As cerebelites agudas surgem geralmente num contexto infeccioso, pós-infeccioso ou após vacinação. Etiologias mais raras incluem a tóxica. A apresentação clínica é habitualmente benigna; no entanto, casos graves, com compressão do tronco cerebral, podem apresentar-se por coma. A propósito de três casos clínicos pediátricos, os autores fazem uma breve revisão desta entidade clínicoimagiológica, dando particular ênfase às suas causas e achados imagiológicos. Casos Clínicos: Apresentam-se três casos pediátricos de cerebelite aguda – pós-viral, após intoxicação aguda com monóxido de carbono e após ingestão acidental de metadona. Discussão: Discutem-se os mecanismos etiopatogénicos na base das alterações imagiológicas encontradas em cada um dos três casos apresentados. Conclusão: Um exame de imagem confirma o diagnóstico de cerebelite e é importante na monitorização dos casos graves. A ressonância magnética (RM) é mais sensível do que a tomografia computorizada (TC) na avaliação de alterações do parênquima cerebeloso.
