The aim of this paper is to systematically review the literature to estimate the overall prevalence of xerostomia/hyposalivation in epidemiological studies. An electronic search was carried out up to February 2018 with no language restrictions. A total of 5760 titles were screened and just twenty-nine papers were included in review and the meta-analysis after a two independently reviewers applied the selection criteria. Data were extracted from PubMed and Web of Science databases. Eligibility criteria included original investigations from observational population-based studies that reported the prevalence of xerostomia or data that allowed the calculation of prevalence of xerostomia and/or hyposalivation. Studies conducted in samples with specific health conditions, literature reviews, case reports and anthropological studies, as conferences or comments were excluded. Sample size, geographic location of the study, study design, age of the studied population, diagnosis methods, and evaluation criteria used to determine xerostomia e/or hyposalivation were extracted for meta-analysis and meta-regression. Multivariate meta-regression analysis was performed to explore heterogeneity among studies. The overall estimated prevalence of dry mouth was 22.0% (95%CI 17.0-26.0%). Higher prevalence of xerostomia was observed in studies conducted only with elderly people. Despite diverse approaches to the condition’s measurement, just over one in four people suffer from xerostomia, with higher rates observed among older people. Moreover, the measurement methods used currently may over- or underestimate xerostomia. These findings highlight the need for further work on existing and new clinical measure and will be useful to determine which one is more reliable in clinical and epidemiological perspectives.
Objectives: To perform a systematic review with meta-analysis to answer the question: is the cervical vertebrae maturation index (CVMI) effective to replace hand-wrist radiograph (gold standard) in determining the pubertal growth spurt in patients undergoing bone growth? Methods: A search in three databases was performed, in which studies were selected that compared one of the two main assessment methods for cervical vertebrae (Hassel B, Farman AG. Skeletal maturation evaluation using cervical vertebrae. Am J Orthod Dentofacial Orthop 1995; 107: 58-66, or Baccetti T, Franchi L, McNamara JA Jr. An improved version of the cervical vertebral maturation (CVM) method for the assessment of mandibular growth. Angle Orthod 2002; 72: 316-23) to a carpal assessment method. The main methodological data from each of the texts were collected and tabulated after. Later, the meta-analysis of the correlation coefficients obtained was performed. Results: 19 articles were selected from an initial 206 articles collected. Regardless of the method used, the results of the meta-analysis showed that every article selected presented a positive correlation between skeletal maturation assessment performed by cervical vertebrae and carpal methods, with discrepancy of values between genders indicating higher correlation for the female gender (0.925; 0.878) than for the male (0.879; 0.842). When the assessment was performed without gender separation, correlation was significant (0.592; 0.688) but lower in the cases when genders were separated. Conclusions: With the results of this meta-analysis, it is safe to affirm that both CVMIs used in the present study are reliable to replace the hand-wrist radiograph in predicting the pubertal growth spurt, considering that the highest values were found in female samples, especially in the method by Hassel and Farman.
Discussão e conclusões: Este diagnóstico representa uma raridade dentro dos carcinomas metastáticos, pois nem o teci-do glandular nem o palato são frequentemente afetados. As-sim, é importante colocar em discussão todos os diagnósticos diferenciais. Um exame clínico e anamnese detalhados repre-sentam ferramentas essenciais na formulação do diagnóstico inicial. A análise imunohistoquímica pode ser um recurso adi-cional no diagnóstico definitivo. Deste modo, a cooperação en-tre o médico oncologista e o médico dentista é necessária para um tratamento holístico e deteção precoce de manifestações sistémicas e metástases à distância destes pacientes. Introdução: Os diastemas correspondem a um espaço pre-sente entre dois dentes contíguos, não havendo ponto de con-tacto entre os mesmos. Apresentam elevado impacto a nível estético, principalmente quando presentes no segundo sex-tante, particularmente entre os incisivos centrais superiores. No que concerne ao tratamento, o mesmo pode passar pelo tratamento ortodôntico, reabilitação com recurso a restaura-ções diretas e indiretas, ou a conjugação de ambos. A escolha do tratamento ideal depende de diversos fatores, tais como a dimensão dos dentes em causa, tamanho do diastema, posição dos dentes e expectativas do paciente. Descrição do caso clínico: Paciente do género masculino, 25 anos de idade, compareceu na consulta de Dentisteria Opera-tória, na Clínica Dentária Universitária de Viseu, com preocupa-ções estéticas em relação ao seu sorriso. Apresentava diastema interincisivo (dentes 1.1 e 2.1), e pretendia que este fosse fecha-do através de um tratamento rápido e económico. Após análise clínica do caso e da sua iconografia foi apresentada a opção do fecho do diastema através de restaurações diretas com resina composta. Foram realizadas impressões e modelos de trabalho, fotografias e enceramento com base na anatomia geral dentária e na obtenção de uma correta proporção dentária. Confeciona-da a matriz de silicone foi realizado o mock-up. Após ajustes ao enceramento, foi aplicado o isolamento absoluto e a restauração foi realizada com sistema HRI Micerium. Em consulta de con-trolo foi melhorado o acabamento e polimento. Discussão e conclusões: A utilização de resina composta no fecho de diastemas pode ser uma solução viável e com bons resultados estéticos e funcionais. A eficácia do tratamento vai depender de vários fatores, sendo os principais a capacidade adesiva, o tamanho do diastema, bem como o tamanho original dos dentes em causa. Para uma excelente adesividade é necessário realizar um protocolo adesivo correto, sendo im-prescindível a presença de isolamento absoluto para evitar a presença de fluidos que possam comprometer a adesividade da resina. O tratamento foi realizado com sucesso, apresen-tando bons resultados estéticos pela mimetização da cor natural dos dentes através da resina.
