differentiated the change in the hierarchy of values after the patient's death and had a positive effect on the level of stress. Conclusions. Summing up this work, it should be noted that the contact of nurses with dying patients exposes them to stress, negative emotions and high psychological burden. Their hierarchy of values changed after the patient's death. The ability to sympathize and proper preparation of nurses has a great impact on ensuring the reduction of suffering and the patient's dying.
Streszczenie: Wstęp. Jednoczesne spożywanie leków i alkoholu może prowadzić do wielu interakcji, niekiedy niebezpiecznych dla zdrowia i życia. Dotyczą także one leków dostępnych w aptece bez recepty, takich jak leki przeciwbólowe i przeciwzapalne, antyalergiczne, antybiotyki czy leki na niestrawność i zgagę. W Polsce 89% Polaków przyjmuje leki dostępne bez recepty. Z kolei 72% dorosłych osób w Polsce pije napoje alkoholowe. Cel. Celem badań była analiza wiedzy studentów kierunku pielęgniarstwo na temat możliwych reakcji wybranych leków z alkoholem. Materiał i metody. Badania przeprowadzono w 2019 roku wśród 66 studentów kierunku pielęgniarstwo. Wykorzystano metodę sondażu diagnostycznego, technikę ankietowania oraz autorski kwestionariusz ankiety. Wyniki. Zdecydowana większość studentów pielęgniarstwa nie dysponuje pełną wiedzą na temat możliwych interakcji popularnych leków z alkoholem. Wnioski. Studenci kierunku pielęgniarstwa powinni pogłębiać swoją wiedzę na temat interakcji, które mogą wystąpić w przypadku spożywania alkoholu i leków. Pozwoli to na ich profesjonalny udział w farmakoterapii pacjentów i prowadzenie wobec nich działań z zakresu edukacji zdrowotnej.
Introduction. In modern therapy of patients with atrial fibrillation, which belongs to the group particularly at risk of stroke, prophylactic anticoagulation is of primary importance. The initiation and subsequent proper monitoring of long-term anticoagulation therapy is very important for this group of patients. The use of oral anticoagulants significantly reduces the risk of ischemic stroke, and thus ensures longer survival and avoidance of permanent disability among patients with atrial fibrillation. Aim. Assessment of the level of knowledge of patients with atrial fibrillation on thromboprophylaxis in internal departments and cardiology of the Provin-cial Specialist Hospital in Włocławek. Materials and methods. The study group consisted of 100 people (63 men and 37 women) with diagnosed atrial fibrillation, taking oral anticoagulants. The research tool was the author's questionnaire. To assess the risk of ischem-ic stroke in this group of patients, the CHA2DS2-VASc scale was used and ques-tions about these factors were included in the survey. Results. Studies show that anticoagulant prophylaxis in the form of oral anti-coagulants is used in every patient with diagnosed atrial fibrillation. Most pa-tients use new generation drugs (NOAC) that do not require dose adjustment based on a normalized INR. Persistent atrial fibrillation was found in 51% of the patients. The most numerous group were patients in the 65-74 age range. The annual risk of stroke in my subjects was on average 11%. In most cases, patients correctly answered questions about the rules of conduct when using oral anticoagulants, factors that increase and weaken the effects of these drugs and the symptoms of their overdose. There were no significant differences in the level of knowledge between the women and men studied, while younger, better educated patients who declared a very good or satisfactory material situ-ation showed more knowledge. Conclusions. The study showed that the level of patients' knowledge about antithrombotic prophylaxis in atrial fibrillation is at a good level and corresponds to current medical knowledge.
Wstęp. Jednym z powszechnie występujących nowotworów złośliwych jest rak jelita grubego, który w około 33% przypadków obejmuje swoim zakresem odbytnicę. Rak jelita grubego jest odpowiedzialny za 10% wszystkich chorób nowotworowych, a obserwowane tendencje wskazują na dynamikę wzrostu zachorowalności na ten rodzaj nowotworu. Najczęściej stosowaną metodą leczenia nowotworu odbytnicy we wczesnych stadiach zaawansowania jest operacja. Objawem zarówno choroby nowotworowej jak i konsekwencji leczenia operacyjnego, stanowiąc często nieodłączny element jest ból. Dlatego też dla oceny efektywności likwidacji pooperacyjnych dolegliwości bólowych niezbędne staje się rozpoznanie ich specyfiki oraz rodzaju aktywności podejmowanej przez personel medyczny i pacjentów. Cel. Celem pracy była ocena skuteczności leczenia dolegliwości bólowych, u pacjentów w pierwszej dobie po przednich resekcjach odbytnicy z powodu nowotworu złośliwego. Materiał i metody. Badania zostały przeprowadzone w Centrum Onkologii im. Prof. Franciszka Łukaszczyka w Bydgoszczy, na Oddziale Klinicznym Chirurgii Onkologicznej wśród 100 pacjentów, którzy zakwalifikowani zostali do zabiegu operacyjnego w obrębie odbytnicy. Badanie było anonimowe i dobrowolne. Do badań wykorzystano polską wersję zmodyfikowanej skali HADS-M. Do oceny natężenia bólu używano skali numerycznej - NRS. Jako narzędzie badawcze zastosowano kwestionariusz ankiety własnej, zawierający pytania z zakresu oceny dolegliwości. W ankiecie zawarto również informacje dotyczące charakterystyki socjodemograficznej respondentów. Otrzymane wyniki poddano analizie statystycznej testem niezależności chi-kwadrat Pearsona, przyjęty poziom istotności statystycznej to p<0,05. Wyniki. Przed operacją najwięcej badanych w spoczynku odczuwało łagodny ból (47%), godzinę po operacji najwięcej odczuwało umiarkowany ból (47%), 4 godziny po operacji najwięcej odczuwało łagodny ból (43%) i również 24 godziny po operacji najwięcej odczuwało łagodny ból (49%), ale jednocześnie u 36% ból nie występował w ogóle. Test Chi-kwadrat wykazał istotne statystycznie zwiększenie bólu godzinę po operacji w porównaniu ze stanem przed operacją, oraz istotne statystycznie zmniejszenie bólu 4 godziny po operacji w porównaniu z okresem godzinę po operacji oraz istotne statystycznie zmniejszenie bólu 24 godziny po operacji w porównaniu ze stanem 4 godziny po operacji. Nieco ponad połowa respondentów (58%) deklaruje, że otrzymywane leki całkowicie zniosły ból. Na podstawie przeprowadzonych badań stwierdzono, że aż 4 na 5 badanych (79%) deklaruje, że dolegliwości bólowe występowały najczęściej w okolicy rany pooperacyjnej. Blisko połowa respondentów (49%) uważa, że ból po usunięciu drenu zmniejszył się. Większość respondentów (72%) zaobserwowała u siebie zwiększenie dolegliwości bólowych podczas uruchamiania po operacji. Najwięcej badanych osób charakteryzowało doświadczany przez siebie ból jako okresowy (27%) lub kłujący (26%). Wnioski. Pacjenci odczuwają istotnie mniejszy ból po zastosowanym leczeniu. Po zabiegu odczuli zmniejszenie dolegliwości bólowych. Najczęściej odczuwane dolegliwości występowały w obrębie rany pooperacyjnej. Usunięcie drenu zmniejszyło dolegliwości bólowe. Najczęściej pacjenci w trakcie uruchamiania odczuwali ból okresowy.
