A 55-year-old man with recent-onset nonspecific chest pain, on evaluation showed an anterior mediastinal mass suggestive of thymolipoma on radiological investigations ( Fig. 1A-C). A huge 30 cm  22 cm  12 cm fatty mass, weighing 5120 g was excised by sternotomy ( Fig. 2). Histopathology revealed pleomorphic thymoliposarcoma.
