Resumen: La constante evolución de la Implantología en los últimos años, ha logrado que el implante oseointegrado, sea un medio de anclaje eficaz y seguro, para las prótesis buco-maxilo-faciales. Los altos índices de éxito, que surgen de los estudios clínicos, confirman que los implantes oseointegrados son el tratamiento de elección para determinados pacientes 1 . El objetivo de este trabajo es la presentación de un caso clínico, en el que se instalaron implantes orales, para anclaje de una prótesis orbitaria. El paciente fue atendido en la Facultad de Odontología de la Universidad de la República, en el Servicio de Prótesis Buco Maxilo Facial, conjuntamente con el Departamento de Implantología Oral y MaxiloFacial.Palabras clave: prótesis buco-maxillo-facial, implantes dentales, prótesis orbitaria, implantes extraorales. Fecha de recibido: 20.06.2017 Fecha de aceptado: 03.08.2017 Introducción Las investigaciones relacionadas al uso de la oseointegración como anclaje para las Prótesis Buco-Maxilo-Faciales (PBMF), han mostrado un incremento significativo en la calidad de vida de los pacientes. Los implantes óseointegrados constituyen una alternativa previsible para la rehabilitación extra-oral de pacientes mutilados faciales por trauma, cáncer o enfermedades congénitas. La planificación del tratamiento, debe ser realizada por un equipo multi e interdisciplinario. A pesar del desarrollo de los tratamientos reconstructivos quirúrgicos, la rehabilitación protésica sigue siendo el tratamiento de elección para un gran número de pacientes. La literatura muestra un alto porcentaje de éxito de los implantes extra-orales, aunque menor, al de los implantes intra-orales. Esta diferencia podría deberse a las condiciones locales del tejido blando peri-implantario (1) . Selección del pacienteConsideraciones generales: Según J. Wolfaardt (2) , los principales beneficios de los tratamientos de prótesis Buco Maxilo Facial implanto-asistida, sobre cirugías reconstructivas son: Menor tiempo de duración de la cirugía; Menor riesgo quirúrgico; El acto quirúrgico puede ser realizado con anestesia local; Resultados más previsibles que con injertos autógenos; No existe sitio donante; Menor morbilidad; Permite el monitoreo del sitio de resección del tumor para diagnosticar y tratar en forma precoz posibles recidivas.Consideraciones en pacientes irradiados. Deberán ser seleccionados cuidadosamente, porque tienen estadísticamente menor índice de éxito, que los pacientes no irradiados (3) . La radioterapia provoca cambios primarios y secundarios en los tejidos duros y blandos. Los efectos secundarios van a depender de la intensidad y del tipo de radiación. Se recomienda que toda cirugía sea realizada durante un intervalo mínimo de seis meses después de la radioterapia (4) . El uso de oxigenación hiperbárica está recomendada por algunos autores. IntroducciónLa utilización de implantes dentales con la finalidad de sustituir dientes perdidos es, a largo plazo, un tratamiento eficaz y seguro, dada la alta predecibilidad de la oseointegración. Actualmente los implantes se colocan donde la rehabilitación lo exija. En todos los casos la disponibilidad ósea, es uno de los motivos de preocupación del clínico al momento de la colocación de los implantes. Las diferentes zonas de ambos maxilares presentan sus propios desafíos para la colocación ideal del implante. La presencia de hueso en altura y ancho suficiente, así como también su calidad, son fundamentales para la correcta inserción de los implantes, capaces de soportar una prótesis dental.Al presente taller le correspondió evaluar la zona posterior maxilar. El maxilar posterior tiene naturalmente baja altura del proceso alveolar, anatómicamente limitada por el seno maxilar. Las raíces de los molares están muy relacionadas con el piso del seno maxilar. En el centro de la cresta alveolar, se encuentra la región de trifurcación radicular, sitio ideal desde punto de vista protético para la instalación del implante, y donde se encuentra la menor altura de hueso del piso del seno. Lo mencionado hace, que la instalación de implantes de dimensiones estándar después de la pérdida de dientes en ésta región, sea limitada. Los procesos de resorción resultantes de dicha pérdida, común a todas las regiones del proceso alveolar pueden agravar esta situación. La neumatización del seno maxilar, debido al aumento de la actividad osteoclástica a lo largo de la membrana sinusal y la ausencia de la "barrera" dentaria a los cambios de presiones aéreas sinusales puede afectar la morfología de este sitio. Estas condiciones determinan un desafío anatómico para el tratamiento del maxilar edéntulo posterior utilizando implantes dentales.Las técnicas de reconstrucción de los defectos del reborde alveolar residual con procedimientos de injerto de hueso autógeno tienen resultados predecibles, que se describen en la literatura. Una explicación de sus buenos resultados es el hecho de no tener reacciones