Entsprechend einer Untersuchung von Madar und Widmer[15] besteht bei 1,5% der Erwachsenen in Europa zu irgendeinem Zeitpunkt ihres Lebens ein venöses Ulkus. Nach Kappert [10] sind diese Geschwüre von der Genese her überwiegend als Endzustand nach tiefer Venenthrombose (TVT) aufzufassen. Dabei ist zu beachten, daß die TVT häufig asymptomatisch verläuft und 50 -60% der Femoralvenen-und bis zu 95% der Unterschenkelvenenthrombosen somit unerkannt bleiben [19]. Nach einer Schätzung von Partsch [28] muß davon ausgegangen werden, daß es in 30% der Fälle mit konservativ behandelter TVT nach etwa 5 Jahren zu mäßig-bis mittelgradigen postthrombotischen Veränderungen kommt, die Frequenz eines Ulcus cruris wird mit 5 -8% vermutet.Die grundlegenden makrozirkulatorischen Voraussetzungen für das Zustandekommen der chronisch venösen Insuffizienz sind gekennzeichnet durch die Präsenz einer (eher seltenen) Restobstruktion, einer progredienten Zunahme der valvulären Inkompetenz in der betroffenen Strombahn mit nachfolgendem Reflux sowie der daraus resultierenden Fehlfunktion der Wadenmuskelpumpe.Infolge dieser prograden Strömungsinsuffizienz der tiefen Venen kommt es auch über die Aktivierung insbesondere der Cockett-Perforans-Venen zu einer pathologischen Kommunikation des tiefen mit dem epifaszialen Gefäßsy-stem und letztendlich zur Ausbildung sekundärer Varizen mit persistierendem venösen Hypertonus im Bereich des Endorgans Haut/Subkutis.Obwohl der genaue Schuldanteil im Einzelfall häufig umstritten bleibt, wird heute mehrheitlich die Klappeninsuffizienz als Hauptfaktor für die Entstehung der schweren Staseveränderungen (Hyperpigmentation, Ulkus) angesehen [11,20,29], während die meist zentrale Obstruktion vorwiegend für die Claudicatio venosa verantwortlich gemacht wird.Die grundlegenden gefäßchirurgischen Techniken zur Behandlung einer valvulären Inkompetenz bei schweren Formen des postthrombotischen Syndroms datieren bereits aus den 60er Jahren. Kistner [12] dokumentierte 1968 die erste direkte Reparatur an einer Venenklappe (Valvuloplastik).