inmunes que puedan interferir con el proceso de remodelación y desarrollo del tejido. Tienen intrínsecamente a los osteoblastos y los factores de crecimiento responsables de los cambios celulares que se producen después de la implantación y que culminan en la remodelación y la formación de hueso en ese sitio 1 . Su desventaja, es la necesidad de un área quirúrgica adicional donante que puede ser de ubicación intraoral, en los casos en que la necesidad de reconstrucción sea pequeña, o extraoral en los casos de medianas o grandes reconstrucciones. En la década de 1980, se han descrito técnicas de injerto de hueso en el seno maxilar con el fin de resolver el problema de la falta de hueso y permitir la colocación del implante. The aim of this study was to compare the use of autologous bone from tuberosity (TUBER) and deproteinized bovine bone mineral (DBBM) in immediate implants with buccal bone defects. A total of 31 patients with one single tooth in the upper anterior region indicated for extraction presenting tomographic buccal bone defect were analyzed. Immediate implantation was conducted for all patients. In one group, DBBM and a collagen membrane were inserted into the buccal defect; in the other group, a small block of bone from tuberosity was used. The primary outcome was facial-palatal ridge thickness (FPT) measured in casts 1 year after function. The implant success rate was 100% in both groups. FPT changes were <0.5 mm and did not differ significantly between groups. FPT reductions in the DBBM and TUBER groups were 1% and 0.6%, respectively, at the gingival margin and 5% and 2%, respectively, at 6 mm apical of the gingival margin (p > 0.05). No significant differences were observed between groups for patient's esthetic, satisfaction, pain and quality of life. Pink esthetic scores for the DBBM and TUBER were 11.5±1.7 and 10.8±1.9, respectively (p=0.37). It can be concluded that DBBM and TUBER did not differ in terms of ridge alterations, peri-implant clinical parameters and patient-reported outcomes.
ResumenEl tratamiento del maxilar superior edéntulo presenta frecuentemente limitaciones de disponibilidad ósea en el sector posterior. Los procedimientos de elevación sinusal adquieren una relevancia notoria para su resolución. La utilización de hueso mineral bovino desproteinizado como oseoconductor está ampliamente documentada. El presente estudio de boca dividida comparó en 20 pacientes la formación ósea en senos elevados, utilizando aleatoriamente, partículas S (0,25-1 mm) vs. partículas L (1-2 mm). El análisis histomorfométrico de las biopsias obtenidas de los sitios a implantar mostró para el grupo S un 42.6 % de nuevo hueso, 42.5% de tejidos no mineralizados y 14.4 % de partículas. El grupo L exhibió un 47.2%, 38.3% y 13.7%, respectivamente. Al aplicar la prueba de los rangos con signo de Wilcoxon se observó un valor P=0.1454 (significación 0,5), no existiendo diferencia estadísticamente significativa entre ambos grupos. No existió rotura de membrana atribuible a la osteotomía produciéndose en dos casos durante el decolamiento. La tasa de supervivencia de los implantes fue del 100% en 1 año. Introducción y antecedentesEl tratamiento implanto-protésico del maxilar superior edéntulo se ve frecuentemente limitado por la ausencia de disponibilidad ósea en el sector posterior, así como por la alta prevalencia de hueso de baja densidad (1) . La pérdida ósea posterior está condicionada por los procesos patológicos asociados a las piezas dentarias que afectan al hueso alveolar, así como por la utilización de técnicas traumáticas de exodoncia y por la neumatización del seno maxilar expresado en prolongaciones alveolares del seno favorecidas por la pérdida de piezas dentarias (2) . Diferentes estrategias son utilizadas para resolver estas situaciones que dificultan o imposibilitan la colocación de implantes posteriores. Entre ellas se describen: la utilización de implantes cortos subantrales (3,4) implantes vestibularizados o palatinizados cuando existe disponibilidad ósea en las paredes respectivas (5) , la colocación de implantes postantrales de anclaje pterigoideo (6) y la recuperación del volumen óseo tanto en la apófisis alveolar como en el aspecto intrasinusal. Las técnicas de aumento óseointrasinusal, conocidas como elevación sinusal, alcanzan una relevancia notoria por su alto impacto en las posibilidades terapéuticas de las situaciones antes descritas, destacándose como ventaja la posibilidad de colocar los implantes en el sitio y con el eje exacto que la prótesis requiere. Se han propuesto diversos abordajes para incrementar la altura subantral disponible (7) , destacándose las técnicas de abordaje crestal, como las desarrolladas por Summers (8) ,Gosci (9) y Trombelli (10) , así como las técnicas de abordaje lateral desarrollados por Salagaray y Losada en 1980 y Tatum en 1986 (11) en uno y dos tiempos quirúrgicos presentando cada una de ellas indicaciones específicas.
